    Warm staff, serious safety concerns

I found the staff often warm, caring and upbeat, with good rehab, lots of activities and a welcoming, clean facility. However I also encountered/witnessed serious problems - poor communication, unresponsiveness, inconsistent professionalism, cleanliness and safety issues, missed medications/theft and reports of staff misconduct. I like the people there, but I would be very cautious about placing a loved one here without careful oversight.

    Summary review

Overall sentiment is highly mixed and polarized: multiple reviewers strongly praise Haven of Meadowbrook for its caring, upbeat staff, active social life, attractive facility, and solid rehab services, while other reviewers report severe and alarming incidents including neglect, theft, medication mismanagement, and staff misconduct. The divergence in experiences suggests inconsistent care and variable performance across shifts, teams, or time periods rather than uniformly positive or negative operations.

Care quality and resident safety: Reviews describe a wide spectrum of care outcomes. On the positive side, many families and residents report "amazingly caring" staff who are patient, attentive, and willing to make things happen, with staff morale described as positive and residents appearing to enjoy life. Conversely, some reviewers reported very serious problems: allegations of neglect and abuse by staff, personal belongings stolen and drawers ransacked, medication taken and not returned, and even claims of staff sexual misconduct. There is also at least one report of a severe UTI/infection scare. These are major red flags that point to potential lapses in supervision, policies, or enforcement of resident protections.

Staff and management: Staff reviews are similarly split. Numerous comments praise individual employees and teams — social workers, nursing aides, and therapy staff receive specific compliments for being helpful, smiling, and creating a warm environment. Several reviewers noted the facility is small and staff "work well together," contributing to a family-like atmosphere. At the same time, other reviewers call staff unprofessional, uninformative, inattentive, or unresponsive. Multiple critiques call for more nursing education and better awareness of resident rights and dignity, suggesting a need for additional training, clearer accountability, and improved communication from management. Reports of repeated operational "shutdowns," poor communication, canceled appointments, and unreliable or dirty transport further indicate management and logistical problems.

Facilities, cleanliness, and amenities: Many reviewers describe the facility as beautiful, clean, and well-equipped — private showers and baths, in-room TVs, and pleasant common areas are noted. Activity programming is a clear strength for many: karaoke, live music, outings, holiday events like Thanksgiving dinners, and other social opportunities are mentioned often as contributors to resident quality of life. In contrast, a subset of reviews describe filthy rooms and a dirty transport van, indicating inconsistent housekeeping standards. There are also contractual/placement concerns; for example, some families were charged for a private room but say their loved one was placed in a shared space.

Rehabilitation and services: Physical therapy and rehab options are positively mentioned, with at least one reviewer indicating a strong rehab program that could justify future move-in decisions. Social services staff receive multiple compliments and appear to be a strength for navigating care plans and resident needs.

Patterns, risks, and recommendations: The most significant pattern is wide variability — some residents thrive and praise the staff and activities, while others report traumatic experiences that affect safety and trust. The presence of severe allegations (abuse, sexual misconduct, theft, medication issues) even if reported by a minority of reviewers, are critical concerns that should prompt verification before placement. Prospective residents and families should conduct thorough due diligence: visit multiple times at different hours, ask for written policies on medication management, incident reporting, background checks, staff training records, and resident rights protections, confirm room type and charges in the contract, review state inspection and deficiency reports, and speak to current families and the social worker. Given the mix of high praise and serious complaints, the facility may provide excellent care in many cases, but there is documented risk of poor outcomes that warrants careful investigation and ongoing oversight if choosing to place a loved one here.
