    Clean facility but inconsistent staff

I placed a family member here and have mixed feelings. The building is clean, warm and well-kept with lovely rooms, great therapy, social activities and many CNAs who truly go above and beyond - compassionate, attentive care that restored mobility and brought peace of mind. That said, staffing and management are inconsistent: occasional rude or unavailable staff, payroll and communication problems, safety lapses and troubling incidents mean you must watch care closely. Overall it can be excellent with the right team, but do your research and stay involved.

    Summary review

Overall impression: Reviews of Lincolnshire Place of Decatur are strongly mixed, with a large number of highly positive accounts praising the staff and facility, alongside several serious negative reports raising safety and management concerns. Many families report compassionate, attentive caregiving, successful rehabilitation outcomes, effective memory care, and a clean, attractive environment. At the same time, there are multiple accounts describing neglect, poor communication, alleged discrimination, threats of retaliation, and safety incidents that should not be overlooked. The result is a polarized picture: for many residents and families Lincolnshire Place is a loving, therapeutic home; for others it has presented significant and possibly dangerous lapses in care or leadership.

Care quality and clinical services: A frequent theme is strong direct-care performance by CNAs, aides, and therapy staff. Numerous reviews describe CNAs and therapists who helped residents regain mobility, improve eating, gain weight, and recover use of walkers — with specific praise for physical and occupational therapy programs and responsive one-on-one assistance. Memory care receives repeated commendation for an affectionate, respectful approach: staff are described as dignified, hands-on (feeding when needed, holding hands), and effective at engaging residents. Conversely, several reviews describe neglectful events (a resident found soiled with urine, missing glasses, walker not placed nearby), unreported injuries, hospitalizations for pain, and at least one report of a death that reviewers associate with inadequate care. These safety-related complaints are serious and appear in multiple summaries, indicating an inconsistent standard of clinical oversight.

Staff, culture, and morale: Many families describe a warm, family-like atmosphere and caregivers who "go the extra mile," are kind, and create peace of mind. Reviewers repeatedly note individual employees who are compassionate, communicative, and trusted. However, reports also indicate variability among staff — some are excellent while others fall short — and complaints that staffing levels are sometimes inadequate. Payroll problems (late paychecks by 2–4 weeks) and managerial issues have been reported; such issues can reduce morale and may contribute to inconsistent care. There are also alarming allegations involving staff disrespect toward a resident, alleged racial bias by a director, and threats of eviction or retaliation when families complain, which raise concerns about the culture of accountability and fairness in leadership.

Management, communication, and transparency: Opinions about leadership are mixed. Several reviewers praise an "open-door" policy by the Executive Director and Director of Nursing and describe accessible, caring administrators. Others describe poor communication, lack of transparency around incidents, unhelpful attitudes from management (a specific manager named in reviews), and fear of retaliation or legal threats when raising concerns. The presence of both accessible leadership and reports of secrecy or distrust suggests uneven management practices or variable experiences depending on who is involved. The safety-related accusations and reports that injuries were unreported or mishandled underscore the importance of verifying incident reporting practices and regulatory compliance.

Facilities, dining, and activities: The physical environment is a clear strength for many reviewers: the community is described as beautiful, very clean, with pleasant smells, attractive apartments, courtyard and nature views, and private rooms. Social life is described positively, with group activities, dining as an opportunity to socialize, and multiple common areas/TV locations. Dining reviews are mostly favorable — the kitchen is characterized as accommodating and meals often match health needs — but several reviewers note inconsistent food quality or temperature and a concern that menus may be carb-heavy for diabetic residents. A minority mentioned unpleasant odors (marijuana) and unkept rooms or missing supplies, which contrast with the many comments about cleanliness.

Key patterns and cautionary points: The most important pattern is the coexistence of many strong, positive caregiving accounts with a set of recurring, serious negative reports. Positive themes center on excellent direct care, effective therapy, loving memory care, and an appealing physical environment. Negative themes of greatest concern are safety lapses (falls, unreported injuries, hospitalizations), neglectful incidents, alleged discriminatory behavior and retaliation, inconsistent leadership, and operational problems (short staffing, payroll delays). These negative reports are not isolated to trivial complaints — they include events that potentially impact resident safety and family trust.

Recommendation for prospective families: Given the polarized reviews, prospective families should perform focused due diligence. Visit multiple times (including nights/weekends), observe staffing levels and resident engagement, tour memory care specifically, ask for details on incident reporting and recent grievance resolution, request information on staff turnover and payroll stability, review the community’s state inspection reports and any deficiency citations, and speak with current families about both positive experiences and any concerns. If a prospective resident has diabetes or special dietary needs, inquire specifically about menu options and carbohydrate management. Finally, ask how leadership handles complaints and whether there are formal anti-retaliation and nondiscrimination policies in place.

Bottom line: Lincolnshire Place of Decatur clearly provides excellent care and a loving environment for many residents — particularly in areas of rehabilitation and memory care — and the facility’s atmosphere and staff are praised repeatedly. However, multiple reviewers reported severe concerns about safety, management transparency, and staff conduct that merit careful investigation. Families who are considering this community should balance the many positive testimonials with the serious negative reports and verify current practices and oversight before making a placement decision.
