    Inadequate care causes tragic outcomes

My experience was mixed but deeply troubling. Inadequate medical staffing and monitoring (no 24-hour respiratory staff, no heart-rate monitoring or proper ventilator for a trach, infrequent/on-call checks) led to delayed emergency response and, in my case, severe injury and my mother's death. Communication and billing were awful - long hold times, unreturned calls, misapplied payments, and even alleged theft of money/clothes. At the same time many nurses, CNAs and doctors were caring and competent, the facility was very clean, meals were good and activities available. I cannot recommend this place unless you can guarantee the specific medical support your loved one needs.

    Summary review

The reviews of Arcadia Care at Toulon present a strongly mixed picture that clusters into two broad experiences: many families and residents praise the staff and daily living environment, while several reports raise serious clinical and administrative concerns that warrant careful scrutiny.

On the positive side, multiple reviewers consistently describe direct-care staff — nurses, CNAs, and some doctors on-site — as caring, friendly, and competent. Several family members say their loved ones are well cared for and describe a family-like atmosphere. The facility repeatedly receives praise for cleanliness and well-kept common areas; rooms are described as adequate, and the main area is comfortable. Dining is often noted as a strength, with hearty, well-portioned meals and desserts that residents enjoy. There are onsite activities such as walking, coloring, and crafts, and the Alzheimer/Dementia unit is explicitly commended by some reviewers. Outdoor space is another plus: a fenced-in back area allows safe outdoor time for residents.

However, a number of reviews report very serious clinical failures. Several allegations involve lack of appropriate monitoring and specialized care for high-acuity residents — specifically no 24-hour respiratory staff, absence of heart-rate monitoring, inadequate ventilator capabilities for tracheostomy patients, and infrequent checks for nonverbal or medically complex residents. Some reviewers link these shortcomings to catastrophic outcomes, including severe injury and the death of a resident; at least two summaries explicitly state negligence related to feeding tubes or respiratory care and warn others to avoid the facility. These claims are among the most alarming patterns and indicate that Arcadia Care at Toulon may not be consistently equipped or staffed to safely manage residents with advanced medical needs.

Administrative and communication issues are another recurring theme. Multiple reviews describe poor customer service around billing: misapplied payments, bank/payment errors, unreturned calls, and an apparent emphasis on collecting payments over addressing family concerns. Reviewers also report long hold times and difficulty contacting staff, and some note negative attitudes from administrative personnel. Staffing shortages are mentioned intermittently; while direct-care staff are often praised, shortages apparently impact services at times. Additionally, several reviewers cite the need for facility updates — furnishings, remodeling, and updated medical equipment — which may affect the environment and clinical capacity.

Safety and security concerns appear in several summaries: incidents of wandering, limited supervision in hallways described as crowded, and allegations of theft of resident money and clothing. These items, combined with reports of delayed emergency response, contribute to a pattern of operational risk that families should consider carefully.

Taken together, the reviews portray Arcadia Care at Toulon as a facility with many strengths in day-to-day caregiving, cleanliness, and nutrition, especially for residents with standard assisted living or dementia-level needs. At the same time, there are multiple, serious reports that suggest the facility may be inconsistent in managing residents with complex medical or respiratory needs and in handling administrative/financial matters. Recommendations based on these patterns: any prospective resident or family should (1) verify clinical capabilities and staffing ratios for specific medical needs (ask specifically about 24/7 respiratory coverage, monitoring practices, and ventilator capability), (2) review incident and state survey records, (3) inquire about security measures and procedures for wandering and missing items, (4) get billing practices and payment dispute processes in writing, and (5) observe staffing and activities during a tour to confirm consistency between positive and negative reports. The overall sentiment is mixed — many families are very satisfied, but the gravity of several complaints makes it essential to do targeted due diligence before choosing this facility for medically complex residents.

    About Arcadia Care at Toulon

Toulon Rehab & Health Care Center is a large skilled nursing facility offering both short-term rehabilitation and long-term care services. Corporate-owned and for-profit, this center is equipped with 136 beds and serves residents seeking support through both Medicare and Medicaid programs. While it is not a continuing care retirement community, the center accommodates a wide array of care needs, from short-term rehabilitation after hospital stays to more extensive, ongoing support for those needing assistance with daily activities and medical care.

The approach at Toulon Rehab & Health Care Center encompasses medical and non-medical needs by providing an average of 2 hours and 31 minutes of nurse staffing per resident each day. The consistency of staffing is commendable, maintaining a strong presence on weekends as well as weekdays, which aids in the continuity of care. Residents receive regular annual influenza vaccinations at a high rate, providing an extra layer of protection for the community. The emphasis on enabling residents to sustain their ability for self-care means that a significant portion of the population is able to maintain independence in moving, eating, and other daily activities.

Short-term rehabilitation at the center aims to assist those recovering from hospitalizations due to stroke, heart attack, infection, accidental injury, or surgeries. Residents participating in this program receive specialized rehabilitative services when necessary. More than half of those discharged from rehabilitation are able to return home after their stay, which is an important marker of success for post-acute care. Emergency room visit rates are closely monitored, with about 23.8% of short-term residents requiring such services, while management continues to focus on reducing falls and other adverse events during a resident's stay.

Long-term residents at Toulon Rehab & Health Care Center benefit from a care model that balances medical oversight with support for daily living. Registered nurses and a team of health professionals collaborate on comprehensive care plans tailored to each resident's medical history, preferences, and goals. The facility provides an environment where policies and procedures are in place for abuse prevention, infection control, and the safe use of feeding tubes when medically necessary and agreed to by residents. Laboratory services and appropriate treatments are delivered according to professional standards, with ongoing attention paid to timely and quality care.

The health and safety of all residents remain a priority, with regular inspections of food preparation, medication management, accident prevention, and maintenance of a safe, comfortable living environment. While occasional infractions have been noted in state inspections—such as ensuring prompt care plan development, safeguarding personal information, and delivering specialized rehabilitation services—these concerns are consistently addressed and resolved. The absence of recent fines or denials of payment reflects a commitment to ongoing improvement and responsiveness.

Toulon Rehab & Health Care Center emphasizes the dignity, rights, and preferences of each individual resident throughout their stay. The care environment is shaped not only by professional oversight and regulatory standards, but also by a philosophy centered on individualized attention, respect, and a safe, welcoming atmosphere. Residents and their loved ones can expect a comprehensive approach to care that adapts to changing circumstances, maximizes independence, and prioritizes both physical well-being and quality of life.
