    Caring staff clean supportive environment

I was very pleased with Countryside Meadows. The nursing and rehab teams were excellent, staff (RNs, CNAs, therapists and NP) were caring and proactive, and they coordinated appointments and communicated with our family regularly. The memory-care unit and common areas were clean, homey and active, rooms were spacious, and overall I felt supported and would recommend it.

    Summary of reviews

Countryside Meadows elicits a broad range of experiences. Many accounts highlight strong clinical strengths: nursing teams, advanced-practice clinicians, and hospice providers are frequently described as attentive and proactive when engaged. The facility’s therapy department (PT/OT/speech) receives consistent praise for rehabilitation outcomes and measurable progress, and several families credit therapists with meaningful functional gains. Social services and admissions staff are often identified as helpful with transitions, insurance navigation, and discharge planning. The memory-care unit and some common areas are characterized as clean and well organized, and the facility supports family involvement through a generally open visiting policy.

At the same time, there are recurring operational concerns. Staffing levels appear uneven, with particular strain on weekends and during off-hours; these gaps correlate with reports of delayed responses to call lights, missed or delayed personal care (including bathing and linen changes), and variable aide availability. Cleanliness and sanitation are inconsistent across rooms and units, producing odor concerns in some areas and issues with bedding and room upkeep in others. Reviewers raised clinical-safety questions ranging from medication and insulin management to oxygen and wound-care coordination; these items suggest opportunities to strengthen clinical protocols and oversight.

Dining and daily routines are another area of mixed experience. While a portion of families praise dietary staff and specific menus (including a well-regarded chef in one unit), others cite inconsistent meal temperatures, periods when the kitchen appeared closed, and difficulty meeting special dietary needs. Activity programming is described as engaging in parts of the facility (bingo, socials, pet therapy), but other units — particularly some long-term or memory areas — are described as having limited or infrequent programming.

Communication and management patterns are uneven. Positive narratives emphasize open, transparent communication from particular nurses, social workers, or admissions staff; negative narratives describe delayed notifications about clinical changes, insufficient family updates, and frustration with administrative responses to complaints. Several reports raise concerns about handling of personal belongings and laundry; one or more accounts allege mishandling or disappearance of items. There are also financial and billing complaints, along with perceptions that leadership responsiveness varies by shift and by which manager is involved.

In summary, Countryside Meadows demonstrates clear strengths in therapy/rehab, supportive clinical staff on many shifts, and certain clean, well-run units (notably memory care). Prospective residents and families should weigh those strengths against documented variability in staffing, cleanliness, meal service continuity, clinical-safety controls, and administrative follow-through. When considering placement, families may benefit from direct, documented conversations about expected staffing levels (including weekend coverage), medication and oxygen protocols, meal accommodations, laundry and belongings procedures, and an escalation pathway for unresolved concerns. A targeted tour of the specific unit and conversations with current families on that unit can help clarify whether the operational consistency meets a prospective resident’s needs.

    About Countryside Meadows

Countryside Meadows offers several levels of care, including Assisted Living, Skilled Nursing, Memory Care, Long-Term Care, Respite Care, and Hospice services, so people can get help when they need it most and stay in one place even if their needs change, and this includes special programs like Moving Forward Rehabilitation and the unique Auguste's Cottage for memory care, where people with dementia live in a secured, homelike setting. Residents live in private or semi-private rooms that come with air conditioning, cable TV, Wi-Fi, emergency response systems, and some have refrigerators and views of landscaped grounds, while the whole place is wheelchair accessible and has a calm, homey feel. There are several common areas to sit and visit, along with a family lounge, dedicated dining rooms including spaces just for those with dementia, and outdoor courtyards and gardens where families can meet or residents can relax. The staff, who are trained in senior care, provide help with daily needs like bathing and dressing, manage medicines, and take care of many health conditions including diabetes and incontinence, and there's a nurse on-site part time alongside a full care team. Meals are served three times a day in community dining spaces using quality ingredients, and guest meals are available. Residents can join in on arts and crafts, music, games, educational programs, and religious or devotional services right on-site, plus there's both indoor and outdoor recreation spaces for social time or quiet moments. The facility provides scheduled transportation for errands or doctor visits, housekeeping and linen services, and both guest and resident parking. For recovery, there is physical therapy and a full rehab department, and the "Road to Recovery" and therapy programs help people regain skills after surgery or illness, with skilled staff setting plans within 72 hours of admission. The memory care team supports residents using structured, person-centered programs that are wellness-focused and based on research, plus the Meaningful Day program gives those with memory loss a set routine and safe environment. Families may use short-term Respite Care or comprehensive Hospice Care, both with medical and spiritual support, letting caregivers get a break or helping loved ones stay comfortable and supported at the end of life. Countryside Meadows keeps a quiet, community feel where people help each other, and the atmosphere stays clean and welcoming. Offsite activities let residents get out and about when they want, and partnerships with nearby health services and hospice ensure extra help is close by. Staff aim to bring peace of mind by focusing on personalized care that fits each resident's needs in a place that truly tries to feel like home.
