    Friendly staff, strong rehab, inconsistent

I found the staff warm, friendly and hardworking - therapy (PT/OT) and many nurses were excellent, and activities/administration were welcoming. But care was inconsistent: slow call-light response, nighttime safety lapses, medication/laundry mixups and at least one serious incident left me worried. Meals, housekeeping and maintenance were hit-or-miss (odors, pests, unfinished repairs, missing clothes) and my room-change requests were ignored. Overall a very friendly place with strong rehab, but I'd be cautious and tour thoroughly before deciding.

    About Heritage Park

Heritage Park sits in Fort Wayne, Indiana, and offers a range of care options for seniors, so folks find independent living, assisted living, garden homes, long-term care, respite, nursing home rehabilitation, outpatient therapy, and a secure memory care center all in the same place, which really saves a lot of worry for families. The staff includes licensed nurses, specially-trained aides, and an interdisciplinary care team that works together in a comfortable, home-like setting with single-story design to keep everybody safe and sound, and they welcome pets and offer private apartments, some with full kitchens, laundry, and big living rooms, or folks can join others in clean, shared community spaces, family lounges, courtyards, and even a TV lobby and theater if someone wants to watch a movie. There's a full activities calendar, a theater room, and a dining program that lets residents pick restaurant-style meals throughout the day, and the living fees often include 30 meals a month in independent living, so that takes some stress out of planning.

Heritage Park gives help to people who need respite care for short-term breaks or while recovering from surgery or illness, and caregivers can step away knowing their loved one gets good support whether the stay's only for a few days or if it goes longer, and specialists are on hand to walk folks through Medicaid applications and answer questions about payments, which helps make the whole process a bit easier. The facility is open 24/7, so help's never far off, and residents press emergency buttons if they need sudden care. There's a full continuum of care, so someone can move into independent living, then transition to assisted living, memory care, or long-term care when needs change. Nurses and aides monitor health and offer things like wound care, fall prevention, wheelchair help, walking programs, and therapy screenings, always focusing on keeping residents safe and as independent as possible, and therapists work on speech, mobility, and daily skills to keep everyone active and involved.

The memory care center has its own area designed around safety, dignity, and a research-based program called Auguste's Cottage, where specialists use memory-enhancing games, activities, and a dementia-supportive space, so people dealing with Alzheimer's or related conditions have the chance for an engaging and secure daily life. The facility uses programs like "Meaningful Day" and "THRIVE Wellness" that fill days with art, music, plants, visiting pets, and exercises to strengthen mind and body, and the staff try to keep things familiar and comforting by encouraging residents to bring personal treasures to decorate their rooms.

For those needing extra care after surgery or illness, Heritage Park runs a "Road to Recovery" program where families, patients, and the care team meet within 72 hours of arrival to set recovery and discharge goals, and they go through therapy and rehab until it's safe to head home again, with home safety checks and custom outpatient therapy plans if more support's needed. The team also works with local hospice providers for people needing comfort and dignity at the end of life, with spiritual, emotional, social, and medical support from staff, clergy, and volunteers.

The staff at Heritage Park are known for treating seniors with kindness and compassion, often becoming like a second family, and the building and gardens stay tidy and welcoming. Services include free transportation, all utilities except phone, emergency response, and lots of amenities like washers, dryers, a beauty shop, maintenance, and housekeeping. Heritage Park keeps active on social media with a Facebook page where folks can see what's happening day to day, and residents have a voice through the council and testimonial programs. The location puts residents close to hospitals, shopping, and restaurants-plus quick access to I-69-which is convenient for outings and emergencies. The staff work to meet each person's needs, whether someone's looking for independence, support, therapy, or just a comfortable place with caring faces around every corner.
