    Clean facility, caring staff, improving

I visited and have seen clear improvement since the ownership change. The building is clean (no urine odor), staff and nurses are caring and hardworking, rehab and food are great, and renovations/Wi-Fi/cable are underway in a pleasant country setting. I'd recommend it - I plan to visit again, though I'm aware there were complaints under prior management.

    Summary review

The reviews for Aperion Care Greenfield reflect a broad mix of strongly positive experiences and serious negative complaints, producing an overall mixed but improving picture. A large number of reviewers praise the staff as friendly, caring, hardworking, and respectful; many specifically commend the rehab team, nurses, and named employees (Shelly, Ian, and administrator Jennifer). Multiple reviewers say the facility is clean and well maintained, note ongoing renovations and updates, and describe a pleasant country setting with amenities such as Wi‑Fi, cable TV, and an outdoor supervised smoking area. Several reviews highlight good food and engaging activities, and a number of family members strongly recommend the community or report that their loved ones thrived and were treated very well.

Conversely, a significant subset of reviews describe troubling conditions and lapses in care. Several reviewers report strong odors of urine or feces, dirty bed sheets, full trash cans, and generally unclean floors or conditions. Some accounts describe inadequate personal hygiene care, lack of attention to residents, and improper medication practices. There are also allegations of staff miscommunication and unprofessional behavior — ranging from rudeness to more serious accusations such as theft of patient funds and alleged misrepresentation about access to accounts or transfers to Aperion. These allegations are serious and recur in a few summaries, contributing to a perception of inconsistent oversight in some cases.

A recurring theme is change over time. Multiple reviewers explicitly note improvement since an ownership change or since renovations and management efforts began. Several comments contrast prior negative impressions under previous management with more recent positive reports about cleanliness, staff attitude, and facility appearance. This suggests a trajectory toward remediation and investment in the building and services, supported by praise for visible leadership and an engaged administrator.

Areas of consistent strength are staffing and rehabilitative services for many residents, meal quality for those who commented (with a few naming the cook), and the facility’s physical upgrades. Areas requiring attention, based on the reviews, include infection control/cleanliness practices in some units, consistency of personal care and medication administration, staff professionalism and communication, and addressing the serious financial-safety allegations raised by multiple reviewers.

In summary, the review set shows a facility with many satisfied families who report attentive, loving staff, good rehab outcomes, and active improvement efforts. However, there are also multiple, substantive complaints — including cleanliness, hygiene, medication issues, unprofessional staff behavior, and allegations of theft — that create a mixed overall impression and point to variability in resident experience. The most consistent patterns are (1) strong praise for staff and recent improvements, and (2) recurring, serious concerns from other reviewers that warrant careful consideration by prospective families and continued attention by management.
