    Thoughtful staff, inconsistent basic care

I've had a long, mixed experience here. Many nurses, CNAs and therapists were thoughtful, responsive and helped with rehab - rooms, activities and common areas were clean and comfortable and it often felt like home. Unfortunately staff quality was inconsistent: food and basic care were hit-or-miss, communication poor at times, and I witnessed neglectful behavior and cleanliness lapses that are unacceptable. Management has improved and will respond when alerted, so I'd recommend touring personally and closely watching care before deciding.

    Summary review

These reviews present a deeply mixed and polarized picture of Brickyard Healthcare - Brookview Care Center, with a wide range of experiences from highly positive rehabilitative and long-term care to severe allegations of neglect and abuse. Multiple reviewers praise the facility for strong rehabilitation services, caring memory care staff, and a home-like atmosphere. At the same time, other reviews recount disturbing instances of neglect, poor hygiene, medication and feeding failures, and even reports that prompted police reports and Adult Protective Services involvement. The result is a facility that elicits both strong recommendations and urgent warnings from different families.

Care quality and clinical outcomes show significant variability. Many reviewers celebrate the OT/PT team and therapy staff for helping residents regain independence; some residents made substantial rehabilitation progress and families explicitly said they would return or recommend the facility. There are numerous reports of attentive nurses and CNAs who are thoughtful, patient, and go above and beyond. Conversely, there are serious, specific complaints about basic care being neglected — failure to feed, failure to administer medications, wounds left undressed, and exposure to feces or urine. At least one reviewer described refusal to call an ambulance and involvement of police and APS, indicating that for some residents the failures crossed from poor service into potentially dangerous neglect.

Staffing and culture are recurring themes and appear inconsistent across shifts and units. Positive comments describe committed long-term staff, courteous receptionists, employee recognition programs, and named individuals (Whitney, Tiffany, Megan, Justin) who provided excellent care. Several reviews note a welcoming, clean environment with staff who make residents feel seen and respected. On the other hand, multiple reviews report rude or abusive behavior, specific allegations of mistreatment (including a named staff member and incidents in shared showers), and descriptions of staff who seem disengaged or "just drawing a check." This variability suggests uneven hiring, training, supervision, or turnover that produces strong contrasts in day-to-day experiences.

Facilities and environment earn mostly favorable marks but are not without concerns. Many reviewers mention newly updated rooms, private and spacious accommodations with private bathrooms, a pleasant Hall of Heroes, outdoor deck seating, and two roomy dining areas. Several families noted daily housekeeping, adequate heating, and low dust. However, there are countervailing reports of foul urine odor and a general lack of cleanliness in certain cases, pointing to inconsistent maintenance standards. Activity programming is consistently noted as a positive, with a broad variety of social and recreational options — bingo, karaoke, themed events, church services, family game days, and prom are cited — which contributes to a more engaged resident experience for many.

Dining and nutrition receive mixed feedback. Some reviews praise food services and highlight a home-cooked meal for memory care residents; other reviewers complain about poor meal quality and state that families resorted to DoorDash to supplement inadequate meals. Medical diets, TV access, and reading/family visit accommodations were reported positively in some accounts, but the mixed comments on meals reflect the broader pattern of uneven service delivery across the facility.

Management and communication show a split pattern: several reviewers applaud responsive management and improvements over time, noting long-term relationships and satisfaction with administrative support. Yet other families report poor communication, feeling unheard, threats regarding staff discipline, and calls for the facility to be shut down. The existence of both praising and condemning narratives indicates pockets of effective leadership and problem-resolution but also cases where leadership failed to address or prevent serious care lapses.

Notable patterns and risks: The most serious recurring concern is the presence of high-severity incidents (neglect, medication errors, failure to assist with feeding, unsanitary care) reported by multiple reviewers — some of which escalated to formal investigations. These incidents raise safety and quality-of-care red flags that cannot be ignored. At the same time, many families report exemplary rehabilitative outcomes, attentive staff, and an environment that feels like home. Prospective residents and families should therefore approach this facility with caution: seek recent inspection records, ask for shift-to-shift staffing information, request references or speak with current families for the particular unit of interest, and tour at different times of day to observe staff interactions and cleanliness firsthand.

Bottom line: Brickyard Healthcare - Brookview Care Center can deliver excellent rehabilitative care, compassionate caregiving, and a comfortable environment for many residents, but there are credible, serious reports of neglect and unprofessional behavior that have led some families to consider the facility unsafe. The facility appears to have both high-performing teams and problematic areas; individuals considering placement should investigate current staffing stability, oversight practices, and incident history before making a decision.

About Brickyard Healthcare - Brookview Care Center

Brickyard Healthcare - Brookview Care Center sits less than five minutes from Methodist Hospital in Indianapolis, IN, where folks get skilled nursing care and a range of services in a facility with 136 certified beds, though on most days about 76 residents live there. Brookview Care Center's owned by Hendricks County Hospital and managed by Brookview Operating LLC since 2012, and the place runs under the Brickyard Healthcare name. The building's had recent renovations which folks say made the layout easier to get around and the common areas feel more comfortable, plus there are outdoor areas where residents can relax or visit with family. There's a Memory Care community for people with Alzheimer's or dementia that includes trained staff, a secure setting, and an outdoor area with safety alerts for wandering; folks can get private suites and amenities like a barber and beauty shop, daily activities, church services, arts and crafts, Wi-Fi, cable TV, family lounges, phone service, laundry, and snacks. Staff help with personal care and offer 24/7 nursing, physical, occupational, and speech therapy, outpatient therapy, short-term rehabilitation, post-acute rehab, and long-term care. Brookview also provides specialized dementia care, dialysis, stroke care, diabetic care, pain management, cognitive training, pulmonary care, VA contract services, hospice, and palliative care. Therapy includes recovery care to help folks regain strength after a hospital stay, and the nurse turnover rate-at 31.7%-sits lower than the state average. The nurse staffing ratio stands at 3.65 nurse hours per resident per day, which is just a bit lower than the Indiana state average. The facility's led by Executive Director Patti Aldridge, who's spent years in long-term care.

Inspection reports show Brookview Care Center's had 36 deficiencies, including some with infection control and notifications about transfers, though so far none caused actual harm but did bring potential for more than minimal harm, and there were also resident rights concerns related to dignity and self-determination. Daily living and recreation get attention here, with activities and support meant to fit each resident's needs, but some inspection reports do note complaints and infection controls that need ongoing monitoring. The staff focuses on being compassionate in their work, aiming for recovery and well-being, and the facility's always working to improve based on inspection findings. People who stay at Brookview get options for skilled nursing, memory care, outpatient therapy, and VA contract services, while also having access to both social and practical amenities like family rooms, linens, housekeeping, and transportation. The building's managed by experienced healthcare professionals who try to maintain a welcoming atmosphere and provide care that's both tailored and practical, keeping things straightforward while covering a wide range of needs for seniors and people with serious health conditions.
