Kaw River Care & Rehabilitation sits in Edwardsville, Kansas, zip code 66111, and has been around since 1973, offering care to the very frail and dependent who need skilled nursing help, and the place has a total of 45 certified beds but on average about 33 residents stay there each day, you see, and Mission Health Communities has been tied to the facility since 2019 with ownership and management changes happening over time, but steady names like Stuart Lindeman and Tina Thomas have remained on the list for some time. The facility holds a skilled nursing unit and offers independent living, assisted living, memory care, and several care options like short-term rehabilitation, long-term residency, and respite stays, so there's a fair bit of flexibility for people who need just a little help or complete nursing care day and night. Kaw River Care & Rehabilitation supports folks through specialized clinical services including wound care, sepsis care, IV therapy, cardiac and pulmonary rehab, and can handle things like CPAPs, BiPAPs, NG tubes, PEG tubes, as well as transportation for hemodialysis. Physical, occupational, and speech therapy are all on the list, too, and there's extra attention given to personal comfort with private and semi-private rooms, a dining room, hair salon and barber, outdoor areas, social spaces, and wifi, making things as comfortable as one could hope in this sort of setting. The facility reported 17 deficiencies on its last inspection in August 2023, including three related to infection control and a resident rights citation about dignity and communication, but inspectors found no actual harm had happened, though there was a potential for more than minimal harm, and across several years there've been 37 total deficiencies. Nurses at Kaw River Care & Rehabilitation spend about 3.88 hours per resident a day, which is a touch below the Kansas average of 4.0 hours, but their nurse turnover rate is 46.7%, a little better than the state average of 50.4%, so some faces do stick around. The facility's connected with Meta platforms like Facebook and Instagram, shares updates there, and has links to larger groups like Mission Health Communities with several indirect owners, but what's always stood out is the emphasis on recovery, rehabilitation, and meeting the needs of each resident to try to improve their quality of life day by day, with a staff that's known for their dedication to respectful and patient-centered care, though no place is without its problems and every year brings an inspection and some lessons to be learned.

Caring staff, leadership needs improvement
I'm very pleased overall - the staff are caring and often go above and beyond, the facility is clean and odor-free, and the warm, small-community feel with activities has helped my loved one thrive. I have, however, noticed leadership instability, high turnover, and gaps in mental-health training; there have been a few troubling incidents (rude/unprofessional behavior, missed care and poor discharge communication) that need fixing. On balance I feel comfortable leaving my family here because of the dedicated team, but I want those training and leadership issues addressed.
