    Excellent therapy but staffing issues

I had a mixed experience. The rehab team, therapists and many nurses/CNAs (Denise the concierge stands out) were excellent - clean facility, good food, engaging activities and real progress toward going home. However staffing and management are inconsistent: slow/ignored call lights, delayed or missed meds, billing/social-work headaches and safety incidents were frequently reported, so check current staffing, incident history and discharge/billing policies before choosing.

About Optalis Health & Rehabilitation of Ann Arbor

Optalis Health & Rehabilitation of Ann Arbor, found at 4701 East Huron River Dr., offers several types of care in one location, including skilled nursing, rehabilitation, assisted living, independent living, memory care, respite care, hospice care, home health, and non-medical home care, so families often use it when someone needs a place with different care options under one roof, and the staff, who are trained for many situations, provide care for both short-term recovery and long-term support. The building has 180 certified beds, with shared and private room choices, and they keep an average daily census of about 124 residents, which means you'll usually find it busy but not overcrowded. Some folks appreciate the amenities that include TVs and internet, lounges, activity spaces, phones in the rooms, plus full-service dining that handles plenty of dietary needs, so people with special diets can count on eating safely, and they take care of housekeeping and laundry, so residents don't have to worry about the chores like they used to at home.

Residents can participate in weekly activities, like gardening or the group outings that're planned all the time, and the activity areas help people stay engaged and avoid feeling lonely. The facility's part of the ProMedica Skilled Nursing and Rehab system and carries an affiliation with Optalis Healthcare, but direct ownership lies with Om Holdco 5 LLC, and some indirect owners include Optum Management Solutions, Inc, Charles Dunn, Rajan Patel, and Hemant Shah, so there's a mix of leadership behind the scenes. It runs as a for-profit limited liability company and is listed in local plans for Michigan, with a Medicare-certified status, and it works with programs like Trinity Health Plan for managed care, plus it can help guide families through insurance and payment questions.

People looking for care should know the facility provides comprehensive rehabilitation and therapy services, with physical, occupational, and speech therapy available both inpatient and outpatient, and the nurses give around-the-clock care, including medication management in the assisted living areas. The staff try to focus on keeping everyone as comfortable and independent as possible, and the memory care program is set up for those living with Alzheimer's or dementia, offering structured activities meant to reduce worry and confusion, while the respite care lets caregivers take a break and know their family member's safe in a supportive spot. For people with breathing troubles, there's respiratory therapy with treatment plans made for each person.

Facility guides help families understand what to expect during a care or rehab stay, and there's help for those planning pre-surgery, plus answers to questions about rehab and therapy. The facility's Cranberry Park communities provide options for independent living and assisted living, with additional specialized memory care. They keep an eye on hospitality and try making each resident feel at home, but families should know some quality marks-like nurse hours per resident per day (3.65) running a little lower than the state average (4.0), and the nurse turnover rate at 50.5% is higher than the state's 46.9%, with inspection reports listing 51 total deficiencies. There were recent citations, including one related to infection control, one about meeting dietary standards, one for keeping the facility safe and clean, and another concerning residents' rights and self-determination. Still, people often choose Optalis Health & Rehabilitation of Ann Arbor since it provides many care types, resources, and amenities in one place, and the staff work hard to help residents recover better and feel comfortable while they stay.
