    Understaffed, outdated facility; mixed rehab

I had a mixed but mostly negative experience at Riverside. It's a small, outdated facility with no outdoor space, frequent understaffing and high turnover that led to poor monitoring (infrequent turning/diaper changes, feeding issues, even a worsening bedsore in my case). Some staff and the rehab team were loving, professional, and got good short-term results, but others were rude, neglectful, and communication/administration felt money-focused. I wouldn't trust them for long-term care (0/10) - possibly okay for brief rehab only.

    Summary review

Overall impression The reviews for Riverside Nursing & Rehab present a starkly mixed picture with strong positive testimonials alongside deeply concerning allegations of negligence and poor care. Many reviewers praise the staff, rehabilitation program, and small, home-like atmosphere. At the same time, multiple reviews report serious lapses in medical care, poor communication, staffing shortages, and facility maintenance issues. The overall sentiment is polarized: some families and former residents describe an excellent, personalized rehabilitation experience and compassionate staff, while others recount incidents that they characterize as neglectful or dangerous.

Care quality and medical oversight One of the most prominent themes is highly inconsistent care quality. Several reviewers report excellent, professional physical and occupational therapy, with effective rehab resulting in rapid discharge home and an overall positive rehab experience. Conversely, there are multiple severe allegations of medical neglect: undiagnosed or poorly managed conditions, inadequate turning and continence care, wounds progressing to severe stages (including a bedsore alleged to have progressed to the bone), cases of malnutrition and dehydration leading to ER transfers, and inconsistent medication administration. Some reviewers specifically attribute life-threatening outcomes to these failures. This inconsistency suggests the facility can provide very good rehabilitative care in some cases but also can fail to provide basic medical monitoring and wound/nursing care in others.

Staffing, staff behavior, and turnover Staff are frequently described in positive terms—kind, caring, loving, efficient, and nurturing—especially by reviewers who had good rehab outcomes or positive long-term care experiences. The small facility size is credited with enabling individualized attention. However, there are also multiple reports of understaffing, frequent sick calls, and high CNA turnover. These staffing problems are linked by reviewers to inadequate feeding assistance, delayed bathing, neglect of hygiene and continence needs, and reduced supervision. Several reviews allege abusive or neglectful behavior, and some describe encounters with law enforcement. The picture is one of variable staff performance: long-tenured, competent leadership is reported by some, while others describe individual caregivers or nurses providing very poor care or being rude.

Facility condition, size, and amenities Riverside is described by many as a small, quaint facility that fosters a home-like environment and individualized care. Some reviewers appreciate the small scale, friendliness, and dog-friendly policy. Positive reports mention monthly outings, activities, and home-cooked meals. However, other reviewers portray the building and common areas as old and rundown, noting a lack of outdoor space or courtyard and small/shared rooms that can be a detractor for certain residents. Activity programming is reported as robust by some families and almost non-existent by others, indicating variability in programming or perception.

Dining and activities Dining receives mixed commentary: several reviewers praise home-cooked meals and enjoyable food, while others imply inadequate nutrition due to feeding assistance failures leading to malnutrition and dehydration in severe cases. Activity offerings also vary—many reviewers highlight daily activities and outings, but other reviews report rare activities or no regular schedule. This again points to inconsistent resident engagement and programming.

Management, communication, and admissions Admissions and marketing personnel receive consistently positive mentions for being easy to work with and facilitating smooth transitions. Some reviewers specifically praise the administrator and Director of Nursing as trusted, competent leaders. Conversely, a number of reviews criticize administration for prioritizing finances, requiring family members to micromanage care, and failing to communicate timely updates about residents’ conditions. Communication lapses and lack of proactive family updates are recurring concerns tied to the perceived decline in care quality in some cases.

Notable patterns and risk signals A critical pattern is the juxtaposition of high-quality rehab outcomes with reports of severe neglect and medical harm. The presence of both long-tenured positive leadership and allegations of turnover, understaffing, and inconsistent care suggests that care quality may depend heavily on which staff are on duty and how well specific clinical issues are being managed. The most serious risk signals include documented claims of advanced pressure injuries, malnutrition/dehydration leading to organ issues, inconsistent medication administration, and police or ER involvement. These merit particular attention from prospective residents or family members weighing this facility.

Conclusion and guidance for readers Riverside Nursing & Rehab appears capable of providing excellent, compassionate rehabilitation and individualized long-term care when staffing and oversight are functioning well—many reviewers attest to positive therapy outcomes, caring staff, and a homelike environment. However, the facility also has multiple reports of severe lapses in basic nursing care, medical oversight, and communication that in some cases resulted in serious harm. Prospective residents and families should weigh these polarized reports carefully: visit in person, ask specifically about nurse staffing ratios, wound and nutrition management protocols, staff turnover rates, and family communication practices; request documentation of training and incident follow-up where relevant. If considering Riverside for short-term rehab, confirm the specific therapy team and recent outcome metrics; if considering for long-term care, probe care routines, supervision, and quality improvement processes to assess how the facility prevents and responds to the kinds of failures described by negative reviewers.
