    Outstanding therapy, issue with behavioral

I placed my mother here and the therapy team was outstanding - they motivated her and she recovered fabulously, returning home at a walker level. The facility is clean and airy with large, tasteful rooms, friendly/caring staff, well-attended activities and a varied menu. My main concern: an outburst required an ER transfer to a geriatric psych unit because the small two-person rooms and unclear protocols left staff unable to manage it. Overall very pleased with the rehab and environment but the handling of behavioral crises needs improvement.

    Summary review

Overall sentiment from the reviews is largely positive about the Springs At the Fountains' everyday environment, amenities, and rehabilitative therapy, but there are notable concerns about the facility's capacity to manage acute behavioral or psychiatric incidents and about clarity of care protocols.

Facilities and environment: Multiple reviewers emphasize a clean, well-maintained facility with open, airy, and large rooms. The decor is described as tasteful and the overall environment as welcoming. The dining area is specifically noted as well maintained and the menu as offering a wide variety, which suggests attention to resident comfort and quality of life. The small size of the facility is mentioned as a factual characteristic; while small size can support intimacy and community, reviewers also note it results in double-occupancy rooms (two patients per room), which some families may view as a drawback depending on privacy needs.

Staff and activities: Staff are characterized as friendly, caring, and courteous. Activity programming appears strong—residents are engaged and activities are well attended—indicating an active social calendar and staff involvement in resident engagement. These positive staff-related comments align with reports of a welcoming atmosphere and suggest day-to-day operations and resident interaction are handled well.

Therapy and outcomes: The therapy team is singled out for particularly strong performance. One reviewer reports that therapists worked well with their mother, motivated her to get moving, and contributed to a successful recovery that allowed the resident to return to her prior residence using a walker. This indicates a high quality of rehabilitative services for at least some residents and successful coordination between therapy staff and families.

Safety, behavioral management, and care protocols: The most significant negative themes relate to behavioral and psychiatric care. One review describes an outburst that resulted in an ER transfer and placement in a geriatric psychiatric unit. That account notes the facility was not equipped to provide a sitter for the resident during the episode and that care protocols were unclear. These points raise concerns about the facility's readiness to manage acute behavioral incidents, the availability of one-to-one staffing when needed, and transparency or consistency in emergency response procedures and communication with families.

Patterns and overall assessment: In synthesis, Springs At the Fountains appears to provide a clean, pleasant living environment with engaged staff, good dining options, solid activity programming, and an effective therapy team that can produce strong rehabilitative outcomes. However, there is a clear and important caveat: reviewers identified at least one serious behavioral/psychiatric event where the facility's capacity to safely manage the situation and communicate care protocols was questioned. Prospective residents and families should weigh the strong everyday care and therapy services against the reported limitations in handling acute psychiatric needs and the presence of shared rooms when evaluating fit for specific care requirements.

    About Springs At the Fountains

Springs At the Fountains is a senior living facility designed for older adults who need different levels of care, and they offer assisted living, memory care, independent living, short-term rehab, and nursing care, so it's a place where someone can find support if they need a little help or more advanced care like skilled nursing or memory care for Alzheimer's, and the staff at Springs At the Fountains help with daily activities like bathing, dressing, managing medicine, and they keep an eye on health with around-the-clock supervision, a 24-hour call system, and help with mobility, bladder and bowel control, and other personal care. Residents live in private suites or shared rooms, which come fully furnished with kitchenettes, private bathrooms, air conditioning, cable TV, telephone, and internet, and if someone needs a wheelchair or has medical disabilities, the building's layout and security measures help keep residents safe and prevent wandering, which is especially useful for people living with dementia. The dining room serves restaurant-style meals with special diets when needed, and residents can join scheduled activities, enjoy a meal together, use a small library, or spend time outside in the garden, and there's even a beauty salon, so people can get a haircut without leaving the building. Housekeeping, linen service, and meal preparation are covered, which takes a lot off someone's plate. The building has a fully sprinklered environment for fire safety, and infection prevention is strong, with high vaccination rates for both influenza and pneumococcal illness among long-term residents. Health and safety inspections have found some issues in the past, including things that caused harm, but those were addressed, and the facility is open about tracking things like pain, pressure sores, infections, and weight loss to make sure residents stay as healthy as they can.

Springs At the Fountains is not in a hospital and is not labeled as a special focus facility, but it does participate in both Medicare and Medicaid programs, which can help people who need financial support, and as a continuing care retirement community (CCRC), the place helps people age in place, meaning residents don't have to move as their care needs change. The staff includes nurses and certified nursing assistants around the clock, and the staffing per resident is about 0.72 RN hours, 0.56 LPN hours, and 2.72 CNA hours per resident, totaling 1.28 staff hours per resident daily. While they offer support for behavioral needs and Alzheimer's care, severe behavioral outbursts usually lead to automatic referrals to the ER, so ongoing behavioral management on-site may have limitations. The owners run several nursing homes, and Springs At the Fountains can accommodate about 38 residents in the living communities, with 50 certified beds and a 94% occupancy rate. Amenities help support independence and comfort, with laundry, cable or satellite TV, Wi-Fi, and options for in-home care or hospice if needed, and if someone wants to take part, resident and family councils help people have a say in how things run and plan community-focused activities for social and mental well-being. Transportation is available for doctor's appointments, and nursing staff coordinate with healthcare providers to manage medicines and treatments. There are safety features built in, like systems to lock outside doors and careful monitoring to help prevent residents with memory loss from getting lost. Recreational spaces like the garden, outdoor patio, and small library help residents spend time as they want, and planned activities keep things interesting. Springs At the Fountains works to offer care that matches what each resident needs, without overpromising, and tries to make its environment comfortable and safe for seniors who want support in daily life.
