    Good rehab; poor long-term care

I had a very mixed experience at Livonia Woods. The rehab teams (PT/OT/SLP) and activities director were excellent - realistic goals, personalized therapy, and they helped my family member improve. Several nurses and aides were kind, organized, and went above and beyond. However the place is chronically understaffed: long call-light waits, rude or unresponsive nurses at times, missed care (soiled diapers, delayed checks), poor communication, and safety/neglect incidents that alarmed us. The building feels run-down, meals are small/mediocre, transportation and scheduling were often mishandled, and costs are high with no Medicaid option. Administration responsiveness was hit-or-miss and ownership changes seemed to make things worse. My takeaway: great rehab if you need short-term therapy and stay hands-on, but I would not trust it for high-dependency or long-term care without constant oversight.

    Summary review

Overall sentiment for Wellspring Lutheran Services - Livonia Woods Campus is highly mixed, with a clear split between consistently strong rehabilitation services and frequent, serious operational and staffing concerns. Many reviewers highlight top-notch physical, occupational, and speech therapy programs that led to measurable improvement and successful discharges. Multiple families and residents praised individual therapists and the rehab team for producing clear functional gains, providing individualized therapy plans, and coordinating care well. The activities director received repeated positive mention as an important contributor to residents’ quality of life.

At the same time, staffing and responsiveness are the dominant negative themes. Numerous reviews report chronic understaffing, particularly on night and overnight shifts, with long call-light response times and delayed nursing attention. Several accounts escalate beyond poor responsiveness to allegations of neglect: residents reportedly left in soiled diapers, showers refused, and delays in emergency responses (including reports of delayed CPR or ambulance calls). These safety-related claims are among the most serious concerns reviewers raise and contribute strongly to negative impressions. Reports indicate variability across shifts—daytime staff are often described as attentive and organized, while nighttime coverage and some nursing shifts are described as short-staffed and unresponsive.

Staff attitude and behavior are inconsistent across reviews. Many family members describe compassionate, kind, and helpful staff who treated patients like family and went above and beyond. Others report rude, hostile, or even abusive behaviors from nurses and aides, with allegations of cursing, threatening conduct, and poor bedside manners. High staff turnover, low pay for aides, and morale issues are cited as likely contributors to this inconsistency. Communication problems are another recurring theme: unanswered phones, missed voicemails, mistaken or missed transportation/appointment scheduling, and perceived lack of administrative follow-through were frequently mentioned.

Facility condition and dining receive mixed feedback. Several reviewers found the building clean, pleasantly decorated, and homey; others described areas as run-down, dirty, or outdated. Dining impressions vary—some cite hot meals and good variety while others complain about cold food, small portions, poor quality, and limited meal options. The dining room size is mentioned as small by a few reviewers, with suggestions that a larger dining area would better support communal meals and socialization. Additional operational issues include reports of missing personal items with unsatisfactory investigations, lab service delays, and occasional limitations on therapy access due to COVID quarantine protocols.

Management and cost concerns are also present in multiple reviews. Some families appreciated prompt problem resolution and found administrators responsive; others felt administration was unresponsive or focused on billing and cost-savings rather than resident care. The facility’s pricing and policies also drew criticism—one reviewer noted that Medicaid is not accepted and expressed concern that high patient charges were not translating to adequate frontline staffing. Discharge planning is occasionally described as rushed, and there are comments about limited access to certain equipment or therapy resources in specific situations.

In summary, prospective residents and families will encounter a facility with a strong rehabilitation reputation and several compassionate, capable staff, alongside repeated reports of serious operational issues—most notably understaffing, inconsistent nursing responsiveness, communication breakdowns, and variable facility cleanliness. The experiences appear to vary widely by shift, unit, and individual caregivers. When considering Livonia Woods, visitors should prioritize direct questions about staffing ratios (especially night coverage), emergency protocols, recent safety records, Medicaid acceptance and pricing, how personal items and theft complaints are handled, and to observe day and evening staffing and meal service. A targeted, multi-shift tour and specific inquiries about the rehab team and documented outcomes can help balance the facility’s clear strengths in therapy against the documented risks related to staffing and safety.
