    Friendly staff but worrying lapses

I have mixed feelings. The building is clean, welcoming, and many staff are caring, friendly and run excellent activities and rehab - my family has been pleased - but chronic understaffing, poor communication and uneven dementia training have led to worrying lapses (sanitation, medication and occasional neglect), so I'd be cautious placing someone there.

    Summary review

Overall impression: Reviews for Intersect Healthcare of Roscommon are strongly mixed, with a sizable portion of reviewers expressing high satisfaction and gratitude while a number of others report serious concerns. Positive voices highlight compassionate, friendly staff, effective rehabilitation, engaging activities, and a clean, well-organized building with large private rooms and a secure dementia unit. Negative reports raise red flags about understaffing, neglect, poor communication, training gaps (especially for Alzheimer's care), and isolated but serious incidents involving hygiene, feeding, and medication handling. The pattern is one of considerable variability in resident experience depending on unit, shift, and possibly the specific staff on duty.

Care quality and safety: Several reviewers describe very high-quality, attentive care: caregivers who go the extra mile, staff who are responsive and loving, and rehabilitative therapy that is effective and helped residents recover quickly. Conversely, other reviewers recount alarming safety and neglect issues. Specific serious incidents were reported, including a resident found in bed late in the morning with food remaining in her mouth, difficulty getting proper feeding assistance, concerns around hospice transfer, and at least one mention of improper medication or comfort care handling. These conflicting reports suggest inconsistent standards of care and potential systemic issues such as staffing shortages and incomplete training that can materially affect resident safety.

Staff, training, and communication: Staff are frequently described as friendly, engaging, and professional, with several mentions of long-tenured, knowledgeable employees and an activities director who excels. Families who had positive experiences emphasized the caring nature of staff and a family-like atmosphere. However, multiple reviews call out chronic understaffing, poor intra-staff communication, and inadequate training—particularly for dementia and Alzheimer's care—leading to neglect or insufficient support for more vulnerable residents. There are also reports of verbal abuse by staff in some cases, which is an especially serious concern when juxtaposed with otherwise glowing accounts of compassion. The combination of praised long-term staff and criticism of management/communication points to uneven staffing practices or inconsistent oversight.

Activities and social environment: Activity programming is a clear strength in many reviewers' eyes. Many describe engaging group events, games with prizes, community involvement, and frequent one-on-one visits. The activities director is specifically singled out for going above and beyond. These programs appear to contribute to a positive atmosphere for residents who can participate. That said, reviewers note that participation is limited for some residents in the Alzheimer's wing, indicating that programming may not be well adapted to all cognitive needs or that dementia-specific engagement is uneven.

Facilities and dining: The facility itself is frequently called clean, organized, and inviting, with praises for large private rooms and a secure dementia area. Therapy spaces and rehab services receive positive remarks as well. Dining receives mixed feedback: multiple reviewers praise the food and customizable meal options, while at least one report describes an unsanitary dining incident. Combined with other safety-related reports, the dining issue underscores the variability in adherence to standards.

Management, consistency, and notable patterns: A central theme is inconsistency. Some families are highly impressed and would strongly recommend the facility; others say they would never place a loved one there. Reports indicate that quality can vary by unit and by shift, with dementia units and overnight/daytime staffing levels repeatedly cited as problematic. Some reviewers note new management but say problems persist, suggesting systemic issues that have not been fully resolved. Additional practical concerns include occasional missing furnishings in rooms and at least one mention of difficulties with discharge or readmission procedures.

Bottom line: Intersect Healthcare of Roscommon shows clear strengths—compassionate staff members, strong activity programs, effective rehab services, and a generally clean, comfortable building with private rooms and a secure dementia unit. However, the facility also has multiple, serious areas of concern raised by reviewers, including understaffing, instances of neglect and verbal abuse, medication and dining safety issues, and inconsistent dementia care. Prospective families should be aware of this variability. When evaluating the facility, it would be prudent to ask targeted questions about staffing ratios, handling of dementia and feeding needs, incident reporting and medication management, hospice and discharge/readmission policies, and to observe mealtimes, activity participation in the dementia unit, and staff-resident interactions across different shifts.

    About Intersect Healthcare of Roscommon

Intersect Healthcare of Roscommon belongs to a larger group that runs several facilities across Michigan, including Mission Point Nursing & Physical Rehabilitation Centers in places like Beverly Hills, Warren, Flint, Clinton Township, Clawson, Clarkston, and Superior Woods, and they've been at it since 2010 with their headquarters in Washington, Michigan. The leadership group includes a Nurse Concierge, a Head of Human Resources, a Director of Operations, plus a Regional Director of Clinical Services who looks after care and nursing business across different sites, and there's about 469 employees working here which is quite a team and shows there's always staff on hand. They focus on long-term skilled nursing care as well as short-term rehab services for folks who need extra help, and there's memory support for those living with memory loss, so the staff tries to keep things centered around each person's unique needs, without a one-size-fits-all approach. Their care covers things like SmarterHeart Cardiac Rehabilitation, Pulmonary Rehabilitation, physical, occupational, and speech therapy, and there's on-site dialysis plus respite care if someone needs a short stay. Senior residents can get 24-hour skilled nursing, and sometimes just knowing there's transport to local appointments or help from a registered dietitian with meals can make things easier day to day. The company has quite a bit of know-how with healthcare rules, licensure, Medicaid and Medicare handling, employee issues, vendor agreements, patient billing, and they're used to dealing with a variety of needs and situations, even facilities that might've had problems in the past. Each person gets an individualized treatment plan, and all of this happens under a system that values good operations and personal care above anything else, working quietly and steadily rather than making a lot of noise about it.
