    Unprofessional overcrowded facility, alleged abuse

I hate this facility. Management is unprofessional and communication is poor; rooms are overcrowded with no seating or privacy and there's a strong odor. I was told about alleged inappropriate physical conduct by staff - worst experience, I would not recommend.

    Summary review

Overall sentiment across the provided reviews is mixed but leans toward negative, with one very positive endorsement contrasted by several sharply critical accounts. One reviewer calls the facility a "wonderful nursing home" and strongly recommends it to loved ones, indicating that at least some residents or family members have had a highly satisfactory experience with care. However, the majority of comments emphasize serious concerns about management, staffing behavior, and the physical environment.

Care quality and safety present a conflicted picture. The single highly positive comment suggests competent care can occur at the facility, but multiple negative comments—most notably an allegation of inappropriate physical conduct by staff—raise significant safety and oversight concerns. Because the allegation is reported in the reviews as an allegation rather than a verified fact, it should be viewed as a serious complaint requiring investigation. Additionally, the phrase "hate it, worst mess" used by a reviewer signals a strongly negative lived experience that could reflect problems with day-to-day care or resident well-being for some people.

Staff and management are frequent targets of criticism. Reviews call out "unprofessional management" and "poor communication skills," and they specifically name administrators Michelle and Quajo as being criticized by reviewers. These comments point to problems in leadership, family/staff communication, or responsiveness to concerns. Poor communication and perceived unprofessionalism can exacerbate other problems (for example, making allegations of staff misconduct harder for families to resolve or causing distrust between families and the facility).

The facility environment and accommodations are another clear area of concern. Multiple reviewers mention overcrowded rooms, a lack of seating, and insufficient privacy—issues that directly affect resident comfort and dignity. A strong odor and explicit cleanliness concerns are also reported, which tie into perceptions of overall facility upkeep and infection-control practices. These environmental problems, combined with complaints about staffing and management, suggest systemic issues with capacity, resourcing, or operational oversight.

Patterns across the reviews suggest inconsistent performance: at least one reviewer had a very positive overall impression while several others reported serious and specific negative issues. The most prominent themes to emerge are leadership and communication problems, environmental and privacy shortcomings, and at least one serious allegation regarding staff conduct. Given the mix of feedback and the severity of some complaints, prospective residents and family members should treat reviews as mixed signals and seek additional, up-to-date information (for example, state inspection reports, recent family feedback, and facility responses to complaints) before making decisions. For the facility, the review set highlights priority areas for attention: investigating any allegations of misconduct, improving management communication, addressing overcrowding and seating/privacy in rooms, and correcting odor/cleanliness issues to restore confidence among residents and families.

The place is large and has a memory care wing that's certified for up to 101 adults, so they know how to handle things like Alzheimer's with special programs and therapies, plus there are safety measures like secure spaces to help keep residents from wandering, and staff will help with bathing, dressing, toileting, grooming, and personal care when that's called for. Meals are cooked fresh every day, and the dining area is set up to be simple and comfortable, and residents can enjoy home-prepared food or visit one of the many nearby restaurants, since the facility sits close to cafes, pharmacies, parks, hospitals, and places of worship, so outings and visits aren't hard to manage, and there's transportation help for appointments or group trips. Bortz Health Care of Warren also acts as a clinic, offering medical services from internal medicine doctors, psychiatrists, and psychoanalysts, while nursing staff provide care like medication support, wound care, and therapy that covers physical, occupational, and rehabilitation needs, and both inpatient and outpatient options are offered under the Bortz Health Care Facilities name, along with Medicare and respite care, which can be good for families who need a short break. Amenities like Wi-Fi, cable TV, washers and dryers, a sprinkler system, guest parking, housekeeping, and laundry are there, and rooms get regular cleaning whether folks pick semi-private or private layouts, which helps everyone live comfortably and safely. The staff coordinates transportation, recreation, and wellness activities to keep life organized, and, while the facility hasn't gotten BBB accreditation or any reviews yet, folks wanting more details can usually check licensing and inspection reports and look into Medicaid or Medicare help when considering costs, just like you'd do for any senior community, and if anyone needs to schedule a tour or get advice about choosing between assisted living and skilled nursing, the staff is ready to help folks understand the process.
