    Dirty facility rude staff theft

I put my mother here against her will and I regret it. Laundry and personal items went missing, rooms were dirty and smelly, carpet stained, towels/towel racks missing, and litter/food left on tables. Staff were rude, short-staffed and inattentive-phones didn't work, documentation and communication were poor, meds and kitchen water were mishandled, residents left alone or not helped to the bathroom, fall/neglect risks obvious, and I even saw staff take residents' chocolates. I moved my grandma out - do not recommend.

    Summary review

Overall sentiment: The reviews present a strongly negative overall impression of O'Neill Care and Rehabilitation Center. Multiple reviewers report serious and recurring problems spanning cleanliness, staff behavior and responsiveness, clinical safety, communication, and basic operations (laundry, phone lines, documentation). The tone across summaries is of frustration and concern, with several reviewers explicitly stating they would not recommend the facility and some reporting that they moved family members out.

Care quality and safety: A dominant theme is concern about basic care and safety. Reviewers describe inattentive or unresponsive staff, residents left without help to the bathroom or unattended for long periods (one report of being left in a room for over an hour), residents at elevated fall risk, and specific incidents suggesting neglect (e.g., residents in wheelchairs getting stuck in doorways). There are also reports of medication and kitchen water problems and poor documentation, which together raise red flags about clinical oversight and safe medication/food handling. One reviewer references an extremely poor health report (described as "worst in nation"), indicating that at least one inspection or report was perceived as severely negative.

Staffing, communication, and culture: Staffing levels and staff behavior are repeatedly criticized. Short staffing is cited as a frequent problem and is linked to slow response times and poor resident engagement. Multiple accounts describe staff being rude to residents and family members, poor bedside manners, and a general culture of staff being treated poorly, which reviewers say affects the care residents receive. Communication problems are also widespread: nonworking phone lines, poor documentation, and unresponsive staff make it difficult for families to get updates or coordinate care. There are also allegations of inconsistent COVID safety practices and staff not wearing masks, which contributed to family distrust of the facility's infection control.

Facilities, cleanliness, and operations: Reviews report dirty rooms, litter on floors, dirt on rec room tables, and foul odors — all indicating inadequate housekeeping and environmental maintenance. Laundry issues and missing clothing are mentioned repeatedly, as are missing fixtures such as towel racks and dirty carpeting. Operational failures extend to phone service outages and poor documentation practices, compounding family concerns about the facility's ability to manage day‑to‑day resident needs.

Dining and food handling: Dining receives particularly negative remarks: food is described as "deplorable," and there is an allegation that staff consumed resident treats (chocolates), which raises both ethical and food safety concerns. Combined with reports of kitchen water problems, reviewers portray the facility's food service as unreliable and sometimes unsafe.

Activities and resident engagement: Several reviewers state that there is no meaningful resident engagement and that the activities program is ineffective. One reviewer called out the activities director specifically as "loud," and others indicated that residents were not encouraged to participate or were physically restricted from exercise. These problems suggest the social and rehabilitative programming is inadequate for residents' needs.

Admissions and family interactions: Some reviewers allege problematic admissions practices, including at least one claim that a relative was admitted against her will. Families also describe being treated rudely and receiving poor communication from staff, leading several to relocate their relatives to other facilities. Reviewers characterize the overall management and culture as poor, and many indicate the facility provides poor value for the cost.

Patterns and severity: The complaints are consistent across many operational areas rather than isolated to one domain. Cleanliness, staffing, communication, clinical safety, and dining issues appear repeatedly, suggesting systemic problems rather than singular incidents. Several specific alarming details (e.g., residents stuck in doorways, medication/kitchen water concerns, staff eating resident food, nonworking phone lines, and alleged poor inspection ratings) elevate the severity of the feedback beyond minor service lapses.

Conclusion: Based on the consolidated review summaries, O'Neill Care and Rehabilitation Center appears to have multiple, recurring deficiencies that impact resident safety, comfort, and family trust. The primary concerns are inadequate staffing and responsiveness, poor cleanliness and maintenance, unreliable communication, questionable food and medication practices, and a negative staff/family culture. Prospective residents and families should approach with caution, seek independent verification (recent inspection reports, on-site tours, direct conversations with management), and consider alternative facilities until there is clear evidence of substantial improvement.

About O'Neill Care and Rehabilitation Center

O'Neill Care and Rehabilitation Center, also called Arbor Care Centers - O'Neill, sits at 1102 N. Harrison St, O'Neill, Nebraska, and has a pretty average size for a nursing home, offering both short-term care for folks coming from the hospital who need a bit of extra help before heading home and long-term care for people who need everyday medical support. The place accepts both Medicare and Medicaid, and it's a for-profit facility set up as a limited liability company. Staff at O'Neill Care and Rehabilitation Center includes registered nurses, LPNs, CNAs, and therapists. They provide quite a few health and therapy services, like physical therapy, occupational therapy, and speech therapy, and have licensed staff providing help around the clock, every day, with nurse and therapy hours tracked and posted. The center focuses on rehabilitation, skilled nursing, and Alzheimer's care, helping people regain strength or manage conditions with physical therapy, speech therapy, occupational therapy, IV therapy, fall prevention, wound care, medical exams, and medication management. There's also help with hospice, palliative care, post-surgical care, post-acute rehab, and outpatient therapy, plus a medical director for oversight. You'll find private and semi-private rooms, meals and dining, transportation, housekeeping, laundry, a beauty salon, and free Wi-Fi and cable. They provide nutrition education, daily activities, a resident council, and a family council, which gives families and residents a way to voice concerns or ideas. For safety, the place's been inspected a number of times, getting varied grades ranging from D up to A over the years, but most recently, it got an overall F rating and was flagged for abuse in the last year, which some folks will want to consider. Even with those concerns, the facility talks about providing care with a focus on wellness, independence, and a comfortable setting. They offer Alzheimer's care, support for the very frail needing intensive nursing care, and assisted living options. Amenities and specialized medical care aim to help seniors recover from illness, injury, or manage everyday needs, but anyone looking at the place will likely want to weigh the health services, daily living features, and the inspection history before making a decision.
