    Good programs, but neglect concerns

I have mixed feelings. The small-town facility, grounds, clean rooms, excellent food, rehab/outpatient therapies and activities are genuinely strong-my mom loves it and many staff are warm, caring and communicative. But I've also seen hostile, undertrained and sometimes racist behavior, nepotism, staff gossiping about residents, denial of medical orders/therapy (only about 20 minutes of PT over six weeks in one case), failure to provide glasses/medications, falls and perceived neglect, and heavy-handed visitation enforcement. It's a nice place with great programs, but come prepared to advocate strongly for your loved one.

    Summary review

Overall impression: The reviews for Accura HealthCare of Tekamah are polarized, with a mix of highly positive experiences and serious, specific complaints. Many reviewers praise the facility for its cleanliness, caring staff, strong rehab and therapy offerings, good food, and a warm small-town atmosphere. However, a significant number of reviews recount troubling incidents involving neglect, denials of prescribed care, and problematic staff behavior. This creates a picture of a facility that can provide excellent, personalized care under some circumstances but also shows inconsistent performance and management shortcomings that have led to harm in other cases.

Care quality and therapy: Several reviewers highlight excellent rehabilitative services — daily physical therapy, outpatient therapies, and a structured rehab program that yields good outcomes. There are also positive notes about mental-health support (monthly psychologist) and tailored activities that engage residents. Conversely, other reviews report severe lapses in clinical care: residents not given prescribed medication or specialist-ordered treatments, minimal physical therapy (e.g., only 20 minutes over six weeks), failure to provide eyeglasses, and neglect of simple but critical tasks such as applying compression socks. Some of these failures are linked to negative outcomes including falls, stitches, bruises, and choking episodes. The contrast suggests significant variability in clinical consistency and adherence to care plans.

Staff behavior and culture: Staff impressions are highly mixed. Many reviewers describe staff as wonderful, kind, quick to communicate, and attentive — qualities that contribute to family satisfaction and residents feeling at home. Yet multiple reviews describe hostile dynamics among employees, allegations of racism and skin-color bias, gossip, false accusations, and protective cliques. Nepotism and family-based hiring are also mentioned, which some reviewers feel undermines professional standards. There are repeated concerns about undertrained or rude nursing staff and a perceived need for compassion training. These conflicting reports point to pockets of very good caregiving alongside cultural and interpersonal problems that can directly affect resident welfare.

Facilities, dining, and activities: The physical environment generally receives praise: clean rooms, well-kept grounds, and an overall solid building. Food and dining are frequently praised for quality and the ability to customize menus, and activities are described as plentiful and tailored. At least one review notes the facility is older but has recently been sold with planned updates, indicating potential upcoming improvements. However, some reviewers reported dirty conditions and lapses in basic upkeep, indicating inconsistency in environmental standards.

Safety, visitation, and family experience: Several reviews mention strict visitation restrictions and enthusiastic enforcement of safety measures. Some families appreciate the safety focus and cooperate with policies, while others feel visitation rules are onerous or enforced unfairly. Communication is generally reported as good in many cases, with families noting quick responses and understanding staff. Still, the reports of denied medical orders, insufficient therapies, and missed protective measures (glasses, mobility aids, compression garments) have led to family distress and loss of trust for some.

Management and operational concerns: Multiple reviews point to management problems — poor oversight, denial of specialist orders, minimal therapy allocation, and alleged favoritism. These operational issues are often cited as the root cause behind clinical lapses and staff culture problems. Where management and staff are aligned and responsive, families report very positive outcomes; where oversight is lacking, reviewers report harm and neglect.

Patterns and recommendations for prospective residents/families: The dominant pattern is inconsistency — the facility can deliver high-quality, compassionate care with strong rehab offerings, but there are also specific, repeatable concerns that have led to serious negative outcomes for other residents. Prospective residents and families should perform targeted due diligence: visit multiple times at different hours, ask about actual therapy minutes and therapy schedules, confirm policies for glasses and prosthetics, inquire how medication and specialist orders are handled, ask about staff training and anti-discrimination policies, observe mealtime and assistance for special diets, and review the grievance/incident reporting process. Asking to speak with current family members or recent discharges about consistency of care and staff responsiveness may reveal whether the positive or negative patterns are more typical at a given time.

Bottom line: Accura HealthCare of Tekamah receives strong praise in several areas — cleanliness, rehab and therapy services, activities, food, and caring staff — but also harbors recurring and serious complaints involving neglect, denial of prescribed care, staff misconduct, and management failures. Experiences appear highly contingent on staffing, management oversight, and possibly shift-to-shift variation. Families should weigh the facility’s positive offerings against the documented risks, verify current conditions and leadership practices in person, and monitor care plans closely if choosing this facility.
