    Large rooms, caring staff, inconsistent

I toured and moved my father-in-law here because the place is very nice - large, clean rooms with lots of windows - and the staff are cheerful, kind and genuinely caring; they've helped him regain some independence while being available to help when needed. There are many activities, but they're largely geared to advanced dementia; I'd like to see more programs that encourage independence for less-impaired residents. My main concern is enormous staff turnover and inconsistent care, which makes me hesitant to fully recommend despite the friendly, helpful team.

    Summary review

Overall sentiment in the reviews is mixed but leans positive about the human side of care and the physical environment. Multiple reviewers emphasize that staff are kind, genuinely caring, and helpful — they "look out for" residents and are available to assist when needed. Reviewers describe staff as cheerful and note a friendly resident community. The facility itself receives consistent praise: rooms are reported as large, clean, nicely maintained, and bright with lots of windows; the site is described as a very nice place with informative tours that include duplex options. There is also a note about flexible meal scheduling which some families appreciated.

Care quality and staff behavior are the most commonly highlighted strengths. Several comments point out that staff encourage independence and self-reliance, and that this approach had a measurable positive impact on a family member's sense of autonomy. This suggests that, for many residents, the care philosophy promotes functioning and dignity. At the same time, reviewers indicate staff are attentive and "doing all they can," which reinforces the perception of a compassionate caregiving team.

However, a significant concern raised across reviews is staffing stability and consistency of care. One reviewer explicitly mentions "enormous staff turnover," and others point to resulting inconsistencies in how residents are cared for. These issues are serious because high turnover can undermine continuity, resident-staff relationships, and overall reliability of daily care routines. A couple of reviews go so far as to say they would not recommend the facility, tying that stance to the variability in care quality related to staffing changes. This pattern is a central caveat for prospective families: while individual staff members may be excellent, frequent personnel changes appear to create uneven experiences for residents.

Programming and activities receive mixed feedback. On the positive side, reviewers note that there are many activities and that programming is specifically tailored for residents with advanced dementia. That focus can be beneficial for people with greater cognitive needs and suggests staff have expertise in memory-care programming. Conversely, several reviewers express a desire for more activities suited to residents with less advanced dementia — those who retain higher levels of independence and engagement. This highlights a potential gap in the activity mix: strong offerings for advanced memory-care needs, but less stimulation or variety for higher-functioning residents who still need social and cognitive engagement.

Management and operations concerns center on turnover and care inconsistency rather than cleanliness or facility upkeep. The physical plant and day-to-day environment are described positively and as well cared for; the main operational weakness is staffing continuity. From the reviews provided, there is no repeated mention of food quality, clinical competence, medication errors, or safety incidents — the complaints are specifically about staffing continuity and program fit. Nevertheless, the taste of some reviewers that they would not recommend the facility indicates that, for certain families, staffing instability weighed heavily in their overall judgment.

For families considering Fountain Park Assisted Living and Memory Care, the reviews suggest a few practical next steps: visit and observe staff-resident interactions during a tour; ask management about current staff turnover rates and what retention strategies they have in place; inquire how the facility maintains care consistency across staff changes (e.g., handoff protocols, primary caregiver assignments); request a sample weekly activity calendar and ask how programming is adapted for residents at different cognitive and functional levels; and, if possible, speak with current residents’ families about their long-term experiences. These targeted questions will help clarify whether the strong points noted in reviews — compassionate staff, encouragement of independence, and a pleasant facility — are likely to be consistently delivered in your prospective loved one's situation, or whether staff instability might undermine those benefits.

About Fountain Park Assisted Living and Memory Care

Fountain Park Assisted Living and Memory Care sits at 1433 North Main Street in Bryan, Ohio, right next to 1391 North Main Street, offering care for older adults needing different levels of support, so you'll find assisted living, memory care, independent living, short-term respite stays, and even adult day care under the same umbrella. This place has nurses on-site 24 hours a day, a full-time activity director who keeps things lively, and an emergency alert system in every room, which means there's always help if someone needs it. Their assisted living is for people who want to stay social and active but need help with daily things like dressing or bathing, and the community welcomes pets like cats and dogs because many folks feel better with their animals by their side.

The Memory Care wing, which sits in its own building, is for folks with Alzheimer's or dementia, and here residents have bracelets that set off alarms if they wander, and the staff use a computerized system to know if someone goes somewhere unsafe, and all staff who work here get training just for dementia care, so people who get easily confused or might act out get extra, specialized support, plus families know their loved ones are in a locked and secure area. Residents with diabetes or incontinence get help with special care plans-there's medication management, help with using the restroom, and even insulin shots when needed, and they're able to care for people who are at risk for falls or need a one- or two-person transfer, with mechanical lifts when necessary.

In terms of daily life, folks can spend time in both indoor and outdoor areas like fenced courtyards, walking trails, a rockers-lined front porch, living rooms, a TV space, and an arts room, and the place really tries to feel homey, with things like ice cream parlors, a library, gardens, and lots of programs, including bingo nights, ice cream socials, gentle exercise classes, card games, gardening, and outings, so no one has to feel bored or stuck inside. There's a full-service beauty salon and barber shop, an activities room, pet therapy visits, and a courtyard, and the team encourages resident-run activities and house social events. People can pick a private, semi-private, or deluxe apartment-furnished or bring their own things-and every room has safety features like walk-in showers and emergency call buttons, plus Wi-Fi, telephones, and individual controls for heating and cooling.

The dining program means you can get restaurant-style meals all day, including choices for allergies or diabetes, and there's a cozy dining area for people who like calmer spaces, and maintenance, laundry, and housekeeping are included, so it's less to worry about. They provide transportation to doctor's visits, on-site medical services like dental or podiatry, hospice and respite care if needed, devotional services, and coordination with other healthcare providers, so residents can stay as their needs change without having to move again. Fountain Park takes assisted living waiver clients and is certified by the Ohio Department of Aging; the staff-like Joni Engels and Angel Thorpe-work to make sure everyone, from mostly independent folks to those with advanced memory loss, gets the support that fits them best. The focus is always on helping people stay as active and comfortable as possible, with programs and technology that keep everyone safe, encourage friendship, and provide peace of mind for both residents and families.
