    Unsafe neglectful filthy abusive care

I stayed/visited my loved one here and it was awful. Staff were often unresponsive, neglectful, and sometimes abusive - residents left unsupervised, missed or late meals (cold or burned), medication errors/delays and missed appointments were common, and basic hygiene/cleaning was neglected. A few caregivers were kind, but too many aides were careless, rude, even dishonest; the place felt unsafe and filthy. I would not trust them with a loved one - in my opinion it should be shut down.

    Summary review

Overall sentiment in the reviews is strongly negative with recurring, serious concerns about clinical care, safety, and facility operations, though a minority of reviewers note positive, caring staff and acceptable conditions. The dominant themes are medication mishandling, neglect of basic needs (including nutrition and personal care), unsanitary conditions, and a culture among some staff of apathy or unprofessional behavior. Several reviews describe severe outcomes (extended hospital stays, ER visits, inability to wake residents, and at least one death), which heightens the severity of the concerns expressed.

Care quality and clinical management: Multiple reviewers report medication errors, delays, and withholding of medications. Specific examples include a Tramadol mix-up where medication intended for one patient was given to another, failure to administer IV antibiotics, and general reports of medications being given and not followed up on. Reviewers commonly describe canceled or missed medical appointments and unresolved medical issues, and some recount being sent from the facility to the emergency room. These descriptions point to inconsistent clinical oversight and serious lapses in safe medication administration and care coordination.

Staff behavior, culture, and responsiveness: Reviews paint a picture of a deeply inconsistent staff culture. On one hand, several reviewers praise particular nurses, aides, and the activities director for being attentive and loving; on the other hand, many reviewers recount aides ignoring call lights, not performing basic duties, or prioritizing lunch and extra food over resident care. There are extreme allegations of staff smoking marijuana in the parking lot, staff theft/financial exploitation, rude and unhelpful front desk behavior, and abusive treatment. These reports suggest both pockets of conscientious personnel and systemic issues with professionalism and accountability.

Dining, nutrition, and feeding: Nutrition-related complaints are frequent and specific. Reviewers allege residents were not fed for extended periods (including whole weekends), diabetic diets were refused or not followed, and meals were served late, cold, or burnt. Several reviewers reported small portions or inadequate juice servings and overall concern about residents losing weight or being 'starving.' A minority of reviews, however, report adequate portions and weight gain, again reinforcing the pattern of inconsistency in service quality.

Facility cleanliness and safety: Multiple reviews highlight unsanitary conditions — dirty rooms, dust, spider webs, filthy wheelchairs — and describe the facility as 'filthy' or 'disgusting.' Safety concerns include residents left unsupervised, falls and injuries, inability to wake residents, and general neglect that some reviewers say presents a real risk to vulnerable patients. These safety and hygiene complaints, combined with the clinical failures reported, form a consistent narrative of lapses in basic environmental and patient safety standards.

Management, communication, and accountability: Reviewers frequently call out poor communication and unresponsiveness from staff and management. Complaints include being hung up on by the front desk, abrupt or unhelpful communication, and lack of follow-through after incidents or medication administration. Several reviewers explicitly call for regulatory action or facility shutdown, reflecting a loss of trust in the facility's ability to self-correct. At the same time, a few reports of good initial impressions and capable staff suggest variability in management effectiveness across shifts or units.

Patterns and takeaway: The reviews collectively indicate an inconsistent standard of care where some staff members provide good, attentive service while systemic issues—medication safety, nutrition, sanitation, staff professionalism, and communication—are repeatedly problematic. The volume and severity of reported incidents (medication errors, unmet dietary needs, unsanitary conditions, alleged substance use and theft, missed medical care, and serious clinical outcomes) warrant caution. For families considering this facility, the reviews suggest the need for careful, ongoing oversight: verify medication handling and administration procedures, ask about staffing levels and training, confirm dietary accommodations in writing, and monitor cleanliness and responsiveness. The mixed reports mean positive experiences are possible but are not guaranteed, and the prevalence of critical safety and neglect allegations is significant and should be weighed heavily.

    About Riverside Healthcare Center

Riverside Healthcare Center sits at 1390 King Tree Dr in Dayton, Ohio 45405, and belongs to the family-owned CommuniCare group, which runs many post-acute care centers across the country, and this facility provides different types of care for seniors, including skilled nursing, long-term care, memory care, assisted living, and short-term rehabilitation, with services like physical therapy, occupational therapy, and speech therapy for those recovering between the hospital and home, and while it's known for taking care of very frail people who are dependent on nursing care, it also has some community-living options, like independent living and assisted living, but specifics about these aren't always clearly listed, and you get room choices like studios or suites, and folks get access to programs that support both emotional and physical well-being, so you'll sometimes hear about bingo nights, crafts, and campus walks happening for residents. There's a focus on person-centered care, which means care tries to match each person's needs, and they've got programs that deal with medical issues like wound care, hospice, and palliative care, and the facility uses equipment for many healthcare needs, but there isn't much public information about the other features you'd find, so you won't see anything official about things like patios, pools, or parking. The nursing home section has a 1-star rating from the Centers for Medicare & Medicaid Services and averages 4.9 out of 10, so people looking at this want to weigh that when choosing a place. Riverside Healthcare Center does have some legal protections in place for things like gender identity, orientation, and different sources of income, and it's associated with Trinity Health Plan as well, but when it comes down to the little details, like what's outside or specific apartment policies, those aren't really listed. The center is off-market right now and isn't up for sale or rent, but it's part of a bigger apartment community in Dayton and aims to create a good experience for residents wherever possible.
