    Warm caring staff, some oversight

I've had a largely positive experience - the nurses, therapists and aides are caring and treat residents like family, therapy is excellent (beautiful gym) and the activities team keeps everyone engaged with gardening, outings, bingo and more. Dining is top-notch and the place feels warm, safe and resident-centered, great for rehab or long-term stays. That said, I've seen some unprofessional/agency aides, occasional neglect and things go missing, so leadership could be firmer. I'd recommend touring to judge for yourself.

    Summary review

Overall sentiment in the reviews is mixed but leans positive: many reviewers emphasize compassionate, high-quality care while a notable minority report serious concerns about inconsistent staffing and occasional neglect. The most frequent praise centers on the nursing staff, therapists, activities team, and dietary department. Multiple reviewers describe staff as "treating residents like family," and highlight clinical competence and attentiveness. Several specific staff members are singled out for praise (for example, an Allison who helps with transportation, shopping, cooking and outings, a Susan who runs bingo, and an Andrea who leads arts programming), which suggests that individual caregivers and activity staff make a strong, positive difference in residents' daily lives.

Care quality and clinical services receive consistent positive mentions. Reviewers point to a walking-focused rehabilitation program, a spacious and attractive therapy gym, dedicated and fun therapists, and successful short-stay rehab outcomes including job-placement assistance in the community. Statements such as "great for rehabilitation" and descriptions of therapists as "top-notch" or "dedicated" indicate that rehabilitation and therapy services are a clear strength. The facility is also described as providing compassionate long-term care, with an emphasis on dignity, resident-centered practices, and teamwork led by nursing leadership (Director of Nursing noted positively in some reviews).

Activities and social programming are another prominent theme. Multiple reviewers commend the activities team for engaging, creative programming—arts projects, an art festival, bingo, gardening, balloon tosses, fishing, and meal outings are all mentioned. These programs are characterized as meaningful and helpful, contributing to a warm, lively atmosphere for residents and helping people reconnect with family and community. The presence of creative, proactive activities staff appears to be a differentiator that many families and residents appreciate.

Facilities and dining also receive favorable comments. The therapy gym is described as "beautiful and spacious," and the dietary department is called "top-notch." Several reviewers explicitly encourage touring the facility, suggesting that the physical environment and amenities are attractive, comfortable, and supportive of rehabilitation and daily life.

At the same time, a clear and important counterpoint appears in several reviews: inconsistent staffing quality and concerns about agency aides. Multiple reviewers report problems with agency staff who are perceived as lazy or unprofessional, and there are reports of personal items going missing and a "clicky" staff culture. More serious allegations include instances of neglect—ignored call bells, lack of attentiveness to residents’ needs, and family members encountering situations (for example, needing to change a roommate's diapers) that made them feel the staff were inattentive. Some reviews express strong negative opinions of leadership, calling it incompetent or profit-driven. These negative accounts are fewer than the positive ones but are severe enough that they represent a significant pattern to investigate further.

The reviews together paint a picture of a facility with many strong elements—compassionate nursing, robust therapy and rehab services, active and creative programming, and positive social atmosphere—but with uneven execution in staffing consistency and management. A reader should note that experiences appear polarized: many families and residents praise the place highly, while some report troubling lapses in day-to-day care and professionalism. If considering this facility, it would be prudent to ask about staff turnover, the use and supervision of agency aides, incident/response procedures for call bells, leadership stability, and to request a tour and meetings with therapy, activities, and nursing leadership to confirm current performance and staffing practices.

About Concord Care Center of Sandusky

Concord Care Center of Sandusky sits on West Strub Road in Sandusky and holds up to 50 licensed beds, although there tend to be about 45 residents each day, and the place is part of the Concord Care & Rehabilitation Centers group and tied in with Aom Healthcare, and the folks who run it, with names like Samuel Sherman, Alexander Sherman, Tzvi Sherman, Yehuda Sherman, Hindy Amsel, Jeffrey Goldstein, and Batsheva Tratner, have each held a fair piece of ownership for a while, and some, like Jeffrey Goldstein and the Shermans, have managed it since 2015. They offer skilled nursing with 24-hour care and services from a team of physicians, social workers, and nutritionists, and they help both short-term and long-term residents, whether they're recovering or they're needing steady support, and they also give families some relief through respite care if someone just needs a break for a short stay. They also focus a lot on rehabilitation, with programs aimed at helping folks regain some strength or get ready to move along to the next level of care, and they've got spaces and routines that try to keep everyone as comfortable as possible, with some events like music activities and brain teasers on an activity calendar to keep things moving. The place does accept both Medicaid and Medicare, along with private insurance, and they've got English-speaking staff and amenities designed to be home-like, and regular health maintenance like massages and what they call back tune-ups happen onsite. The nurse staffing sits at 3.04 hours per resident each day, which falls below the Ohio state average of 3.7, and the turnover among nurses is on the higher side at 56.3% compared to the state's 51.0%. Inspections haven't been perfect either, with 33 total deficiencies, including problems found in infection control, resident rights like dignity and communication, and safety-or accident prevention-under quality of care, though the team there has established infection-control protocols that have been worked on after being cited. You'll find it's not accepting new patients at the moment, but if someone's staying there, they receive the kind of medical and support services that are pretty common in nursing facilities, always with a focus on both everyday needs and some ways to keep minds and bodies active.
