Five Counties Nursing Home - Assisted Living Center is a well-established skilled-nursing care facility located in the heart of Lemmon, South Dakota. Surrounded by a community that takes pride in its rich history and Western heritage, the center offers residents an environment that balances the warmth of small-town living with a standard of professional health care. The town of Lemmon, recognized for landmarks such as the Petrified Wood Park, Grand River Museum, Hugh Glass Memorial, the Kokomo Gallery and Studio, and the picturesque Shadehill Reservoir, provides a lively setting with plenty of recreation and activities for both residents and their families to enjoy.
The facility itself has deep roots in the region’s commitment to healthcare. Its beginnings can be traced back to November 15, 1951, with the opening of the Five Counties Hospital, at the time the most advanced medical facility within a five-county area, offering 44 beds, multiple surgical wards, labor and delivery rooms, and 24-hour care. On September 9, 1962, the center expanded to include a dedicated nursing home addition, further enhancing the level of service provided to the community. Over the years, through changes in management and the resolve of dedicated local leadership, the facility has thrived and continued to serve as a crucial resource for area residents, now owned and operated by a non-profit corporation known as The Lemmon Area Medical Association.
