    Caring staff, but safety concerns

I love the warm, small-town, home-like feel and the compassionate, hardworking staff - nurses, CNAs and cooks genuinely care and often go out of their way to support residents. Since new management arrived I've noticed worrying administrative and kitchen issues: poor communication, rude phone staff, staff distracted by phones, food reimbursement problems, and on occasion residents left soiled or neglected for hours (I'm aware of a serious UTI/sepsis case and an unexplained disappearance). I'd recommend this place for the outstanding caregivers, but only if you're prepared to advocate strongly for your loved one because leadership and safety need improvement.

    Summary review

Overall sentiment in the collected reviews is mixed but leans positive around direct caregivers and the facility atmosphere, while containing several serious and specific negative allegations that merit attention. A large number of reviewers praise the frontline staff — nurses, CNAs, cooks and other caregivers — describing them as compassionate, hardworking, and attentive. Multiple reviews highlight 24/7 nursing availability and staff who go out of their way for residents, contributing to a sense of family-style care and residents and families feeling treated with respect. The building itself is often described as smaller and more intimate, with a home-like, small-town feeling, clean rooms, a quiet neighborhood, and activities that support closer relationships and more individualized attention. Several reviewers explicitly recommend the facility and call it the best place for their loved ones, citing a positive culture and teamwork among employees.

Despite the many positive comments about direct care and atmosphere, there are recurring and serious concerns in several reviews. Some reviewers allege neglect, specifically that residents were left sitting soiled for long periods of time or confined to wheelchairs for extended parts of the day. A few reviews report serious medical outcomes tied to care issues, including urinary tract infection, sepsis, and at least one hospice death characterized as due to poor care. These are not isolated minor complaints and, because they concern resident safety and clinical outcomes, they stand out as high-priority issues that prospective families should investigate further.

Management, communication, and safety present mixed signals across the reviews. While some reviewers praise management as hardworking and supportive, others report a lack of care from the administrator and note that the facility is under new management. Several complaints focus on poor communication and rude phone staff, including a disturbing report about a disappearance or escape with inadequate information provided to family members. There are also mentions of staff being distracted by phones and of rude behavior toward both staff and residents. Together these items suggest inconsistency in leadership, staff accountability, and incident communication practices.

Dining and kitchen operations are also a mixed area. Multiple reviewers say the food is good and that the cooks are caring, but other comments point to kitchen issues and problems with reimbursement or availability of food expenses. These complaints suggest variability in meal service or in administrative handling of related financial matters, and they could be symptomatic of the broader inconsistencies reported elsewhere in operations.

Patterns across the reviews show a clear split: direct caregivers and the day-to-day, resident-facing experience are frequently praised and often called exceptional, while administrative oversight, communication, and certain safety/clinical incidents are the most common and most serious negative themes. Because the complaints include allegations of neglect and adverse clinical outcomes, they should be viewed as significant concerns rather than minor service issues.

Recommendations for families considering Evergreen Healthcare Center based on these reviews: visit in person multiple times (including evenings/weekends), speak directly with nurses and CNAs about staffing levels and daily routines, ask for documentation on recent incident reports and how escapes or disappearances are handled, request information on infection control protocols and clinical oversight (especially related to UTIs and sepsis prevention), inquire about management turnover and how complaints are escalated, observe mealtimes and activities, and review state inspection and licensing reports. The facility appears to offer strong, compassionate hands-on care in many cases, but the presence of repeated, serious negative reports means due diligence is especially important before making placement decisions.
