    Caring staff serious systemic issues

I had a mixed but memorable stay: the nursing and therapy teams (especially Nicole, the BOM, and several long-tenured staff) were kind, professional, went above and beyond, and the place felt home-like with a clean facility and an on-site rehab gym-overall better than other facilities I've seen. However, the building is outdated, billing and management were problematic, staffing shortages and rude management created serious safety and care concerns (reports of therapy gaps, missing care plans, elopement and choking incidents, mail/laundry issues), and it's expensive (~$6,000/month) with a 1-star CMS rating. I appreciate the wonderful staff but would be cautious because of the systemic problems.

    Summary review

Overall impression: The reviews for Floresville Nursing Center are highly polarized. Many reviewers emphasize warm, committed frontline caregivers and a close-knit, family-like atmosphere where staff — particularly long-tenured employees and a named business office manager (Nicole) — are praised for dedication, kindness, and going above and beyond. At the same time, multiple reviews raise very serious, systemic concerns about management, safety, clinical oversight, and administrative reliability. The juxtaposition of strong day-to-day staff performance with alarming reports of neglect, abuse, and operational failures produces a conflicted picture: families may find exceptional individual caregivers but also significant organizational risks.

Care quality and safety: Several reviewers describe excellent hands-on care, especially for rehabilitation after surgery, noting an on-site rehab gym and an effective therapy/nursing team. However, other reviews report critical lapses in clinical care and resident safety. Specific incidents cited include a choking event, a resident who eloped and was not found for hours, and allegations of neglect and abuse. One reviewer reported a CNA with assault charges. Other operational issues tied to care quality include claims that therapy was not provided as expected and that individualized care plans were absent. These safety and clinical concerns are reinforced by the facility’s CMS 1-star rating mentioned in the summaries, indicating serious regulatory or quality problems that prospective families should investigate further.

Staff, culture, and workplace environment: A clear strength in many reviews is the presence of long-tenured, experienced staff who create a welcoming, home-like environment and help ensure smooth transitions for new residents. Multiple reviewers call out staff kindness, professionalism, and a team-oriented culture; some staff even describe it as the "best workplace ever." At the same time, there are contradictory reports about management and organizational culture: some describe rude or "nasty" management and an "ignorant" administrator, as well as poor treatment of agency nurses. These conflicting accounts suggest uneven leadership and potentially variable conditions depending on unit, shift, or department.

Facilities and services: Reviewers commonly state the facility is clean but also note the building is outdated and in need of updates. A positive operational point is the availability of on-site rehabilitation and a rehab gym. Negative service-related items include claims that the kitchen lacked a dietitian, issues with mail privacy (mail opened), and reports of laundry theft or loss. There is also at least one minor maintenance complaint (a shredded flag needing replacement) that, while small, contributes to an impression of inconsistent facility upkeep.

Administration, billing, and oversight: Several reviews raise concerns about administrative competence and reliability. Specific complaints include serious billing problems, costly monthly rates (cited around $6,000/month), and poor physician oversight. The business office has a praised individual (Nicole) who is described as tireless and helpful, but broader comments about rude management and significant billing issues suggest administrative dysfunction in other areas. The facility’s low CMS rating further signals the need for scrutiny of regulatory compliance and oversight.

Patterns and contradictions: The strongest and most frequent positive theme is the quality and dedication of many frontline staff members; this appears repeatedly and is the reason multiple reviewers recommend the team for rehabilitation and compassionate care. The strongest negative themes are severe safety incidents, allegations of neglect/abuse, management and administrative failures, and billing/cost concerns. The coexistence of devoted caregivers and serious system-level failures suggests that while individual employees may perform admirably, organizational leadership, policies, or resources may be insufficient to ensure consistent, safe, and compliant care across the facility.

Implications for families and decision-makers: Based on these reviews, Floresville Nursing Center presents a mixed risk/benefit profile. Prospective residents and family members should weigh the reported strengths in hands-on caregiving and rehabilitation services against the documented safety incidents, the facility’s CMS 1-star rating, and administrative concerns (billing, privacy breaches, staffing shortages). Important due diligence steps—consistent with the issues raised—would include asking for evidence of current staffing levels and turnover, review of recent inspection and deficiency reports, documentation of care plans and therapy schedules, clarification of billing practices and rates, and direct questions about any reported incidents (elopements, choking, staff criminal history). Observing a care shift in person and speaking with both long-tenured caregivers and management may help reconcile the polarized experiences described in these reviews.

Bottom line: There are clear strengths—compassionate, experienced frontline staff, on-site rehab, and positive interpersonal experiences—that make the facility attractive for some types of rehabilitation and daily care. However, multiple red flags related to safety, management, billing, and regulatory standing are serious and must be resolved or clarified before making placement decisions. The mixed nature of reviews suggests that individual staff members can provide excellent care, but systemic problems could compromise resident safety and administrative reliability unless addressed.
