    Compassionate staff but safety concerns

I had a mixed experience. Many nurses, CNAs and PT/OT therapists (Alexis's CNA class was excellent) were compassionate, skilled and service-oriented - residents were often well cared for, the rehab and activities were good, and the food and cleanliness were praised. But under new management I also saw serious safety and maintenance problems (unattended or incontinent residents in common areas, broken equipment, pests, plumbing/electrical issues), inconsistent staffing and inappropriate staff behavior, so leadership needs to fix those gaps before I'd fully trust the place.

    Summary review

The reviews for Jourdanton Nursing and Rehab present a highly polarized picture with strong, repeatable praise alongside serious, recurring criticisms. On the positive side many reviewers highlight dedicated, compassionate caregivers and skilled therapy staff. Multiple comments single out the physical and occupational therapy teams as effective, and several former patients or families report successful rehab outcomes (including hip replacement recovery). A number of reviewers praise the staff as welcoming and supportive, note that residents are well cared for and safe, and credit new or responsive management (including an ADON named Nikki) with improvements. The facility is also described by some as very clean, with good food and a meaningful activity program that includes daily activities and occasional outings. In addition, the facility offers an appreciated CNA training program and instructors (Alexis is named several times), which reviewers described as informative and career-enhancing.

Despite the positive reports, an equally strong and troubling set of complaints appears across the summaries. Many reviewers report neglectful conditions: residents left unattended in hallways or living rooms, patients found wet or in dirty clothes, and staff using inappropriate language. Several accounts raise concerns about staff quality and turnover — including claims that good, caring employees were removed — which contributes to inconsistency in care. Hygiene and maintenance issues are repeatedly mentioned: roaches, persistent bad smells, and descriptions of the building as run-down or a "dump." Concrete infrastructure problems (electrical and plumbing failures, broken equipment, hazardous hallways) and shortages of supplies were also described, some to the point where reviewers suggested the facility should be closed or demolished.

Dining and daily life are another area of mixed feedback. While some reviewers rave about the food, others complain about no hot meals, late meal service, and small/poorly loud TVs in common areas. Activities are a bright spot for many patients, but entertainment and amenities appear uneven. Safety and trust issues are raised beyond basic cleanliness: concerns about staff backgrounds, reports of family member scams, and instances of what reviewers perceived as an unsafe environment indicate broader worries about oversight and resident protection.

Taken together, the reviews point to a facility with significant variability in resident experience. When staffing, management, and therapy teams are engaged and consistent, residents report excellent care, successful rehabilitation, and a supportive atmosphere. When staffing is inconsistent, or when maintenance and supply problems arise, reviewers describe neglect, hygiene failures, and safety hazards. The pattern suggests systemic instability — possibly tied to staff turnover, management changes, or resource constraints — producing starkly different outcomes depending on time, unit, or shift.

In summary, Jourdanton Nursing and Rehab receives high marks for its clinical rehab capabilities, some very committed caregivers, and a strong CNA training program, but it also faces multiple serious operational problems reported by other reviewers: inconsistent staffing and supervision, neglect of residents, cleanliness/pest and maintenance failures, irregular meal service, and safety concerns. These mixed reports warrant careful follow-up: verifying which units/shifts are delivering the praised care, addressing pest control and infrastructure repairs, ensuring adequate supplies and equipment, stabilizing staffing and retaining skilled caregivers, and strengthening oversight and background checks to rebuild consistent, safe care for all residents.

    About Jourdanton Nursing and Rehab

Jourdanton Nursing and Rehab is the only nursing home in Jourdanton, located at 1504 Oak St, and has room for up to 60 residents in a single-story building with both studio and suite options, each with private bathrooms and fully furnished spaces, and while the overall nursing home gets a two-star CMS rating and a general grade of D, its most recent facility inspection earned an A+ and it does best in regulatory reviews, though occupancy sits around 67 percent lately, probably changing a bit since the recent shift in ownership and staff. Residents will find registered nursing, therapy, and many medical professionals on site daily, with skilled nurses, a medical director, social workers, pharmacists, respiratory therapists, and dieticians offering both long-term and short-term care, plus specialized services like stroke recovery, wound care, amputee rehab, cardiac and respiratory therapy, dentistry, podiatry, laboratory work, x-rays, and pain management, which means residents get help for a wide range of needs if recovering from injury, surgery, or living with illness over time. The facility has a resident council and a family council so voices get heard and concerns can be shared, and it participates in welfare programs, mental wellness support, and concierge services, while memory care services offer a safe place for seniors living with Alzheimer's or dementia and programs to help with memory and daily routines. Staff provides help with baths, dressing, eating, personal hygiene, medicine management, and transfers, and nurses are present 12 to 16 hours a day, with a 24-hour call system for emergencies. Therapy is available, including physical, occupational, speech, and recreational therapies, and there's inpatient rehab for post-hospital recovery from things like strokes, heart attacks, or injury. The facility takes both Medicare and Medicaid, allowing for both private pay and public options. Amenities focus on fitness and recreation, with a garden, an outdoor area, community dining, a fitness room, a game room, a small library, a salon, and activity spaces, while housekeeping, linens, air conditioning, cable TV, kitchenettes, transportation (including for non-medical trips), and restaurant-style meal service are all included, offering meals tailored to dietary needs, like diabetes diets. The home organizes scheduled daily activities, resident-run programs, planned trips, and events, aiming to build community and keep people engaged, and support is given for a smooth move-in if someone's transitioning into care. The nursing home isn't part of a retirement community, but it does belong to the Sava Senior Care network. The facility's goal is to improve quality of life for seniors by offering a warm, safe setting, focusing on respect, experienced staff, and personal care plans, going week by week and day by day to help each person regain strength and independence, whether someone needs short-term rehab, skilled nursing, memory support, or longer-term help with daily life.
