    Caring staff but poor administration

I'm grateful the direct-care staff treated my loved one like family, gave loving, above-and-beyond care (they even arranged a private graduation), and therapy/recreation were decent. But administration was unresponsive and incompetent, staffing shortages and some nurses neglected patients, and the facility felt run-down - strong odors, flies, dirty rooms, broken/missing equipment, and too few activities.

    Summary review

Overall sentiment in the reviews is mixed but leans toward concern: several reviewers praise individual staff members for compassionate, family-like care and recount specific positive events (for example, staff arranging a private graduation ceremony and school participation). Physical and speech therapy services are present, a recreational room and a large common room for activities like dominos and bingo exist, and multiple reviewers indicated the food was acceptable. These positive notes indicate that parts of the staff and some facility offerings can support residents well and that some families have had good experiences.

However, the most frequent and serious themes are consistent negative reports about staffing, cleanliness, facilities, and management. Multiple reviewers report staffing shortages, high turnover, and inconsistent caregiving quality — ranging from staff who go above and beyond to nurses described as "horrible" or neglectful. There are explicit allegations of neglect and abuse, as well as reports of residents being treated poorly or not receiving timely attention. These accounts are compounded by instances where equipment was broken (walkers, TVs), chairs went missing or were inappropriate and caused pain, and families were not notified about problems.

Facility condition and hygiene are recurring concerns: reviewers describe the environment as run down, dingy, and at times "prison-like," with strong odors, flies, dirty rooms, and soiled clothing. Small room size and limited equipment were also noted, and several comments point to systemic underfunding or constrained resources that likely contribute to the degraded physical environment. Some positive comments called the place "nice" and complimented certain staff, but the balance of comments about odors, unsanitary conditions, and broken items points to significant facility maintenance and sanitation issues that need attention.

Management and organizational issues are a distinct and prominent theme. Multiple reviewers described unresponsive administration, perceived incompetence by leadership (including specific reference to the administrator and a leader named Don), reports of employee harassment, and HR inaction when concerns were raised. These complaints are tied to staff morale problems and resignations; some reviewers explicitly connected poor leadership to lower quality of resident care. Families also reported a lack of communication and notification about incidents, which heightens distrust in leadership and reduces transparency.

Activities and therapy paint a mixed picture: while physical and speech therapy services exist and there are spaces for group activities, several reviewers felt the facility was "not rehab focused" and that there were not enough meaningful activities for residents. In short, programs and spaces are present but may not be sufficiently staffed, funded, or organized to meet resident needs consistently.

In summary, reviews reflect a facility with clear strengths in pockets of compassionate caregiving and some programmatic offerings, but overshadowed by systemic problems: staffing shortages and turnover, inconsistent clinical care, troubling sanitation and maintenance issues, and leadership/HR failures that undermine both staff and resident well-being. The most actionable patterns to address would be improving staffing stability and training, addressing sanitation and maintenance deficits, repairing or replacing broken equipment, increasing and publicizing engagement/activities, and taking concrete steps to improve leadership responsiveness and employee relations to rebuild trust with families and staff.

About Llano Nursing And Rehabilitation Center

Llano Nursing And Rehabilitation Center has 96 certified beds and sits under the management of Slp Llano LLC since April 2017, with Joshua Leonard starting managerial control in October 2024, and they try to make residents feel comfortable while making sure everyone's well cared for, and they always say they want to provide love and attention, which you can see in how they let residents shape their space, whether they're in one of the private or semi-private rooms, letting folks have a home-like feel with things that matter to them. The facility usually has about 24 residents per day, and it offers lots of health services like skilled nursing care, rehabilitation, behavioral and hospice care, memory care, wound care, dialysis, and transportation for appointments, plus you'll find long-term, respite, transitional care and other specialized services, all backed by staff who say they've been picked for their skills and caring approach even though the nurse staffing rate is about 3.56 nurse hours for every resident each day and nurse turnover has reached 69.6%, which is a detail folks may want to keep in mind. Llano Nursing And Rehabilitation Center has been cited in inspection reports 28 times overall, including two infection-related issues, and has had trouble employing enough qualified staff for food and nutrition services-like not having enough trained dieticians, and there've been violations for not always getting or preparing food properly or following professional standards, and they've also been cited for not meeting some of the rules that protect residents from infections. Some areas have been renovated with fresh paint and new floors, so it feels updated in places, and the staff uses new technologies and follows industry standards, aiming to give care that helps the physical, emotional, and spiritual lives of people living there, and they show an interest in meeting the special needs of this particular community.
