    Clean, caring, attentive, professional staff

I've had my mom here since 2016 and couldn't be happier - the facility is very clean, the staff are friendly, professional, attentive, and family-focused, with great activities and long-tenured, caring team members. They provide excellent clinical care (even for ventilator/trach patients), strong rehabilitation, low readmission rates, and they address issues immediately; I highly recommend them.

    Summary review

Overall sentiment across these review summaries is strongly positive. Multiple reviewers emphasize that Meridian Care at Grayson Square provides compassionate, attentive, and professional care. Praise is consistent for both direct care (nursing and respiratory support) and for administrative responsiveness. Many reviews express gratitude and trust in the facility, with several long-term family relationships noted (for example, a resident present since 2016 and long-tenured staff like Ms. Rico with nine years). The facility is frequently described as "awesome," "fabulous," and "very good," and reviewers often say they would recommend it.

Quality of care emerges as a standout strength. Reviewers highlight excellent clinical care including specialized services for ventilator and trach patients and a very low hospital readmission rate, indicating effective medical management and rehabilitation outcomes. Strong rehabilitation services and successful long-term care are mentioned repeatedly, with family members reporting that their loved ones are "well taken care of," receiving attentive support and showing positive improvements. The facility is credited with being professional, patient- and family-focused, and delivering consistent clinical competence.

Staff culture and leadership receive particular praise. Review summaries note caring, kind, and attentive staff with a smiling, friendly demeanor. Several comments point to staff who seem happy to work there, which reviewers tie to quality care and a positive environment. Leadership — specifically the Director of Nursing (DON) and the administrator — is described as "awesome," and reviewers cite timely resolution of issues. Examples of named staff visits (e.g., Nancy Welch) and long-serving team members provide concrete evidence of staff engagement and continuity.

Facility environment and services are also frequently lauded. The building and rooms are described as clean and free of unpleasant odors (contrast is made with a prior facility that smelled of urine), contributing to a comfortable atmosphere. Reviewers appreciate the social and enrichment offerings: activities are called "awesome," with celebrations that include residents and families. These elements reinforce a family-oriented culture and support resident quality of life beyond clinical care.

Notwithstanding the overwhelmingly positive feedback, there are some concerns to note. A small number of reviews reference staff turnover or changes, with a few comments about unqualified or unorganized staff during those transitions. While these criticisms are not the dominant theme, they suggest occasional variability in staff performance or onboarding that could affect consistency. Another point raised is that CMS ratings available to the public may be outdated, creating a potential mismatch between current performance and publicly visible metrics. These issues appear limited in frequency but worth monitoring by prospective families.

In summary, the aggregated reviews portray Meridian Care at Grayson Square as a clean, well-managed facility with strong clinical capabilities (notably for complex respiratory care), effective rehabilitation, and a warm, family-focused culture. Leadership is trusted, staff are generally described as kind and professional, and activities promote resident engagement. The primary caveats are sporadic staff changes and a few reports of unorganized or underqualified personnel during those periods, plus concerns about outdated external ratings. Overall, the dominant pattern is one of reliable, high-quality long-term and skilled nursing care with many satisfied family members and recommenders.

About Meridian Care at Grayson Square

Meridian Care at Grayson Square sits at 815 E Grayson St in San Antonio, Texas, and has 90 beds in a nursing home setting with options for one to four bedrooms, including rentals like apartments, townhomes, and condos, plus independent and assisted living spaces like cottages and studios, so there's a mix of private and shared spaces across the campus, and the apartments let residents bring their pets if they want. The place isn't listed for rent or sale publicly and there's no clear information about what's included with utilities or special features in each home, but each unit does come with private bathrooms, a balcony or patio, and controlled air and heat, which makes things a bit more comfortable, and while it's not clear what kind of parking or entryways are included, there's outdoor space, gardens, walking paths, and places where people can gather outside which can be really nice for fresh air or just having a walk. The community offers a full schedule of meals with a professional chef and all-day dining, three meals a day, snacks, plus options for special diets like for allergies or diabetes, and you get housekeeping, laundry, and family support services without extra complications. You'll find things like a fitness room, wellness room, library with computers, beauty salon, barbershop, a little convenience store, arts and music rooms, theater, game and activity rooms, plus cable TV, a kitchenette, telephone, air conditioning, Wi-Fi, furnished rooms, and spaces for socializing. There's always plenty to do since there are community activities every day, move nights, and wellness programs, and people can use massage therapy or emergency medical care if needed. The staff provides skilled nursing and long-term care and helps with daily tasks like bathing, dressing, and moving about, covering needs for folks who need a little support or those who are mostly bed-ridden and need round-the-clock supervision, so the care team can help with medication, personal care, incontinence, diabetes, catheter or colostomy management, and hospice and respite stays, including speech, physical, and occupational therapies. The staff offers psychological support and resident/family counseling, and there's also a 24-hour call system and 12 to 16-hour hands-on nursing care available, so supervision and support are pretty consistent. The place pays attention to legal protections around gender identity and orientation, and smoking is allowed on site. Visitors and residents alike will notice the mix of social, recreation, and wellness opportunities, but there's not much detail about any further amenities or services that might be available, and the community has a good Medicare rating-4 out of 5 stars-and has been certified for Medicare and Medicaid since 1996. There have been 10 deficiencies but no fines, penalties, complaints, or payment denials reported recently, and reviews average about 4.6 out of 5 stars from people online. Meridian Care at Grayson Square is managed by Morningside Ministries, and it handles care services from light daily help to skilled nursing for people needing intensive medical attention, though details about rental rules, parking, or exact floor layouts aren't available. The staff handles emergency alerts and move-in coordination, provides regular transportation and parking, and the place is open from 8:00 AM to 5:00 PM. Overall, Meridian Care at Grayson Square is set up for seniors who want some independence with access to care or need full nursing support, and it offers a mix of care, housing, and daily life help with a steady routine and clear focus on ongoing support.
