    Neglectful unsafe unsanitary nursing home

I would rate this place a 0 if I could - do NOT send your loved one here. In my experience my relative suffered neglect: missing sheets/pillows, unmade bed, strong urine odor, soiled clothing, missed meds, lost records, ignored/delayed call lights, and mishandled urinary care that led to severe complications. Staffing is chaotic - a few caring CNAs were overshadowed by rude, unprofessional administration, high turnover, and apparent indifference; there were falls, infections, dehydration, and even suspected theft. A nice patio and a couple helpful employees don't make up for unsafe, unsanitary, and negligent care - please take your loved ones elsewhere.

    Summary review

Overall sentiment across these reviews is predominantly negative, with numerous and repeated allegations of neglect, poor hygiene, safety failures, and mismanagement at Paramount Senior Care Centers at San Antonio. While a minority of comments describe positive elements (convenient location, some caring CNAs, 24-hour care, and occasional clean/appealing areas like a renovated patio), the dominant themes are systemic problems affecting residents’ daily care, safety, and dignity.

Care quality and clinical safety are major areas of concern. Multiple reviewers report missed or delayed medication administration, lost medical records, and mishandling of urinary catheters (Foley left in too long) that allegedly led to severe outcomes including sepsis and hospice placement. There are repeated allegations of dehydration, untreated urinary tract infections, infected nails, and soiled clothing or bedding. Several reports describe falls with injuries (cuts and a broken hip), and there are extremely serious and alarming allegations including sexual assault by staff and an overdose event; these are presented by reviewers as safety failures and point to potentially grave staffing and supervision shortcomings.

Staffing, behavior, and responsiveness are frequent complaints. Reviewers describe CNAs and nurses who are perceived as lazy, overwhelmed, or absent—call lights ignored or responded to after long delays, failure to provide baths unless specifically requested, and instances where family members felt compelled to clean or care for residents themselves. Some reviewers acknowledge individual CNAs who were good with residents, but they note that those CNAs were sometimes reduced or reassigned by management. Reports also repeatedly criticize front-line staff and administration as rude, unprofessional, defensive, or only working "for the paycheck." Visible signs of poor staff management include high turnover, multiple administrators over short periods, and allegations of management not holding staff (including DONs and nurses) accountable.

Facility conditions and daily living environment are additional problem areas. Numerous reviewers mention strong urine odor, stained bedding, missing pillows or sheets, unmade beds, and generally unclean shared rooms. Safety issues in the environment are highlighted by an incident where a water spill was left uncleaned for 45 minutes while staff conversed, and by rooms lacking working call buttons or phones. Reviewers also note the absence of visible staff identification (no badges), which contributes to perceptions of disorganization and lack of professionalism.

Operational and administrative concerns appear throughout the reviews. Families report lost medical records, slow or unreliable mail delivery, and even perceptions of theft. Several reviewers describe management as indifferent or dismissive when concerns are raised, with at least one reviewer explicitly stating administrators lied on reports. The facility is described by some as having financial or organizational instability, with mentions of multiple administrators and bankruptcy concerns. These management-level issues compound the care and safety problems described at the staff and facility level.

There are isolated positive notes: some reviewers found the location convenient and the facility initially a good fit; a number of commenters praise certain CNAs or say they received respectful, helpful care from some staff members; and a few reviewers mention a clean and safe environment or responsive management. However, these positive remarks are relatively sparse and often contrasted against more severe negative experiences from other reviewers.

In summary, the review set paints a picture of a facility with serious and recurring problems in resident care, staff responsiveness, hygiene and cleanliness, and management accountability. The most critical and frequent issues are neglect (including missed meds and poor hygiene), safety incidents (falls, infections, and very serious allegations of assault and overdose), and administrative failures (high turnover, poor communication, and alleged dishonesty). While some staff members and features receive praise, the volume and severity of negative reports suggest families should exercise caution, conduct thorough in-person visits, verify staffing and safety protocols, and closely monitor care if considering this facility. The reviews collectively recommend careful vetting and, in many cases, to seek alternative care options.

Medicaid, Medicare Parts A and B, private insurance, Hospice, Respite Care, HMO plans, and various medical services, appliances, and supplies are accepted, and the place takes pride in a warm, friendly environment where everyone's supposed to feel at home, even if things aren't perfect, and no one promises the world.
