    Compassionate staff but inconsistent care

I've seen both sides: many compassionate, attentive staff - nurses, therapists, the DON, social worker Lisa and activity director Loretta - who provide family-like, 24/7 care, good meals, activities, and a generally clean environment that lifted residents' spirits. However, chronic problems also stood out: poor communication and unanswered phones, staffing shortages and long delays, hygiene neglect and reports of abuse, billing/transfer issues, and a dated building in need of TLC. I'm grateful for the dedicated caregivers but concerned about inconsistent care - check staffing, oversight, and follow-up closely before deciding.

    Summary review

Overall sentiment: Reviews for San Antonio North Nursing and Rehab are highly polarized. A substantial number of reviewers praise individual caregivers, therapy teams, and the activities department, describing warm, family-like relationships, attentiveness, and good clinical therapy outcomes. However, an equally substantial portion of reviews raise serious concerns about systemic problems: poor communication from leadership, inconsistent staffing, facility condition, safety incidents, and alleged neglect or abuse. The net impression is one of a facility with strong frontline individuals but significant operational and managerial weaknesses that lead to widely varying resident experiences.

Care quality and staffing: Many reviewers singled out nurses, CNAs, therapists, and certain leaders (names mentioned repeatedly include Deanna, Lolita, Loretta/Laretta, Rosa, Brenda, Lisa, and the Director of Nurses) as compassionate, attentive, and highly effective. Several families reported 24/7 attention, timely medications, good therapy outcomes, and staff who know residents by name. Conversely, other reviews detail lapses in basic care: unbathed residents for days, soaked or soiled incontinence pads left unchanged leading to rashes, long waits for bathing or room cleaning, and allegations of staff neglect or inaction. Multiple accounts describe nights or weekends with questionable staffing levels, patients lying on the floor, or calls for help unanswered — serious safety red flags that contrast sharply with the positive reports.

Leadership, communication, and administration: A recurring theme is poor communication and unresponsiveness from administration. Numerous reviewers report unanswered phones, long email reply delays, an unreachable administrator, and a nonfunctional website contact form. There are also claims of transfers without adequate notification, inadequate follow-up from admissions or social work, and billing/refund disputes. Positive reviews mention a helpful social worker (Lisa) and involved management at times, but the dominant managerial narrative is one of inconsistency. Several reviewers expressed intent to escalate complaints to state regulators or ombudsmen due to perceived lack of accountability.

Facility condition and safety: Reviews frequently describe the building as underfunded, outdated, or in need of serious upgrades. While some reviewers note visible improvements and renovated areas, others call the facility dilapidated and unsanitary. Safety is a prominent concern: reports of patients in distress in hallways, lying on floors, or screaming for help are particularly alarming, as are allegations that a relative died after a short stay and that the facility did not adequately respond. These accounts, combined with reports of lost clothing and poor night/weekend staffing, suggest systemic risks that warrant investigation.

Activities, therapy, and meals: This is one of the facility's stronger areas according to many reviewers. The activities director (Loretta/Laretta) and her team receive strong praise for organized visits, including bringing activities to bedridden residents, Bible study, and a lively activities center. Therapy programs and staff are often described as excellent, contributing to positive outcomes for many residents. Dining feedback is mixed but leans positive overall — many reviewers compliment the meals and kitchen staff, although isolated complaints about poor meals or too many smoke breaks were also noted.

Variability and patterns: A clear pattern across reviews is extreme variability: some families describe an excellent, caring environment with great outcomes, while others recount neglect and unsafe conditions. Several factors appear to influence experience: time of day/shift (nights and weekends more problematic), specific staff on duty, and possibly changes in management or facility areas (some mention improvements, others continued deterioration). Multiple reviewers warned prospective families to verify current conditions themselves, check staffing levels and communication protocols, and speak directly with the named caregivers or department heads.

Recommendations and concerns: Based on the reviews, anyone considering San Antonio North should conduct careful, up-to-date due diligence. Key recommended checks include: recent state inspection reports and complaint history, current staffing ratios (especially nights/weekends), direct observation of care routines (bathing/incontinence care), verification of how administration handles communication and billing disputes, and inspection of patient rooms and common areas for cleanliness and repairs. Families should ask about dementia-specific care protocols if that is relevant, confirm how transfers and notifications are handled, and inquire about activity and therapy schedules. The mixed reports of strong individual staff and serious organizational problems suggest that outcomes may depend heavily on who is on duty and how management is addressing the documented issues.

Bottom line: San Antonio North appears to have many dedicated, compassionate staff and strong therapy and activities programs that produce positive experiences for some residents. However, persistent and serious complaints about communication breakdowns, inconsistent staffing (notably nights/weekends), hygiene and safety incidents, facility condition, and administrative responsiveness are common and concerning. The facility may be appropriate for some individuals if current management has effectively addressed these systemic issues, but the volume and severity of negative reports warrant caution, close monitoring, and verification before choosing this facility for a loved one.
