    Caring staff, strong rehab program

I placed my mom here after a fall and we've been very pleased - the building is beautiful and spotless, the rehab program is strong, and the staff are consistently caring, professional, and treated her like family. We felt welcomed and supported every visit; I'd recommend this facility while noting there's always room for small improvements.

    Summary of reviews

Advanced Rehabilitation and Healthcare of Vernon elicits predominantly positive feedback for its compassionate staff, rehabilitation focus, and modern facility, while also generating a set of operational concerns that prospective families should evaluate. Many accounts emphasize a welcoming, family-like atmosphere, visible teamwork, and clinically focused rehabilitation services that integrate with VA and local community resources. Multiple reviewers highlight strong leadership, attentive nursing and caregiving, and active social programming; the building and resident rooms are frequently described as clean and well maintained.

Care quality is characterized by two contrasting patterns. Numerous reviews describe attentive, dignified care from nurses and aides and successful short-term rehab stays; reviewers also credit staff with acting promptly during emergencies and providing reassuring family communication. However, a number of accounts indicate a decline in care quality over time, with delays in attending to resident needs and concerns that staffing levels contribute to slower response times. These comments point to variability in day-to-day clinical responsiveness rather than a uniform standard of care across all shifts.

Staffing and conduct are central themes. Praise centers on caring, professional teams who create a friendly, familial environment; leadership and teamwork are singled out as strengths. At the same time, reviewers describe inconsistent caregiver professionalism, occasional unprofessional interactions with visitors, and gaps in CNA instruction and competency. These patterns suggest that training consistency and staff supervision may vary across units or shifts.

Dining and activities receive mixed marks. Several reviewers report enjoyable meals and an active recreational calendar (bingo and other social opportunities were mentioned), while others describe inconsistent food quality. This variability suggests meal satisfaction can depend on timing, menu, or individual expectations.

Facility condition and sanitation present a similar split. Many families call the building attractive and well kept with easy access and pleasant common areas. Contrasting comments raise sanitation concerns specifically about interior cleanliness and housekeeping consistency. Prospective visitors may want to observe housekeeping routines and common-area conditions in person.

Management is generally viewed positively when it comes to coordination and emergency responsiveness; reviewers credit leadership with creating a coordinated team environment. Still, the pattern of concerns about staffing levels, CNA training, responsiveness, and occasional declines in care quality points to operational areas for improvement that could affect the resident experience.

Notable patterns for decision-making: strong rehabilitation services, compassionate staff, and a modern facility are recurring strengths. The primary operational risks identified are inconsistent interior cleanliness, variable dining, staffing-related delays, and uneven caregiver training/professionalism. Families touring the facility should observe staff–resident interactions across different shifts, ask about staffing ratios and CNA training programs, and sample the dining experience to assess consistency.

About Advanced Rehabilitation and Healthcare of Vernon

Advanced Rehabilitation and Healthcare of Vernon sits in Vernon, Texas, and serves seniors who need skilled nursing, rehabilitation, hospice, and memory care all under one roof, and although there's not a lot of special information floating around about this place in general, what is known adds up to a good-sized for-profit nursing home with the ability to serve up to 120 residents, with an average of around 111 there each day, so things stay lively but not overcrowded, and folks can get help for a broad range of needs, from inpatient and outpatient rehab therapy-including speech, wound, physical, occupational, and respiratory therapies-on up to long-term nursing care for those who truly need around-the-clock support because of frailty or lasting illness. The healthcare team includes all sorts of staff such as registered nurses, LPNs, CNAs, medical directors and doctors, nurse practitioners and physician assistants, dieticians, feeding assistants, pharmacists, social workers, respiratory therapists, speech/language pathologists, and therapy aides, so the clinical know-how is covered and folks who need specialized care get the right attention. The place accepts both Medicare and Medicaid, and while there's no resident council or family council here, the staff says they keep the focus on personalized care plans and offer respite care as well, so families have some options when they need support. Advanced Rehabilitation has amenities like transportation services, a mix of cozy rooms and common areas that've had recent fix-ups to make them feel comfortable and bright, and appealing outdoor spaces for fresh air or a chat. The facility runs person-centered programs and daily activities meant to support people's social, mental, and physical health, but there have been past inspection deficiencies-including those related to infection control and making sure everyone gets activities and care plans that fit their needs-which the facility has worked on addressing; still, their inspection score keeps at an A+ and their long-term care quality sticks at an A, both ratings higher than average even with the 18 deficiencies marked and a higher nurse turnover rate of 70% compared to the state norm. Advanced Healthcare Solutions oversees it, and since July 2021, Advanced Hcs Llc has handled management, with Teddy Lichtschein, Robert Meisner, and Eliezer Scheiner named on the records. The staffing levels put them a little above the state average for nurse hours, at about 3.47 hours per resident per day, which helps everyone get the help they need, but the high turnover does say something about staff changes over time. Overall, the place keeps its focus on supporting the independence, dignity, and comfort of those who need nursing care, short-term rehab, or memory support, and the attention to safety and medical needs stays current with an accredited program and regular inspections. You'll find up-to-date details on their own website, so it's easy to review recent inspection scores, see service options, and read about the care approach, and while the place may not have every bell and whistle or council group, it does aim to make long-term and rehab care steady, attentive, and as homelike as possible.
