    Caring staff, activities; inconsistent communication

I appreciated the friendly, professional and attentive staff, an engaged Activity Director, a clean, updated one-floor building with wide halls, garden and on-site PT - residents seemed social and well cared for. My concerns: inconsistent meals (sometimes light or carb-heavy), occasional missed room cleanings, staff turnover and poor responsiveness to complaints/calls. I'd recommend for the caring atmosphere and activities, but confirm dietary accommodations and communication expectations up front.

    Summary review

Overall sentiment across the reviews is mixed but leans positive: many reviewers praise the staff, activities, cleanliness, and the community atmosphere, while a smaller but important subset report inconsistent care, management responsiveness issues, and occasional cleanliness or dining concerns. The most frequently cited strengths are the staff’s friendliness and professionalism, the active programming led by an engaged Activity Director, and the facility’s clean, updated, one-floor layout with wide hallways. Several reviewers felt the environment was home-like with a small-town feel, appreciated the garden and fresh-produce touches, and noted on-site physical therapy and a pleasant dining room as positives.

Care quality and staff: Multiple reviewers emphasize attentive, tender, and professional caregiving. Staff are frequently described as friendly and able to answer care questions; several reviewers explicitly recommend the community on that basis. The Activity Director receives particular praise for engagement and programming. However, there is a noticeable pattern of inconsistency tied to staff turnover: some reviewers reported a decline in care and responsiveness after personnel changes. A minority of reviews describe poor responsiveness from staff and even lack of basic care, indicating variability in the resident experience depending on staff stability and specific shifts or teams.

Facilities and cleanliness: Most reviewers report a very clean, welcoming, and updated facility with new furniture, fresh paint, and nicely updated pictures. The single-floor layout and wide hallways are seen as practical assets. That said, there are isolated but serious negative remarks: a few reviewers said rooms were not cleaned, or the community smelled and was dirty. These outlier comments contrast with the majority but are meaningful because they point to lapses in housekeeping consistency.

Dining and dietary concerns: Dining receives mixed feedback. Several reviews praise the dietary team and housekeeping; others describe meals as "ok" or "average." Specific criticisms include meals being high in processed carbohydrates and an inability (or unwillingness) to honor dietary restrictions for some residents. A recurring, more minor theme is that weekend (Saturday) meals can be lighter or less impressive, with one review mentioning ice cream as a small treat on less-fabulous Saturdays. Overall, dining appears acceptable to many but inconsistent and potentially problematic for residents with special dietary needs.

Activities and social engagement: Activity offerings are a clear strength. Multiple reviewers note lots of programs, social interaction among residents, and staff efforts to engage residents. The presence of a dedicated Activity Director and visible attempts to keep residents involved are consistently mentioned as positive differentiators.

Management and communication: Reviews show mixed experiences with communication and management responsiveness. Several reviewers commend clear communication and responsiveness, while others report serious problems: complaints to corporate were reportedly not addressed, and the director did not return calls in at least one account. This split suggests variability in administrative responsiveness that can affect family confidence and the overall experience.

Notable patterns and recommendations for prospective families: The dominant pattern is that Sage Meadow De Pere offers a warm, activity-rich, and generally well-kept environment with caring staff and useful amenities (on-site PT, garden, updated common areas). However, there are recurring concerns about inconsistency—especially tied to staff turnover—and specific operational issues such as variable housekeeping and dietary accommodations. Because of this mix, prospective residents and families should prioritize clarifying certain operational details during a tour: ask about current staff turnover rates and staffing stability, request examples of how dietary restrictions are handled, inquire about housekeeping schedules and oversight, and meet the Activity Director and kitchen staff if possible. Also consider visiting on a weekend (e.g., a Saturday) to see how weekend meals and activities compare to weekday offerings.

In summary, many reviewers recommend Sage Meadow De Pere and highlight genuine strengths in staff, activities, and facility upkeep, but a meaningful minority describe experiences that raise concerns about consistency in care, management responsiveness, cleaning, and dietary accommodations. The community appears to deliver strong, compassionate service in many cases, but prospective families should verify current staffing, housekeeping practices, and dietary policies during their visit to ensure the level of consistency they expect.

    About Sage Meadow De Pere

Sage Meadow De Pere sits in De Pere, Wisconsin, and offers many different living and care options for seniors, including independent living, assisted living, memory care for those with Alzheimer's or dementia, and nursing home care, with services changing to fit what each person needs, and that's one thing people seem to like, along with respite care for people who need a short stay or a break for a caregiver, and they also help with hospice for end-of-life care. The staff at Sage Meadow De Pere stay awake all night for emergency help and know how to help people with bathing, dressing, taking medicine, and more serious things, like using mechanical lifts for folks who can't walk and help with diabetes care, including insulin injections or sliding scale therapy, and incontinence care, so help is always there, no matter the hour. The staff speak English and take time to find out what each resident prefers so care can be as simple or as involved as someone needs.

People who like to be social can find plenty to do, with onsite activities like games, movie nights, and day trips planned by an Activity Director, as well as devotional services available either in or outside the community, and regular meals in a shared dining room where the kitchen staff cook meals to help with special diets, keeping nutrition and taste in mind. Social spots in the community like a game room, lounge area, and garden give spots for people to meet and talk or join a card game, and there are walking paths outside for when the weather's good, plus a community house for gatherings and a calendar full of community-sponsored events. There's housekeeping and laundry, including dry cleaning, so chores don't get in the way, and for those who need it, move-in help is available to make settling in easier. Rooms come furnished, and the place is set up to be easy to get around for anyone using a wheelchair or needing a little extra help walking.

Sage Meadow De Pere has a rule against smoking inside, whether in shared spaces or in private rooms, but welcomes pets, and they'll consider bigger dogs if someone asks, which is something some people will find unusual, and there's complimentary transportation for going shopping or to appointments. Memory care spaces have security features and trained staff who understand the extra needs of people with dementia, like confusion or wandering, and keep an emergency alert system on hand for all residents. The staff and aides keep watch and guide people toward as much independence as possible, no matter which care level they're living under, using community programs called Community-Based Residential Facility (CBRF) and Residential Care Apartment Complex (RCAC) to match each person's changing needs. The community holds high health standards, even earning a Wisconsin Department of Health Services' deficiency-free survey, and staff, from housekeepers to caregivers to cooks, try to keep daily life warm, clean, and well cared for, so people feel at home and safe.
