    Excellent therapy; understaffed and unsafe

I had a mixed but ultimately disappointing experience. The short-term rehab and therapy are excellent - world-class therapists (Jeff) and several outstanding caregivers (CNAs Lucy and Betty, LPN Donna); rooms are spacious, clean, newly remodeled and wheelchair accessible. But the facility is severely understaffed and many aides/nurses were inattentive or uncaring: call buttons ignored, long waits, missed meds, and safety concerns. Communication and billing were poor - missing discharge papers/med lists, phone lines unanswered, and finance even threatened collections. Admin (Ray) and therapy staff are improving things, and I'd recommend it for short rehab stays only; I would not trust it for long-term care.

    Summary review

Overall sentiment across these reviews is mixed but leans toward polarized experiences: many reviewers praise the facility’s rehabilitation and therapy services, while others report serious and recurring operational and nursing-care problems. The most consistent strength cited is the quality of the therapy department and individual therapists, accompanied by multiple personal mentions of staff who were caring and effective. Conversely, frequent reports of understaffing, poor communication, medication mishandling, and billing/administrative failures generate significant concern and distrust among family members.

Therapy and rehabilitation emerge as Clearwater Care Center’s strongest, most consistently praised component. Multiple reviewers explicitly recommend the short-term rehab unit and call the therapy teams “world class,” noting efficient programming, effective clinicians, and positive therapy-related activities (one reviewer specifically mentioned therapist Jeff and a pizza party during therapy). Commenters felt therapy staff had great personalities and got measurable results; several reviewers said they would recommend the facility specifically for rehab stays.

Direct caregiving receives mixed marks. Several reviewers singled out individual CNAs and LPNs (Lucy, Betty, Donna) as compassionate and efficient, and some accounts describe attentive nursing, timely pain management, and good skin care. However, an opposing and substantial thread describes severe understaffing, staff “ignoring” call buttons, long waits for help, and caregivers who appear desensitized or uncaring. This dichotomy suggests uneven staffing levels or variable staff performance across shifts or units. The extremes range from “pleasant staff” and “kind and helpful” to descriptions such as “horrible,” “hell on earth,” and plans to move residents out.

Medication management and clinical safety are prominent concerns in negative reviews. Examples include a nurse forgetting a Parkinson’s medication, missing antidepressants, and finding medications that belonged to another resident. One reviewer reported being discharged with no paperwork or medication list and a lack of explanation from nursing, while another noted the director of nursing’s voicemail was not working. These incidents point to lapses in medication reconciliation, handoffs at discharge, and accountability/communication procedures—issues that pose direct risks to resident safety.

Facility condition and housekeeping are generally reported positively by several reviewers: newly remodeled, spacious, wheelchair-accessible rooms; clean building with no odor; and housekeeping that “keeps the room clean.” At the same time, a subset of reviewers reported unclean rooms and inattentive housekeeping, indicating inconsistency in environmental services. Dining and activities receive occasional positive notes — food quality is said to be improving and therapy-hosted events were enjoyed — but there are also examples of operational issues (e.g., breakfast tray not delivered until 10:30 a.m., failure to explain the menu) that reflect variable daily-service reliability.

Management and administrative communication present a mixed picture. One reviewer credits administrator Ray with improving the facility’s reputation, while others report poor communication from nursing and finance, billing disputes, and even threats of collection. Several reviewers noted a lack of basic administrative processes: no care conference held, staff not introducing themselves, inoperable phones or voicemail, and insufficient explanations at discharge. These administrative shortcomings exacerbate family members’ frustration when clinical or caregiving problems arise.

Patterns and takeaways: the facility appears to offer high-quality, effective therapy and has staff members who provide compassionate care, particularly in short-term rehab contexts. However, there are repeated and serious reports of understaffing, inconsistent nursing care, medication errors, communication failures, and billing/administrative problems. The result is a split narrative—excellent rehab outcomes for some residents alongside safety and satisfaction concerns for others. Families considering Clearwater Care Center should weigh the strong therapy program against documented operational risks, and should proactively verify medication management practices, staffing levels, discharge procedures (written medication lists and paperwork), and financial/billing processes prior to placement. Clear documentation of expectations and regular care conferences may help mitigate some of the variability described in these reviews.

    About Clearwater Care Center

Clearwater Care Center sits on Heights Drive in Eau Claire and holds space for up to 161 residents, and folks looking for care have access to both private suites and shared rooms, all fully furnished and with private bathrooms, telephones, cable TV, air-conditioning, and Wi-Fi, and you'll also see kitchenettes in some units. There's a 24-hour call system, so help's always around if something's needed, with caregivers and nurses available day and night for things like medication, help with bathing, dressing, moving between spaces, and daily activities, and there's a focused effort on keeping residents safe with a highly protected and closely monitored environment to prevent wandering. English-speaking staff, including certified nurses, licensed medical practitioners, certified nursing assistants, and physical therapists, work together to provide skilled nursing care and help with mobility, medication, and medical needs, and the team coordinates with outside healthcare providers too.

Clearwater Care Center does aid with transportation for appointments and makes sure meals are both prepared and served, plus there's regular housekeeping, laundry, and linen service. Seniors will find lots of activities including movie nights, music and arts programs, outdoor groups, a fitness room, a sauna or wellness space, walking paths, a community garden, and a library, plus resident-run events and social programs. There's also a game room, activity spaces, common areas outside, and plenty of options for those who need transportation or parking. Medicaid, Medi-Cal, and Medicare are accepted forms of financial support here, and the facility holds various state licenses and skilled nursing certifications under number #3224.

Lately, Clearwater Care Center has made a decision to close after years of financial struggles, and they're working closely with state authorities and patient advocates to move residents to other homes. The facility's main focus now is helping residents relocate safely and making sure everyone's supported through the process, and the center has stayed open until all residents have moved. You'll find their updates shared on Facebook, but there's not much more current detail about what's available while the center winds down its care.
