    Caring staff but dirty, unresponsive

I had a mixed stay: the nurses, therapists and support staff were caring, helpful and often went above and beyond, making me feel safe and at home, but the facility was frequently dirty and smelly, call lights were slow, management often unresponsive, and rehab progress was poor (I even found feces on a toilet seat). I'm grateful for the hardworking crew, but I'd caution others before choosing this place.

    Summary review

Overall sentiment about Crossroads Care Center of Fond du Lac is mixed, with a clear split between many strongly positive experiences and several serious negative reports. A sizable portion of reviewers praise the facility for its caring, compassionate, and dedicated staff, noting specific teams — nurses, housekeeping, maintenance, kitchen, and therapy — as providing good service. Multiple reviewers describe the environment as safe and home-like, report a smooth and timely admission process, and say staff went above and beyond to support residents and families. Rehabilitation after hospitalization is cited as a clear positive by several reviewers who say therapy and nursing helped with recovery and that they would recommend the facility.

At the same time, a number of reviews raise significant concerns about cleanliness, staff responsiveness, and respect for residents' dignity. Several reports describe the facility as dirty and smelly, with explicit reports of unsanitary bathroom conditions (including feces on a toilet seat), which are serious and recurring complaints. There are also strong criticisms about staff unresponsiveness and long call-light response times from some families, even though other reviewers reported fast responses — indicating inconsistency in day-to-day operations or staffing. Communication problems and miscommunication between staff and families are noted, and some reviewers expressed a lack of faith in therapy progress for certain residents.

The staff themselves are a central, polarizing theme: many reviewers single out individual caregivers and teams as compassionate, helpful, and hardworking, often expressing gratitude and recommending the facility. Conversely, other reviewers describe unfriendly or unresponsive staff and even recommend avoiding the center entirely. There is at least one explicit mention of state involvement or official complaints, which elevates the concern level for prospective families and suggests that some issues may have been severe enough to trigger external oversight.

Dining and housekeeping receive mixed mentions — some residents and families find the food palatable and note that daily housekeeping occurs, while other reviewers point to overall uncleanliness. Rehabilitation and therapy are similarly mixed: several reviewers praise rehab services and successful recovery after hospitalization, while others report stalled progress and dissatisfaction with therapy outcomes.

In summary, Crossroads Care Center of Fond du Lac appears to deliver very positive experiences for many residents and families, driven largely by caring, committed staff and functional therapy and support services. However, there are recurrent, serious concerns around cleanliness, staff responsiveness, dignity/respect issues, communication, and inconsistent therapy outcomes. These divergent reports suggest variability in care quality — possibly tied to staffing, shifts, or specific units — and indicate that prospective residents and families should tour the facility, ask about recent inspections or state citations, inquire specifically about cleaning protocols, call-light response metrics, staff turnover, and therapy plans, and seek recent references from current residents or families to get a clearer, up-to-date picture before deciding.

About Crossroads Care Center of Fond du Lac

Crossroads Care Center of Fond du Lac sits at 115 E Arndt Street in Fond du Lac, Wisconsin, and it has 50 certified beds for residents needing skilled nursing care, and since it opened in 2007, it's run by a private healthcare company that operates in Wisconsin, Illinois, and Ohio, with Joseph Brandman, Aaron Topper, and Igor Rebel as its direct owners and Joseph Brandman and Aaron Topper managing since December 2017. The staff numbers between 1,001 and 5,000 across its locations, and even though the nurse turnover rate at this site is 40%, which is under the state average, the daily staffing works out to 3.80 nurse hours per resident, a bit below the state's typical 4.1 hours. The facility was remodeled and keeps things clean, with a high staff-to-resident ratio, but inspection records show 24 total deficiencies in recent reports, including issues with resident rights, food storage and serving, and two infection-related matters, though none caused actual harm, with the latest findings coming from June 2024 and March 2025 complaint visits.

Crossroads mainly focuses on skilled nursing, short-term rehab, long-term care, respite care, and several therapies including physical, occupational, speech, infusion, wound therapy, and specialty Alzheimer's and dementia care, and it even includes subacute nursing services if someone's just leaving a hospital or surgery. The therapy team works six days a week in a large gym, and residents get access to dialysis on site as well if needed. Folks here find staff who prioritize personal care plans and talk openly with families and care providers, and there's always a nurse or therapist around the clock to respond to needs. Accommodations include single and shared rooms, and there are places like activity rooms, a beauty salon, lounge areas, outdoor patios, and well-appointed dining spaces.

The kitchen staff teams up with a dietitian to plan meals that suit many dietary needs, so residents can enjoy nutritious, tasty food, and there's fine dining with options for birthday or holiday celebrations or just regular meals. The place also has three facility dogs that visit and spend time with residents, live entertainment, regular outings, cable TV, Wi-Fi, in-house laundry, and housekeeping. For people who want spiritual or religious activities, those are available too. Daily activities are planned to keep people engaged, and residents can use phone lines and join in on both group and individual programs. Crossroads Care Center of Fond du Lac keeps a focus on safety and clean spaces, but records show some areas where improvement is needed, especially with residents' rights and nutrition management. Overall, this place delivers a range of care and therapy services, a strong mix of amenities, and stays committed to keeping the approach centered on each resident's needs and preferences.
