    Caring staff; management, safety concerns

I found the frontline staff kind, caring, and professional - attentive nursing, engaging activities, good food options, and residents seemed happy and well cared for. That said, management felt untrustworthy: unclear ownership/temporary director, calls not returned, paycheck/financial concerns and what felt like financial exploitation, plus high turnover and short staffing. I also saw safety and cleanliness issues (broken door, nonfunctional alarms, dusty rooms, food strewn) and noted a significant regulatory fine for a deficiency. In short: excellent caregivers, but serious management, safety, and cleanliness problems that need urgent fixes.

    About Dove Healthcare - Lodi

Dove Healthcare - Lodi, located at 700 Clark St. in Lodi, Wisconsin, is a 50-bed skilled nursing and rehabilitation center that provides 24-hour care for people needing long-term, post-acute, or end-of-life support, and while the building is modern and equipped for comfort with things like an outdoor patio, one- or two-bedroom apartments, tenant-controlled heating and cooling, washer and dryer in every apartment, basic cable, Wi-Fi, heated underground parking for a fee, accessible ramp entry, and easy access to amenities such as a fitness center and storage units, the focus really remains on healthcare needs with trained staff like skilled nurses and certified nursing assistants always available-even offering special care for wounds, memory care needs at the Rutledge Home, ventilator care, and specialized therapy programs covering physical, occupational, and speech needs. Now, the dining setup includes a daily continental breakfast and lunch three times each week, bi-weekly light housekeeping helps keep things tidy, and everyone can enjoy utilities included in the monthly rate. People recovering from a hospital stay, needing rehab or ongoing skilled nursing, can find programs here that support mobility, recovery, and planning for discharge to other settings, and this often includes help from a whole team of professionals, like a dietitian, therapists, nursing directors, and social services staff-including Tom Graves as Administrator and Katie Prellwitz as Director of Nursing. There's a strong emphasis on providing a positive environment, with managers aiming for compassion, dignity, respect, and constant improvement, though the facility has seen several deficiencies over the years, totaling 55 documented inspection issues, with specific problems rated as severity "J" for quality of life and care, especially related to pressure ulcer prevention and treatment, and severity "F" for issues with nutrition, such as not always getting food from approved sources and not always meeting standards for storage, preparation, or serving, along with 7 infection-related deficiencies, which is important for anyone wanting to understand the full picture. The nurse staffing hours are a bit above the state average, coming in at 4.33 hours per resident per day compared to 4.1, but there's a high nurse turnover rate at 98.1%, which is much higher than the state average. The center also offers educational classes for the public, like CNA, CBRF, and medication aide training, and operates as part of the wider Dove Healthcare system, sharing staff and values with their sister locations, including independent living apartments at Settlers' Trail and added amenities like fitness classes led by therapists. On-site management and maintenance help keep things running day-to-day, with directors like Melissa Guethlein in nutritional services, Josie Wakefield in recreation, Adam Kirchoffner in environmental services, Jamie Rasmussen as the registered dietitian, Carley Priske as the rehabilitation manager, Melissa Albrecht as the physical therapist, and Kelsey Transue as the physical therapist assistant, and since February 2019, Ilan Richland has held managerial control, while a merger with Dove Healthcare was completed in March 2024, bringing some ongoing changes, all while services remain focused on comprehensive health management for seniors in the Lodi area.
