    Great rehab stay, caring staff

I stayed here for rehab and was very pleased. The building is beautiful and well-maintained, rooms and common areas are clean and quiet, and the staff are wonderful-caring, helpful, and punctual with therapy, which really improved my recovery. Group dining fostered camaraderie and everyone was welcoming despite religious affiliation; weekends were a bit slower but not a problem, and the food was decent.

    About Ovation Sarah Chudnow

Ovation Sarah Chudnow sits in Mequon, Wisconsin, and follows Wisconsin's laws for senior communities, like licensing, criminal background checks, fire safety checks, and regular state reports, so folks can feel safer about the care they get. The building is close to grocery stores, coffee shops, and pharmacies, which makes it easy for residents to get out if they want, plus there's on-site parking and transportation for trips to medical appointments. Residents can choose from independent living, assisted living, skilled nursing, short-term rehab, hospice care, memory care, and adult day services, and people can move between these if needs change, because the community allows aging in place and flexible care. There are 41 apartments for independent and assisted living, coming in one-, two-, and three-bedroom sizes, and 25 studio- and one-bedroom assisted living apartments. The memory care area has 9 studio apartments made for folks with mild to moderate memory loss, and the 25-bed skilled nursing center offers rehab and end-of-life care. Certified RCAC apartments, which are state-inspected and accept Medicaid, are available, while Registered RCAC apartments don't take Medicaid and only get checked if someone makes a complaint.

Nurses are on staff around the clock to help with everything from daily living like bathing and dressing, to more involved medical care, and residents can buy extra care services if they need them beyond the included hours. Community Based Residential Facilities (CBRFs) provide up to three hours of nursing a week, while Residential Care Apartment Complexes (RCACs) can give up to 28 hours, depending on what a resident needs. Staff also help with things like medication, wound care, pain management, and meals-including diabetic and kosher meal plans. Folks with memory problems get attention from staff trained for memory care and dementia support, and there's a dementia waiver and special programming to keep minds active and safe. Rehabilitation runs through their Rehabilitation Center for things like stroke, joint replacement, or recovery from a fall. The facility is licensed under numbers ALF# 0018248 and 0018247 and carries a Medicare rating of 4.8 stars out of 5.

Residents enjoy plenty of social and recreational programs-like game nights, live music, lectures, Tai Chi, chair yoga, exercise classes, arts and crafts, story time, book clubs, tabletop games, and outings. There's a movie room, library, fitness center, art studio, salon, outdoor patios and gardens, walking trails, and a general store, so folks stay busy or find some quiet when needed. Pets are welcome, with some rules, and there's even pet visitation for comfort. Residents get free WiFi, cable TV, in-room kitchen appliances, and central air. Meals are prepared on-site, served in a community dining room, and include kosher options along with special diets, and families are welcome to join for meals. Housekeeping, laundry, and linen services come with each apartment, and folks can ask for private cleaning help, too. Amenities cover a wide range, such as in-house dry cleaning, spiritual services, scheduled outings, fitness and wellness programs, and help with paperwork for things like VA benefits and insurance when paying bills.

The staff takes pride in giving person-centered care, focusing on each person's needs, independence, and happiness. The community also does regular safety checks, keeps up-to-date reports, and works with the state's Bureau of Assisted Living. Rooms have measures like emergency call systems, housekeeping, and maintenance on hand. Family outreach and educational resources are available to help loved ones keep up with care plans and changes. Entry fees are about $3,000, and payment options include checks, credit cards, and insurance plans. People can come for short-term respite stays or settle in for long-term care. The community aims to create a steady, connected space where folks can age comfortably, with support when it's needed and activities to enjoy, without overpromising what life is really like in a senior community.
