    Run-down unsafe neglectful understaffed facility

I would not recommend this place. The building felt run-down and dirty (peeling paint, moldy curtains, filthy bathrooms), safety was poor (unsafe elevator, unsecured doors, understaffed nights, slow call-light response), and basic care was neglected-missed showers/toileting, delayed fall notifications, infection/medication mishandling. Food was bland and inadequate, and many staff were rude or uncaring (though a few therapists and nurses were compassionate and helped with rehab). Management was largely unresponsive; only recent leadership changes seem to be trying to improve.

    Summary review

    Overall sentiment and themes:

The reviews for Crossroads Care Center of Milwaukee are highly polarized but lean strongly toward negative overall experiences. Multiple reviewers describe serious, systemic problems: a run-down, dirty facility with peeling paint, mold, unpleasant odors (notably urine), and unsanitary bathrooms and shower areas. Reviewers repeatedly raise concerns about safety and supervision — residents left unattended, mattresses or residents on the floor, bruises and injuries allegedly inflicted by staff, and multiple falls with delayed notification. There are also reports of severely inadequate infection control and wound care (open wounds exposed, improper cleaning), which compound the safety risks. Several families reported filing complaints with the state and believing the facility should be shut down based on observed conditions.

    Care quality and staffing:

Care quality reports are inconsistent and range from exemplary to dangerously negligent. On the positive side, many reviews praise a strong therapy team and individual caregivers who are dedicated, patient, and effective — examples include successful hip-replacement rehab, timely medication administration for some residents, and named staff who stood out (e.g., Natasha, David). Activities staff and the activity program itself are repeatedly cited as one of the brighter spots.

Conversely, a large number of reviewers describe severe neglect and poor direct care: missed basic hygiene tasks (teeth brushing, hair brushing, showers), inappropriate diapering practices (diapering when residents can use a toilet, delayed changes causing rashes), and lost personal items due to laundry errors (missing dentures/clothing). Chronic understaffing is reported, particularly overnight, which reviewers connect to slow call light responses (reports up to 45 minutes), one-nurse third shifts, and insufficient supervision that leads to safety incidents. Alarmingly, at least one reviewer alleged staff impairment (attempting to access medication carts without keys, crawling on floors, falling asleep, cigarette accidents) and said management did not adequately address these incidents.

    Facilities, cleanliness, and environment:

Many reviewers describe the physical environment as dilapidated and depressing: rusty fixtures, moldy privacy curtains, peeling paint, crowded hallways filled with wheelchairs, long elevator waits, and an overall depressing atmosphere. Bedding and rooms were cited as filthy in some accounts (dirty mattress, no pillows or bedding). Shared rooms create privacy concerns for families; some reviewers recommend private rooms. Basic room amenities were noted as lacking in some cases (no in-room phone, no TV). These conditions are often cited alongside reports of theft risk and a general lack of security (doors unlocked at night, insufficient entry security), heightening family concerns about resident safety.

    Dining and daily living services:

Food quality is a frequent complaint: bland meals, cold eggs, missing condiments, and a general sense that dining is inadequate. However, a minority of reviewers found meals acceptable or noted that their dietary needs were accommodated. Laundry issues are a persistent negative theme — lost or mixed-up clothing and dentures undermine dignity and create ongoing family frustration.

    Management, communication, and responsiveness:

A major recurring theme is ineffective or unresponsive management. Numerous reviews state that complaints were ignored, that management failed to discipline or address staff misconduct, and that communication with families and between shifts was poor. Some reviewers described rude, dismissive, or unprofessional behavior from staff and managers (including a director of nursing who talked down to residents). Several reviewers reported being threatened or treated disrespectfully when raising concerns. However, there are also several recent comments indicating a change in leadership: multiple reviewers mention new administration, improving star ratings, responsive administration, and being impressed by recent management changes. These positive notes suggest some efforts at quality improvement, though reviewers differ on whether changes are meaningful or widespread.

    Notable patterns and actionable concerns:

The most concerning and recurring patterns are: (1) safety incidents (falls, bruises, unattended residents), (2) severe cleanliness and infection-control problems (mold, filthy bathrooms, exposed wounds), (3) inconsistent staff quality and possible impairment claims, and (4) management unresponsiveness to complaints. At the same time, therapy/rehab services and certain individual staff members receive consistent praise, indicating the facility is capable of providing high-quality care in some areas. This creates a pattern of inconsistent care — high variability by unit, shift, or specific staff members.

Bottom line recommendation based on reviews:

Families should approach Crossroads Care Center with caution. Reviews indicate urgent, systemic issues that have led some family members to file state complaints. If considering this facility, verify the most recent state inspection reports and enforcement actions, ask for documentation of staffing levels and incident resolution, and tour the specific unit(s) where a prospective resident would live. Also speak directly with therapy staff and the current administration about improvements under new management. For short-term rehab, some reviewers had excellent outcomes and praised the therapy team; for long-term placement, the repeated reports of neglect, poor hygiene, safety incidents, and management failures are significant red flags unless you can confirm consistent, documented improvements since the cited incidents.

About Crossroads Care Center of Milwaukee

Crossroads Care Center of Milwaukee sits at 3216 W. Highland Blvd. in a quiet residential area, right along a city bus route, and offers both short-term rehab and long-term skilled nursing care in a building with 95 certified beds, some private and some semi-private, and residents can use three dayrooms or lounges, a beauty salon for grooming, and a nice landscaped backyard and patio for relaxing outdoors, all while staff try to meet both physical and emotional needs. It's run by Joseph Brandman and Aaron Topper since September 2014, with Gittel Brandman and Aaron Topper sharing ownership, and it's part of a group with other facilities across Wisconsin and Illinois. It provides care for the very frail and those needing intermediate care, giving services like occupational, physical, and speech therapy, as well as wound care, IV therapy, and even HemoDialysis, and if someone requires tube feedings or hospice care, those services are available too. Registered nurses and therapists follow personalized care plans based on each resident's needs, orders, and goals, and residents get their own cable TV, wireless internet, and dedicated phone lines. The dining offers what many call fine meals, and activities range from music concerts and river cruises to festivals, zoo trips, casino visits, arts and crafts, and even ice cream socials or bowling, plus regular exercise programs and social events like corn roasts. Now, the facility has reported 37 deficiencies in recent inspection reports, showing issues like not always giving enough food or fluids, having infection-related problems, and not always providing proper urinary or bowel care for those who need it, with several complaints connected to quality of life and care. It carries a nurse staff turnover rate of 53.7%-a bit higher than the average in the state-and nurses at Crossroads Care Center give about 2.88 hours of care per resident per day, which is lower than Wisconsin's average of 4.1 hours. The staff focus on following CDC and government safety guidelines and will set up personalized tours for families thinking about moving in. Payment options cover Medicare, managed care, private pay, various Medicaid and Family Care plans, hospice, and some insurance plans like Anthem and Molina, making it possible for a range of clients to get service if beds are open.
