Plymouth Health Services sits quietly at 916 E Clifford St. in Plymouth, Wisconsin, and people can see the lawns are kept up and it's a peaceful spot, with lots of trees and clean, simple landscaping, and it's one of the six higher rated communities in town with an average score of 6.7 out of 10 from those who use it, though the nursing home there has a 1-star rating from the Centers for Medicare & Medicaid Services, which means families should look closely at those reports. The community is part of Paracor and has links with North Shore Healthcare and Mercy Health Services, and also connects with Atrium Post Acute Care of Plymouth, and carries a focus on health services for different needs, with skilled nursing, assisted living, memory care, home health, hospice, palliative, and rehabilitation-even brain injury rehab, which isn't too common everywhere. People who move in have different choices for studio room layouts, and many rooms are private, set up with telephones and televisions so residents can stay connected, and there's even a Reiki Room for those who want that kind of treatment. The staff try to give residents a comfortable and supportive place to live, with lots of help for daily activities, and the campus always feels quiet and welcoming. There's telehealth as an option when doctors say it's needed, along with an automatic defibrillator on site, and a full pharmacy, which makes medicines and treatments easier to get. The place runs its own lab tests, X-rays, EKG, bladder ultrasound, and also offers venous doppler. Meals and activities get organized for everyone, and when people are getting ready to leave, there's discharge planning too. Short-term rehab is available for folks who need a little help between the hospital and home, while those with higher needs can access skilled nursing or intermediate care, mostly for people who depend on nursing most of the day. They don't leave out palliative, respite, and hospice for those who need comfort and end-of-life care. Plymouth Health Services also gets involved in the Sheboygan County Chamber, giving back to the community, supporting government advocacy, professional development, and workforce programs, plus it runs different member groups and has a business directory if anyone's looking for resources, job listings, or events in the area. Staff are trained to focus on each resident, helping them feel heard and looked after. There's activity and dining services and some ways to keep everyone engaged, so while the facility isn't perfect, the range of services means people can find something that fits their current stage of life.

Unsafe, dirty facility; negligent staff
I had a mixed but ultimately negative experience. A few staff and therapists were genuinely caring and went the extra mile, but nurses were often disengaged, judgmental, and ignored doctors' orders - there were medication mix-ups and hospice moved my loved one after one day because it felt unsafe. The building is old, dirty and prison-like, the food was awful, communication and management were indifferent (they refused a refund when I moved out mid-month), and personal belongings went unaccounted for. I left early for safety reasons and would avoid this facility despite some kind employees.
