    Pleasant hallways tiny rooms neglect

I lived here. I liked the older building's wide, windowed hallways and the smaller, home-like feel with more personal space, but rooms are tiny and there's no private bathroom. I experienced persistent roach problems, late dinners, and staff neglect - including withholding meds, verbal and other abuse by caregivers, and once being locked out. I would not trust this place with a loved one.

    Summary review

Overall sentiment in the reviews is mixed but leans strongly negative because of several serious safety and care concerns. On the positive side, reviewers note that Autumn Oaks feels smaller and more home-like than some larger assisted living communities. Multiple comments praise the facility’s more open hallways and windows and say residents can have more personal space and closer attention from staff and family. A few reviewers explicitly state that residents appeared "well cared for" and that the setting could be good for some loved ones seeking a smaller, more intimate environment.

However, these positives are overshadowed by repeated reports of severe problems with care quality and resident safety. Multiple review items allege caregiver abuse, including verbal abuse and broader accusations of abuse of vulnerable adults. There are also explicit claims that medications were withheld and that residents experienced neglect. Those issues represent high-severity concerns and suggest potential lapses in staff training, supervision, or management oversight. Several reviewers say they would not trust the community with their loved ones because of these incidents.

Facility condition and cleanliness are a mixed picture. While the building’s older character is noted as contributing to larger hallways and more windows, reviewers also point out drawbacks of an older facility: smaller private rooms, the absence of private bathrooms, a strong unpleasant smell in parts of the building, and reports of roaches. These maintenance and sanitation complaints add to the negative impressions and can compound worries about overall care standards.

Operational issues and day-to-day living complaints also appear in the reviews. One specific recurring complaint is late dinner timing, which may reflect scheduling or staffing problems. Another concerning report describes being locked out of the building—this raises red flags about access control, staff responsiveness, and emergency procedures. Taken together with the reports of neglect and medication withholding, these operational failures suggest inconsistent implementation of basic assisted-living responsibilities.

Patterns across the reviews point to inconsistency: some families and reviewers feel the environment is caring and adequate, while others report serious abuse, neglect, and hygiene issues. That contrast may indicate variability in staff performance, shift-to-shift differences, or isolated incidents that have not been adequately addressed. Regardless, the presence of both positive caretaking impressions and very serious safety allegations means prospective families should treat the facility cautiously and seek more information.

In summary, Autumn Oaks appears to offer a smaller, more personal, home-like setting with physical features many reviewers like (larger hallways and windows, more personal space). At the same time, multiple reviewers report very serious problems — caregiver abuse, withholding of medications, neglect, pest and odor issues, late meals, and security/access problems — which collectively create significant concern about resident safety and consistent quality of care. These recurring, high-severity issues are the dominant theme and should be investigated and verified before placing a loved one in this community.

    About Autumn Oaks Assisted Living

Autumn Oaks Assisted Living sits in Slinger, Wisconsin, with the address listed as 227 and 229 E Washington St, and offers state-licensed supportive living with care going beyond meals and a room, so folks who need help with bathing, dressing, walking, nutrition, and medication get steady support, and staff watch for safety and health around the clock, which some families find brings peace of mind. The community has private apartments, some in renovated homes and some in newer buildings, and residents can furnish and decorate their own places, or keep it simple if that's what they like, with options for studios, one-bedrooms, or two-bedrooms, and pets are allowed. The facility has a two-story building with good natural light, activity rooms, and things like laundry and housekeeping included, and all meals served restaurant-style, and the dining is adjusted for diabetic needs.

There's staff on-site twenty-four hours, and the trained care team helps with grooming, medications, and mobility-and, folks with memory loss, including Alzheimer's and early to moderate dementia, can move into the dedicated Memory Care or Alzheimer's/Dementia Care Units, where the team runs six daily activities from brain exercises, games, flower arranging, and baking, to group walks and Scrabble matches, hoping to slow down memory decline and keep bodies and minds active. Each resident has a care assessment when they move in, so the services match their situation, whether it's stand-by help, daily tasks, safety checks, or monitoring medical conditions like diabetes. Autumn Oaks also supports people needing short stays, so caregivers at home can take a break, or residents can recover after a hospital visit, and respite care is available for both short and longer periods.

Folks who don't need full-time nursing home care but do need more than simple help can live at Autumn Oaks, and the facility classifies who can live there by mobility and mental ability, with Class AA for those who can leave the building by themselves, and more supportive classes like AS and CNA for people who need some help moving or getting out in emergencies. Nurses can give up to three hours of skilled care weekly for those who need it, but families with residents needing more can ask for exemptions, if needed.

There are also specialized memory care programs and Alzheimer's specialty care offered across more than one location, using different care units, and there's support for ambulation, stand-by help, podiatry, diabetic care, and on-site laboratory work, plus a beauty salon for folks wanting to keep up appearances. Amenities make life more comfortable, such as scheduled outings, activity options, laundry, and housekeeping. Seniors get three meals each day, and in between meals, there are programs for exercise, socialization, and rest. For paying, Autumn Oaks Assisted Living has several prices, sometimes all-inclusive or paid for as needed, and they accept Community Options Program (COP) or Family Care funds, so some financial help comes through local agencies if a resident qualifies.

Autumn Oaks supports seniors seeking a middle ground-folks who can't safely live alone, but who aren't quite ready for a skilled nursing facility, and the facility maintains a warm and safe environment that feels like home to many. The staff strives to offer companionship and a sense of community through group activities and family advisory services, and the location near Slinger lets families visit or stay involved. Overall, with choices for living arrangements, help with daily life, and specialized memory care, Autumn Oaks gives residents and families options and support that cover both the basics and some unique memory and dementia needs.
