    Warm staff, concerns about management

I toured this facility for my mom and overall I'm pleased: staff are warm, caring and helpful, the office and management have been responsive, the building is clean and bright, apartments are roomy with in-building laundry and lots of activities (church, crafts, exercise), and nurses/caregivers communicate well. Maintenance and move-in help were good and hospice support was excellent. My biggest concerns are inconsistent food quality, staffing reliability/high turnover and some disorganized upper management (rate hikes and occasional security/odor issues). I would recommend it with reservations.

    Summary review

Overall sentiment across the reviews for Liberty Village of Tomah is mixed but leans positive with notable caveats. Many reviewers consistently praise the staff on the floor level — caregivers and nurses are repeatedly described as caring, attentive, helpful, and willing to go the extra mile. Office staff and maintenance receive multiple mentions for being responsive and supportive, including move-in assistance and timely answers to questions. Several families report that nurses manage medications well and keep them informed about behavioral or eating changes; hospice support was also cited as handled well. These positive staff interactions contribute to residents and families feeling safe, welcomed, and satisfied in many cases.

Facility and apartment features are frequently commended. Multiple reviewers describe the building as clean, well-kept, and tidy, with in-building laundry on every floor, sitting rooms on each level, ample storage, and spacious apartments suitable for single occupants. Floorplans are described as bright, cheery, and comfortable; residents can bring their own furniture which adds to the home-like environment. Common spaces — including a large, tidy dining area, library, and gathering rooms — and a pleasant wooded location support a welcoming atmosphere. Some amenities such as salon visits and religious services are available and appreciated. However, a few specific maintenance issues were noted (for example, a soaking tub out of commission and a removed workout room), indicating that while the property is generally well-maintained, not every amenity is always available.

Activities and social programming are a strong highlight. A robust weekly activities schedule with a wide variety of options (Bingo, crafts, puzzles, music, exercise classes, entertainment, and church services from several community churches) is repeatedly praised; many residents reportedly enjoy and participate in these offerings, and family members note that their loved ones "love" the activities. This active programming appears to be a consistent strength of the community and a primary reason many reviewers recommend the facility.

Dining and food quality emerge as a recurring area of inconsistency. Several reviewers report that meals are nutritious, have good portion sizes, and are "really good" or "consistently hot." Yet an almost equal number of comments describe poor or unhealthy food, with specifics such as dry hamburgers and dry chicken, limited meal items, and bread that dries out. These conflicting accounts suggest variability in meal preparation, menu offerings, or possibly changes over time or by meal staff. Prospective residents and families may want to taste the food and ask about current menus and meal-service processes before making decisions.

Management and staffing issues present the clearest source of divided opinion. Some reviewers praise proactive, phenomenal, or new management that has cleaned up the environment and improved the atmosphere; others report disorganized upper management, staff turnover, and allegations that management disregards good employees. Short staffing and high turnover are mentioned multiple times and are associated with concerns about service consistency and care quality. Financial issues are also noted: several reviewers mention rent/rate increases (including reports of rates raised twice in a year) and some link these increases to empty rooms or to management explanations blaming staff pay. Security worries were raised in at least one review (front doors unlocked at night), which is a concrete and serious concern that should be clarified with the community.

A notable pattern is variability in experience — many reviewers provide glowing recommendations, while a smaller but significant group advises against the community or reports poor care. This split often correlates with perceptions of management stability and staff consistency: when management and staffing are described positively, so too are food, care, and overall satisfaction; when management is perceived as disorganized or turnover is high, negative experiences appear more frequently. In short, Liberty Village of Tomah offers many strengths — especially in resident engagement, cleanliness, and compassionate direct-care staff — but there are important and recurring concerns around food consistency, management practices, staffing stability, and a few safety/amenity issues. Prospective residents and families should weigh the strong positive reports about daily caregiving and activities against the reported variability in management and dining, ask specific questions about current staffing levels and security measures, and, if possible, visit for a meal and an extended tour to confirm the current environment.
