    Comfortable care, friendly staff, concerns

I moved into a clean, updated private two-bedroom and immediately felt calm - nice kitchen, outdoor access, no offensive odors and quick, personal help (they even mounted my TV and brought ice water when I arrived). The food is good with daily specials, the dining room, beauty shop and chapel are pleasant, and activities are engaging (bingo, entertainers, bus outings, school visits), plus transportation, rehab, gym, podiatrist and a Thursday hair salon. Staff and caregivers are caring, professional and responsive - maintenance helped hang pictures and doctors pay attention - but since the ownership change I've noticed fewer CNAs, longer wait times and some amenities removed. Overall I feel safe and cared for and have peace of mind, though I am watching safety issues (an unlocked area and slippery floors from humidity; safety strips are planned).

    Summary review

Overall sentiment in the reviews is mixed but leans positive on day-to-day resident experience while showing clear concern about staffing and management changes. Numerous reviewers emphasize highly personal, caring interactions: caregivers and staff are repeatedly described as attentive, compassionate, and willing to go above and beyond (examples include bringing ice water, helping with move-in, and quickly mounting a television). Many residents and families praised the calm, welcoming arrival experience and said the facility provides peace of mind. Cleanliness is a strong positive theme: reviewers note updated rooms, renovated apartments, private two-bedroom options, pleasant kitchens, and no offensive odors.

The facility’s amenities and activities are frequently highlighted as strengths. Reviewers list a robust calendar that includes bingo, bus outings, musicians and entertainers, interactions with local school children, and spiritual programming with chapel services. On-site clinical and personal services are another plus: podiatry, speech pathology, a beauty shop/hair salon (noted as available on Thursdays), rehab services, and a gym with equipment. Transportation to nearby stores (Wal‑Mart/Myer) and access to both assisted living and nursing home levels add to the convenience and continuity of care for residents. Maintenance responsiveness is also singled out—staff helped hang pictures and addressed issues promptly—which contributes to the impression of a well-run day-to-day operation.

Dining and communal spaces receive positive comments as well: reviewers mention good meals with a daily special menu and a pleasant dining room and kitchen environment. These recurring notes about food quality and atmosphere support the broader characterization of a facility that focuses on resident comfort and social engagement. Outdoor access and a generally safe, calm environment are additional factors that reviewers appreciate.

However, there are significant and recurring concerns related to facility condition and, more importantly, management and staffing. Several reviewers describe the building as older and cite humidity-related problems that make floors slippery; safety strip improvements were mentioned as planned but not yet fully resolved. A specific safety concern was raised about certain living areas not being part of a locked-down section, which prospective residents or families should confirm.

More troubling are multiple comments that care and services declined after an ownership/acquisition change. Reviewers report staff departures, fewer certified nursing assistants (CNAs), reduced amenities, lower morale, and longer wait times for assistance. These accounts suggest operational strain affecting service levels and resident experience. Financial and placement issues also appear: reviewers note Medicaid pays a set amount (implying limitations on what Medicaid covers) and there is uncertainty about private room availability. Taken together, these patterns point to risk that day-to-day positives (cleanliness, activities, caring staff) could be undermined if staffing and management issues persist.

In summary, Presence Sacred Heart Assisted Living is described by many reviewers as a clean, activity-rich facility with caring staff, good food, and useful on-site services (podiatry, salon, rehab). Those strengths create a generally pleasant resident experience and sense of safety. At the same time, multiple reviewers raise credible concerns about an aging building, specific safety hazards from humidity, and—most importantly—reports of declining care, staffing shortages, and amenity reductions following an acquisition. Prospective residents and families should weigh the strong, consistent praise for staff and programming against the reported operational and safety issues, and should ask current management about recent turnover, current CNA staffing levels, specific safety fixes (e.g., floor treatment and locked-area policies), and how Medicaid and private-room options are handled given the reviews’ concerns.

About Presence Sacred Heart Assisted Living

Presence Sacred Heart Assisted Living sits at 519 N Main St in Avilla, Indiana, and it's been in business for about 14 years, so folks around the area have come to know it as a place with several levels of care for seniors, all under one roof, which some people call a continuing care retirement community or CCRC, meaning you can move in while you're still pretty active, maybe choosing one of the apartments or duplex units, and then if you ever need more help, there are assisted living, skilled nursing, or even memory care options right there on the same grounds. The community provides both independent living and assisted living, so seniors can get as much or as little help as they need, and they won't have to move if their needs change. The staff, many inspired by Catholic roots through Provena Health and Ascension Living Mission, look after daily needs like meals, medication management, bathing, and getting dressed, while helping folks stay independent, which people seem to appreciate. There's round-the-clock nursing staff for medical needs, especially for those who need skilled care, rehabilitation, or even wound care, and there's help available for things like laundry and housekeeping too, so you don't have to worry about chores piling up. There's a special secure area with 24-hour staff for residents with Alzheimer's or dementia, where specialized memory care programs offer memory-enhancing activities and support that fits their needs. Residents get access to prepared meals each day, and there are social activities, educational opportunities, religious services, group outings, and scheduled transportation if you need to get around town. The place is designed to be safe, with comfortable apartments and common spaces, and the staff tries to keep everything clean and well-maintained. You'll find a range of health services, including hospice, podiatry care, therapies like physical and speech, respite care, and even emergency services when that kind of help is needed. With flexible floor plans, access to retreat stays, and attention to the mind, body, and spirit, the community offers a secure and comfortable choice for seniors who want to remain active and involved as they grow older, all at 519 N Main St, Avilla, Indiana.
