    Neglectful care inconsistent leadership unsafe

    I placed my dad in the rehab wing and was horrified by the care: catheter bags left full and near the wall, slow nurse response, basic hygiene neglected (dirty, stained sheets, unbathed, nails filthy), oxygen/dialysis/doctor visits not handled, and poor communication - POA and family weren't notified even after his decline and hospital transfer. Some staff - a caring custodian, a few nurses and therapists - were kind and helpful, and the grounds and common areas are clean and peaceful. But leadership, staffing, and safety are inconsistent, with reports of cover-ups and missing items; overall I would not recommend this facility.
