    Caring staff, excellent therapy, small

I placed my brother here and overall I'm pleased. The staff are friendly, attentive and compassionate, they keep me updated on his progress, and the skilled clinical team - exceptional PT, great OT and an amazing speech therapist (Joe) - has helped a lot. It's a small, family-like facility that's very clean and updated, accepts veterans/various insurances, but rooms are small/shared and outdoor space is limited. Meals are hit-or-miss and could use improvement, though a new administrator has already improved cleaning.

    Summary review

Overall sentiment across the reviews is positive, with recurring praise for the staff, rehabilitation services, and the facility’s small, family-like atmosphere. Multiple reviewers emphasize that caregivers are kind, compassionate and professional. Communication with families is highlighted as a strength: staff provide regular progress reports and are described as attentive and helpful. Several comments indicate that staff frequently go above and beyond, making both residents and family members feel supported. The presence of a full clinical team and a transitional program is noted, reinforcing that the facility provides skilled-level care appropriate for rehab and longer-term needs.

Rehab services receive particularly strong endorsements. Physical therapy, occupational therapy, and speech therapy are repeatedly singled out as excellent, with one reviewer naming an exceptional speech therapist (Joe) and others calling the therapists friendly and highly effective. These consistent remarks suggest the facility is especially strong in post-acute and restorative care, which would be important for families seeking intensive rehab services.

The facility’s physical environment is described mainly in favorable terms: indoor areas are clean, well-kept, and smell pleasant; furniture and fixtures have been updated; and grounds are described as beautiful. The small size of the facility contributes to a sense of closeness and family, which many reviewers appreciate. That said, the urban location creates some tension in the feedback: while the grounds are attractive, outdoor space is limited, and reviewers note an urban setting with restricted outdoor areas.

Space and accommodations are a consistent area of concern. Most rooms are average-sized and shared, and several reviewers point out that rooms are small for two people and offer limited storage for personal items. This practical limitation is important for potential residents who want space for belongings or private rooms. Cleanliness is generally praised, though one reviewer contrasted earlier issues with a report that cleaning has improved under a new administrator—this indicates past variability in housekeeping that appears to be addressed.

Dining receives mixed reviews. Many reviewers describe meals as average to above average, with generally good to very good options. However, at least one family member found the meals unappetizing and explicitly suggested the facility needs another cook. This points to inconsistency in culinary quality: while some residents and families are satisfied, others feel dining could be improved.

Activities and engagement are noted positively, with organized options such as bingo and live music helping to create an engaging environment for residents. The combination of therapeutic services, social activities, and attentive staff contributes to an overall impression of a facility that provides strong clinical care along with social engagement.

Management and administration show signs of responsiveness. Reviewers mentioned a new administrator and noted improvements in cleaning and room condition, suggesting leadership is making tangible improvements. The facility’s acceptance of veterans and various insurances is a practical advantage for prospective residents.

In summary, Warren Barr Lincoln Park is portrayed as a small, well-kept urban skilled nursing and rehab facility with a compassionate, professional staff and strong therapy programs. Its strengths are staff quality and communication, excellent rehabilitative services, a full clinical team, and a close-knit atmosphere. Areas to consider are room size and storage limitations, limited outdoor space typical of an urban site, and some inconsistency in dining quality. Families seeking strong clinical rehab care in a smaller, family-oriented setting are likely to view this facility favorably, while those prioritizing private, spacious accommodations or consistently high-quality dining may want to clarify those specifics during a tour or discussion with administration.

    About Warren Barr Lincoln Park

Warren Barr Lincoln Park works as a for-profit rehabilitation center with 109 certified beds and usually about 77 residents each day, and though they focus on providing person-centered care and recovery, people should know the center's had 37 deficiencies on inspection reports, with four infection-related, plus one each related to care planning and pharmacy service, showing the need for improvement in some areas. The facility uses modern technology and offers amenities designed for both comfort and patient recovery, so you'll find well-appointed guest rooms, private suites, and relaxing common spaces, and they do try to balance comfort and safety for everyone staying there. Affiliated with Legacy Healthcare, they work as part of a sister-center network, following set standards of care, and they offer services like post-surgical rehabilitation, short-term rehab, orthopedic rehab, wound care, cardiac programming, post-transplant care focused on liver and kidney, plus stroke and neurological rehabilitation, and their full-time therapy staff includes physical, occupational, and speech therapists who provide therapy in a medically supervised setting, even for infection prevention and graft function after transplants. There's licensed nursing care on site, with roles such as directors of nursing, RNs, LPNs, assistant directors of nursing, and restorative aides, but nurse turnover rates are higher than some would like, at about 64.2%, and the nursing staff gives around 3.61 nurse hours per resident per day. The building is set up with state-of-the-art physical therapy equipment throughout, a pharmacy service that should support recovery, dining and housekeeping to help keep things clean and comfortable, and amenities aimed at making recovery or a short stay easier on guests and visitors alike. While their focus is on high-quality transitional care in a boutique-style environment and the staff say they're dedicated and compassionate, the records show they need continued attention to recordkeeping and pharmacy services. Families will find physical, occupational, and speech therapy available every day, post-hospital rehabilitation programs, and care plans designed to manage follow-up needs, especially after major surgeries or transplants, with nurses and therapists focusing on professional, patient, and respectful care as best as possible.
