    Warm staff, unreliable care, unclean

I have mixed feelings. I appreciated the warm, caring staff (some RNs and PT were excellent), the active calendar, homey building, swing shuttle and church support-but care was spotty: call lights often ignored, slow responders, a resident was once left after a fall, odors, unclean sheets/floors/nails, inconsistent meals, billing screw-ups and safety concerns for memory patients. Because of those reliability and cleanliness issues, I can't confidently recommend it.

    Summary review

Overall sentiment across the reviews is mixed: several reviewers praise specific caregivers, therapy services, and certain amenities, while others report serious operational, cleanliness, and safety problems. Multiple accounts emphasize that high-quality, personal nursing care is available — particularly once nurses get to know a resident — and that the physical therapy team is excellent. Several reviewers also described staff as friendly, helpful, and understanding in many areas, and noted that the facility offers a homey atmosphere, plenty of activities, and a well-equipped space for church services. The swing-based transportation feature was specifically called out as a convenience.

However, these positives are tempered by recurring concerns about inconsistent staffing and responsiveness. Many reviewers described the staffing as spotty with mixed politeness and service levels: some staff are considerate and attentive, while others are indifferent. There are multiple reports of slow first-response times, ignored call lights, slow call-backs to family inquiries, and short staffing on weekends. These operational problems contributed to at least one report of a resident falling and being left stuck for a period, and there were explicit safety concerns raised for memory-care residents.

Cleanliness and hygiene are significant recurring negative themes. Reports include sheets left unchanged for a week, crumbs on floors, dirty nails, floors not clean overall, and even urine/feces odors in some areas. These issues point to inconsistent housekeeping and infection-control practices. Several reviewers also reported feeding and hydration problems for some residents, which compounds the safety and wellbeing concerns.

Dining and food quality are uneven and often criticized. Meals were described across the spectrum from fair to atrocious, with specific complaints about repetitive ingredients (peas mentioned frequently). While the dining area itself was called adequate, the food quality and variety appear to be a notable area for improvement.

Activities programming receives mixed feedback: some residents and families appreciate the abundance of activities and say residents are entertained and well cared for, while others noted an unclear activity calendar and frequent cancellations, which undermines the reliability of programming.

Administrative and management issues appear in several reviews. There are multiple mentions of billing and insurance errors or “screw-ups,” unclear or slow communication from staff, and facility decor and amenities that lag behind newer competitors. Navigation through parts of the building is sometimes difficult, which can affect ease of movement for residents and visitors.

In summary, Chris Jensen Health & Rehabilitation Center shows strengths in individualized nursing care (when continuity is achieved), strong physical therapy, active spiritual programming, and pockets of friendly, caring staff and activities. The facility’s weaknesses center on inconsistent staffing and responsiveness, cleanliness and hygiene lapses, food quality issues, activity scheduling reliability, and administrative errors. These patterns suggest that quality of experience may vary substantially depending on staffing levels, shift, and which staff members are on duty. Prospective residents and families should investigate current staffing ratios (especially on weekends and evenings), ask about housekeeping and infection-control protocols, verify fall-prevention and memory-care safety measures, review recent survey/inspection results, and clarify billing procedures and the activity schedule before deciding. If considering this facility, try to meet the primary nursing/therapy staff who would be assigned, and confirm concrete remedies the facility has in place for the consistently reported concerns (cleaning frequency, call-light response time, meal plans, and insurance/billing accuracy).

About Chris Jensen Health & Rehabilitation Center

Chris Jensen Health & Rehabilitation Center sits at 2501 Rice Lake Road in Duluth, Minnesota, offering skilled nursing and rehab care since 1969, and it has 170 certified beds with about 153 residents right now and isn't accepting new patients at this time, but it's got a bed occupancy rate of 90%, so you know there are usually folks around, and it's run as a for-profit corporation under the St. Louis County ownership, working with Medicare and Medicaid to provide different levels of care, though it's not a hospital, Special Focus Facility, or a Continuing Care Retirement Community. The building's fully sprinklered for fire safety, and it uses medication management systems along with 24-hour nursing supervision, with a staffing ratio of 0.84 RN hours, 0.61 LPN hours, and 2.46 CNA hours per resident, coming out to 1.45 total staff hours per resident which might be something people want to compare with other places, and the staff speak English with some also knowing other languages.

The center handles short-term recovery along with long-term stays, so people can come for wound care, subacute skilled nursing, neurorehabilitation, dementia care, hospice care, or even treatment programs for alcohol use or depressant substance abuse, which some places don't offer, and they also do things like pain management, psychiatric evaluations, vision and hearing screenings, and dental care, and they'll help with diet counseling and pain management if you need it. Rehab services here use their own "Rehabilitation Gym," "Wound Care Clinic," and "IV Therapy Suite," and there are both inpatient and outpatient rehab treatments with a full team including physical, speech, and occupational therapists. There's also respite care if folks only need temporary help, so family caregivers can get a break, and the place is set up with private and semi-private rooms and different communal activity areas so no one has to be stuck alone all day unless they want to.

Chris Jensen's health data shows some things stand out, so about 10% of short-stay residents reported moderate to severe pain, and 1% had delirium, while 76% got influenza shots and 64% got pneumococcal vaccines, with 9% experiencing weight loss and 3% having urinary tract infections; about 57% of low-risk residents lost bowel or bladder control, and 28% reported feeling more depressed or anxious, which can happen as people age, while 1% were physically restrained, 7% of high-risk and 1% of low-risk folks had pressure sores, and among long-stay residents, 11% needed more help with daily activities and 2% had moderate to severe pain, with 95% influenza and 87% pneumococcal vaccination rates. The center is required to tell residents, families, and doctors about any injuries, health changes, changes in treatment, or discharge right away, and it has data on health inspections, nursing staff, and quality measures ready for anyone to see.

Amenities include private and semi-private rooms, therapy gyms, and spaces for activities, and the facility makes use of newer technology for rehab equipment to help people recover their strength, and offers personalized treatment plans for different needs, like memory care for Alzheimer's or dementia, along with community service projects and recovery programs. The staff have a range of experience, and there are Resident and Family Councils for people who want a say in how things go, plus counselors and senior living resources for those needing help or advice. The admissions team is available by phone, though they're currently not accepting new residents, and anyone interested in the facility or needing to update their information can find resources and support. Chris Jensen Health & Rehabilitation Center aims to help people recover, regain strength, and manage health challenges with an experienced team in a warm, down-to-earth setting where both short-term and long-term needs are considered.
