    Clean facility with safety concerns

    I placed my mom here and overall I'm impressed: the newer facility is very clean, staff are warm, knowledgeable and responsive, food/activities (even an on-site ice cream parlor) and rehab/therapy are excellent, and administration/finance are professional and Medicaid-savvy. Downsides: rooms are small and often shared, the memory-care unit feels crowded with narrow hallways, parking/curb appeal is limited and it's on a busy street. There have been a few serious incidents (neglect/CNA verbal abuse and some hospitalizations), so I'd recommend visiting, asking about staffing and incident follow-up before deciding.
