    Poor care, neglect, infections, avoid

I moved my mother in hoping for better care, but promised changes never happened. Care was poor-long nurse-call waits, delayed pain meds, broken/missing wheelchairs and lifts, ineffective PT-and her health declined with infections and bedsores. The room was foul-smelling, dirty and cluttered in an outdated building with tiny shared bathrooms; meals were unappetizing, very small, sodium-heavy and the diet never changed. Some aides and cleaning staff were helpful and staff kept family informed with quick COVID tests and physician checkups, but overall it felt neglectful and not worth the money. I do not recommend this facility.

    Summary review

Overall sentiment across the reviews is mixed to strongly negative, with a substantial number of serious concerns about care quality and safety. While a minority of reviewers note positive experiences—helpful aides, friendly cleaning staff, routine physician checkups, in-house activities, and timely COVID test results—many more describe persistent problems that directly affect residents' health and comfort. Several reviews describe promises of improvement that were not followed through, contributing to distrust of management and staff.

Care quality and resident safety are the most frequent and serious themes. Multiple reviewers reported neglectful nursing practices, delayed or missed pain medication, and night staff failing to remain attentive. There are explicit reports of health decline while in the facility, including infections and development of pressure ulcers/bedsores. Long waits after pressing nurse-call buttons and allegations of a night nurse sleeping on duty underscore a pattern of slow or unreliable response to residents' needs. Equipment failures (broken or missing wheelchairs and lifts) and ineffective or minimal physical therapy were also cited; together these issues suggest that both daily care and rehabilitative services may be inconsistent or insufficient.

Staffing and communication present a mixed picture. On the positive side, reviewers mentioned that aides can be helpful and cleaning staff are pleasant; some families praised open communication and regular updates from the facility, including quick COVID test results and physician follow-ups. At the same time, reviewers described a lack of trust in staff competence and honesty, and noted a broad variation in staff quality. The presence of some communicative and caring employees does not appear to fully counterbalance reports of neglect and poor clinical practices.

Facilities and cleanliness comments are contradictory. A few reviewers noticed shiny floors and existing cleaning efforts, but there are stronger and repeated complaints about dirty, foul-smelling rooms and cluttered personal spaces. The physical plant is described as old and outdated, with small shared bathrooms and limited privacy. Equipment problems such as broken lifts and wheelchairs raise safety concerns and likely contribute to reduced mobility and increased risk for residents who need assistance.

Dining and nutrition were another common area of dissatisfaction. Several reviewers called the food unappetizing, noted very small portions, and described meals that were high in sodium or unchanged despite dietary needs. A couple of reviewers did report acceptable meals or simply that meals were provided, but the dominant impression is that food quality and portioning are inadequate for many residents.

Activities and rehabilitation are limited according to the reviews. While there are some in-house activities, many reviewers felt options were insufficient and that rehabilitative services (physical therapy) were ineffective or lacking, which is particularly relevant for residents admitted for recovery or improvement. The facility also experienced a COVID outbreak, which reviewers noted; the facility did provide quick testing and communication, but the outbreak itself is a concern.

Reputation and value concerns close the picture: reviewers described poor value for money and one review referenced an official low ranking (second-worst rating in the state). Taken together, the most frequent and serious issues—care neglect, medication delays, infections, pressure ulcers, equipment failures, poor food, and unclean rooms—outweigh the positive remarks for most reviewers. Families considering Lincoln County Nursing & Rehab should weigh the reported instances of helpful aides and open communication against repeated, specific accounts of clinical and operational failures. The pattern suggests the facility has some strong individual staff members and certain administrative communication practices, but persistent systemic problems that affect safety, hygiene, nutrition, and rehabilitation for many residents.

About Lincoln County Nursing & Rehab

Lincoln County Nursing & Rehab sits at 1145 East Cherry Street in Troy, Missouri, and offers skilled nursing, nursing home care, memory care, hospice care, and rehabilitation services for people who need help after a hospital stay, a serious illness, or surgery and you can find 90 beds there with private, semi-private, suite, and companion room layouts for residents to choose from, though as of June 2025, only 22 beds will be certified and there are about 77 people living there right now. The facility works to support residents' personal routines and gives each person help with daily needs like bathing, dressing, medication, moving around, and meals while making sure skilled nurses and staff are around all day and night for medical care, supervision, and emergencies, and when someone needs help with complex care like tracheostomies or physical, speech, or occupational therapy, those services are available, too. There's training for staff covering topics like Dementia & Alzheimer's care, Medicaid, and RAL courses, with state caregiver certifications, ongoing staff education, and policies to prevent abuse, plus careful screening for employee backgrounds and keeping surety bonds for resident funds, and though some staff files have missing background checks, the facility's supposed to check the Family Care Safety Registry and Nurse Assistant Registry before hiring.

Residents can relax in a place filled with homelike touches, private bathrooms, cable TV, kitchenettes, air conditioning, and high-speed internet, and rooms come in different sizes. For activities, there are arts and crafts rooms, a fitness room, a library, a spa with a sauna, outdoor gardens, walking paths, business and activity rooms, a movie theater, and common spaces where people gather, as well as programs like movie nights, music, outdoor programs, and resident-led events. The dining room offers meals made by a professional chef with options for people who have diabetes or food allergies, and a dietitian oversees nutrition. Transportation helps get residents out and about, and there's parking for visitors.

Housekeeping, laundry, move-in help, spiritual and family support programs, and salon services are all available, and nurses, a nutrition specialist, and therapy staff provide hands-on support. The community follows strict policies for medical orders, keeping careful records, and giving proper tracheostomy and infection care, and it accepts both Medicare and Medicaid. Lincoln County Nursing & Rehab's ownership is for-profit, it's not a CCRC (Continuing Care Retirement Community), and though the building is large, the staff aims to keep things homelike and respectful of each resident's needs, abilities, and wishes when planning daily life, so people can feel safe, cared for, and part of a community, even when they're facing serious health concerns.
