    Clean facility; inconsistent, sometimes neglectful

I found the facility clean, remodeled and secure with spacious walkable grounds, a lovely courtyard and waterfall, good therapy, many activities (crochet, church, library) and mostly consistent, restaurant-style meals and attentive, friendly staff. However, chronic understaffing and poor communication caused abrupt, sometimes neglectful care - missed baths, soiled sheets, room moves without notice, unanswered calls, a fever left untreated and confusing discharge/release instructions. Some staff and management were kind and responsive, but the inconsistent care and safety lapses leave me hesitant to recommend it.

    Summary review

Overall sentiment is mixed with a distinct split between many positive experiences and a set of serious operational and care concerns. Several reviewers praise the facility for its rehab and therapy services, clean and recently remodeled spaces, attractive outdoor areas (enclosed garden, waterfall, courtyard), and a generally welcoming, homey atmosphere. Multiple comments highlight responsive and courteous staff, helpful administrative attention, a smooth admission process, and a strong activity calendar that includes crochet, bridge classes, workouts, and group therapy. Dining receives generally positive notes as well: many reviewers report consistent, on-time meals, restaurant-style or dignity dining areas, and even delicious meals for some residents. The facility also offers practical amenities such as a library, church service, beauty salon, and orderlies, and several reviewers specifically call the rehab program strong or outstanding.

Care quality and staffing present the most significant theme of concern. While many reviewers describe staff as attentive and caring, an equal number or more report staffing shortages, inconsistent caregiver behavior, and serious care lapses. Complaints include bathing neglect, failure to follow physician orders, soiled sheets and clothing, missing personal items, and shift-change lapses. There are alarming accounts of unresponsiveness—particularly on weekends—phone calls going unanswered, delayed medical attention (an example given was a patient left with a fever for an extended period), and even a police welfare check. These reports suggest variability in day-to-day care that prospective residents and family members should investigate carefully.

Management and communication show a similar mixed pattern. Some reviewers praise receptive management and quick office attention, but others recount poor communication around logistics: room moves without notice, missing or incomplete discharge/release instructions, and confusion about release dates. Operational issues also include maintenance and comfort problems such as no hot water, nonworking TVs, laundry errors, and small room sizes. Food is another area of divided opinion: although many praise consistent and tasty meals (and some report unwanted weight gain), some reviewers feel food quality could be improved. Cost is noted as a downside by some reviewers.

Facilities and activities are generally strong selling points: the property is described as clean, remodeled, well-kept, and secure, with ample common spaces and structured activities that encourage resident engagement. The therapy department and rehab services receive multiple positive mentions, making the facility attractive for short-term rehab stays. However, recurring operational and staffing concerns — especially those that impact direct resident care and safety — temper those positives.

In summary, Grace Living Center-Bethany shows many strengths: clean, updated facilities; good therapy and rehab programming; varied activities; pleasant outdoor spaces; and numerous reports of caring, courteous staff and good dining. At the same time, there are repeated and sometimes serious complaints about understaffing, inconsistent care, communication failures, logistical errors (room moves, discharge confusion), and a few maintenance/service shortfalls. These patterns suggest the facility can provide an excellent experience when staffing and communication align, but there is variability in performance that warrants careful inquiry. Prospective residents or families should ask specific questions about current staffing levels (including weekend coverage), discharge and communication procedures, laundry and personal-item policies, and room size/amenities during tours and before admission.

    About Grace Living Center-Bethany

Grace Living Center-Bethany sits at 7000 Northwest 32nd Street in Bethany, Oklahoma, and folks living here can find everything from independent living to assisted care, memory care, and skilled nursing inside one place, which is handy if needs change. The building's got 161 certified beds, though only about 97 folks lived there as of June 2025, so there's still space, and rooms come fully furnished with private bathrooms, air conditioning, cable TV, Wi-Fi, kitchenettes, and phone lines, which helps some feel at home quicker, and each room has an emergency alert for safety. Staff support residents with basic needs like bathing, dressing, moving, meals, medication, and special diets, like for folks with diabetes, and you can see nurses from 12 to 16 hours a day, with help always around the clock. There's a daily schedule to cover care, meals, and activities, and the community puts together things like movie nights, day trips, exercise programs, arts, and crafts, plus some faith-based events and reading, and residents can even run their own activities. There's a garden with walking paths, a sauna, hot tub, beauty parlor, library, fitness and game rooms, computer center, and shared dining room where a chef organizes meals, including for allergies and other needs. For those who can't get out on their own, transportation for doctor visits, errands, and other outings is arranged, while housekeeping, laundry, and even dry cleaning are covered in the services as well. The memory care part stays secure with activities and programs for folks with Alzheimer's or dementia, and a staff always keeps an eye out to prevent wandering. Residents can also get therapy services like physical, occupational, and speech therapy, and therapists aim to help folks bounce back from injuries or health problems, focusing on regaining movement and self-sufficiency. Grace Living Center-Bethany accepts Medicare and Medicaid and is set up as a for-profit partnership, not owned by a large organization, and it's not in a hospital. The staff tracks regular vaccinations and has programs to help prevent infections, plus there are councils for residents and families to share concerns or talk things through. You'll see both Mr. Joe Elbow, Associate, and Ms. Jessica Bieger, Administrator, on the management team, while Erin Davis leads as Administrator with a focus on long-term care. The whole place works to meet up-to-date care standards and provides day-to-day help for all kinds of needs, not pushing itself as the fanciest spot around, just set up to cover a lot of bases for people and families who need real support, and if anyone wants to know more, they can look up more details at gracelivingcenters.com.
