    Clean, friendly staff, smooth rehab

I stayed here for a short rehab stay and was very pleased. The facility is clean and nearly new, staff are friendly, caring and helpful, and the nurses and therapy team were fantastic and communicative. Admission-to-discharge was smooth, the dining and activity spaces are pleasant (daily calendar, exercise room, bingo), and they were helpful with Medicaid info. I would highly recommend this place.

    Summary of reviews

The reviews present a mixed but coherent portrait: the facility is new, well maintained, and staffed by many clinicians and aides who are described as friendly and knowledgeable, with a particularly strong rehabilitation program. Physical and occupational therapy receive consistent praise for effectiveness and communication, and many families report smooth transitions from admission to discharge. The buildingโ€™s finishes, housekeeping, large dining rooms, exercise facilities, and proximity to hospital and EMS are repeatedly noted as strengths.

At the same time, persistent operational weaknesses are a recurring theme. Staffing levelsโ€”especially overnight and on certain shiftsโ€”are frequently characterized as inadequate, and the facility appears to rely on contract or agency personnel in ways that reduce continuity of care. Several reviews point to gaps in clinical leadership and oversight, which reviewers connected to inconsistent medication administration and monitoring, delays in responding to calls for assistance, and uneven attention to personal-care tasks such as bathing.

Sanitation and incontinence-care concerns are also raised alongside otherwise clean common areas; reviewers describe isolated odor or cleanliness issues in some dining and communal spaces. Activity programming is available (exercise, bingo, daily calendar items), but several families found offerings limited or lacking a dedicated activities director, resulting in variable engagement for longer-stay residents. Dining impressions vary: the facility provides regular meals and housekeeping, and some residents enjoy the food, while others find choices limited or the menu too health-focused for individual taste.

Management and communication present a contrast: case managers and many frontline staff communicate well and are described as helpful during admissions and therapy. However, administrative processes are characterized as bureaucratic at times, making it harder for families to obtain information quickly. A small number of more serious allegations were noted by reviewers, including a request for licensure action against an LVN; these items stand apart from the general operational themes and may warrant direct follow-up with the provider and regulators. Overall, the facility appears well suited for short-term rehabilitation needs, with strengths in therapy and facility upkeep, while families considering longer stays or residents with complex clinical or memory-care needs should clarify staffing patterns, clinical oversight, and activity programming before committing.

    About Cibolo Creek

Cibolo Creek Health and Rehabilitation Center sits at 1440 River Rd, Boerne, TX 78006, and it does a good job offering skilled nursing and both short-term and traditional care to folks who need a safe place to heal or live for longer stretches, with services like interdisciplinary care that brings together different healthcare professionals who try to coordinate things for better results, and they've got this team approach to transitional care, rehabilitation care including physical, occupational, and speech therapy, as well as chronic care for those dealing with ongoing conditions, so you'll see them working hard to help with wound care and hospice or respite care too, which covers end-of-life care and short-term breaks for families. People get access to registered dieticians who make sure meals and nutrition fit each person's health needs, and the staff understands a lot about how important kindness and comfort are, trying to help folks feel at home with friendly faces and a focus on shared memories. The place gets involved in the local area through outreach and volunteer programs, so residents stay connected to the Boerne community. If anybody has a concern, the long-term care ombudsman is there to help fix problems quietly and at no cost. The center focuses on keeping things as welcoming and warm as possible, with staff helping people through everyday needs and recovery, and they do what they can to make life a bit easier for residents and their families.
