    Clean facility, inconsistent care, communication

    I appreciated the clean, spacious facility, great PT and engaging activities, but overall I'm dissatisfied: clothes went missing, phone communication and nurse contact were poor, care was inconsistent (falls, scratches, long fingernails), food was often too salty or inadequate so we brought pureed meals, and the laundry irritated sensitive skin.
