    Neglect mismanagement abuse fatal outcome

    I placed my loved one in this large, older facility that advertises PT/OT, podiatry, dentistry, psychology and eye care - but the care was poor and staff were often unresponsive or mean (a few were helpful). Communication and management are chaotic: a wheelchair disappeared and we were billed for it, medications were delayed, rooms were dirty (sticky floors, clothing in plastic bags), catheter supplies outdated, and mismanagement led to bedsores and surgery. My loved one fell, suffered a skull fracture and brain bleed, and later passed away here - we are still seeking answers. Food was good under Woodstock but went downhill under Kindred (PB&J-level meals). They promise new management and updates, but I cannot recommend this facility.
