    Troubling neglect, some improvements noted

I had a mixed but ultimately troubling experience. My mom developed scabies, worsening bedsores and inadequate wound/respiratory care while under what felt like unqualified, overwhelmed staff-night shortages and what I viewed as money-driven, ethically questionable decisions preceded her death. Since then a new Administrator and DON, daily APRN rounds and weekly Medical Director oversight have improved staff ratios; the therapy team is excellent, meals have gotten better, and many nurses and aides are compassionate and helpful. Persistent problems remain - cleanliness, poor aseptic technique, shared bathrooms, no audible call lights, and occasional unresponsiveness at night. I appreciate the caring staff I met, but I cannot fully recommend this facility.

    Summary review

Overall sentiment in these reviews is mixed but highlights two clear and recurring patterns: (1) rehabilitation and therapy services, along with some recent management and clinical oversight changes, are frequently praised; and (2) core nursing, safety, infection control, and night-shift staffing issues are repeatedly reported and — in several cases — described as causing serious harm. The facility elicits strongly polarized reactions from family members and residents, with many describing caring, competent staff and good therapy outcomes, while others report severe neglect, safety hazards, and even allegations of abuse or avoidable death.

Care quality: Reviews indicate a wide variance in clinical care. Many reviewers praise the therapy team as “wonderful” or “excellent,” with good rehab outcomes and some residents stating they would use the facility again for rehab. At the same time, there are multiple, specific complaints about nursing care: poor pain management; inadequate wound care (notably elbow dressing issues and worsening bedsores); postoperative complications alleged to require further surgery; refusal or failure to maintain hydration and meals in some cases; and reports of scabies or other infection concerns. Several families recount events they believe contributed to severe deterioration or death, which raises serious concerns about clinical competency and oversight in specific cases. These reports suggest inconsistent standards of clinical care across shifts, units, or time periods.

Staffing and responsiveness: A frequent and serious theme is insufficient staffing at night and during busy times. Multiple summaries reference not enough staff to cover multiple halls at night, staff who are overwhelmed, and instances where residents must yell for assistance because call lights are not audible or ignored. Conversely, other reviewers highlight compassionate, attentive nurses and CNAs and note that many staff are hardworking and caring. This split suggests variability: some shifts or units appear well-staffed and responsive, while others are understaffed and unresponsive.

Safety, infection control, and facilities: Several reviews cite safety hazards and poor facility practices. Reports include unsafe bathroom configurations or fixtures, lack of audible call light systems, inadequate aseptic technique, cross-contamination, and insufficient sterilization practices (e.g., cups washed only once). The absence of respiratory support devices such as BiPAP/Trilogy and related billing or administrative issues for such equipment were also noted — a significant clinical and safety concern for residents who require respiratory support. These complaints point to both physical environment problems and procedural lapses in infection control and equipment availability.

Management and clinical oversight: Some reviewers note positive changes in leadership and oversight: a new Administrator and Director of Nursing, daily APRN rounds, weekly Medical Director presence, and the ability to contact the Medical Director in emergencies. Several families credit these changes with improved compliance with OLTC regulations and an overall improvement in meals and responsiveness. Other reviews, however, allege that staff or leadership are “money-focused,” prolong life support for billing reasons, or otherwise act unethically. The existence of both strong praise for recent leadership and severe accusations of mismanagement suggests that improvements may be recent or incomplete, and perceptions vary widely by family experience.

Dining and activities: Dining services are reported as inconsistent but showing improvement. Some reviewers specifically say dietary services improved toward the end of stays. Conversely, others report inadequate meals or refusal to refill water. Activities such as bingo and picnics are available and contribute to a “small, homey” feel; however, memory-impaired residents may be unable to fully participate, which is expected but worth noting for families seeking memory-care appropriate programming.

Overall patterns and takeaways: The reviews portray a facility with clear strengths in therapy and, according to multiple accounts, a compassionate core of staff and positive recent administrative changes. However, there are repeated and serious complaints about nursing care, night staffing, infection control, responsiveness to calls, wound care, equipment availability, and a few grave allegations concerning neglect and resident deaths. The picture is one of high variability: some residents receive excellent, compassionate care and achieve good outcomes, while others appear to experience significant lapses that have serious consequences. Prospective residents and families should weigh the facility's strong rehab reputation and recent leadership changes against the recurring reports of clinical and safety issues, and should ask targeted questions about night staffing ratios, infection-control protocols, wound-care procedures, availability of respiratory equipment, call-light systems, and recent quality-improvement outcomes or survey results before making decisions.
