    Neglectful care despite compassionate staff

My experience was mixed but mainly negative. Call lights were ignored for 30-40 minutes, needed treatment supplies were missing, nurses rarely rounded, and my family had to provide items and even perform trach care; CNAs sometimes left residents on the toilet and maintenance failed to fix broken showers, toilets, and furniture. Food was poor and the facility felt rundown with staffing shortages and worrying reports about inspections and licensing. On the positive side, several caregivers-notably Quita, Brandi, and the rehab team-were compassionate, attentive, and went above and beyond, and activities/therapy were good. Overall I would not trust this place without constant oversight.

    Summary review

Overall sentiment: Reviews for The Blossoms at Midtown Rehab & Nursing Center are strongly polarized. A substantial portion of reviewers praise specific caregivers and therapy staff, calling some employees compassionate, professional, and attentive. At the same time a large body of reviews alleges serious neglect, abusive behavior, medication errors, safety problems, and management failures. The result is a mixed but highly divergent portrait: excellent care experiences reported by some families and residents versus disturbing claims of neglect, harm, and administrative dysfunction reported by others.

Care quality and clinical concerns: One of the most frequent and serious themes involves clinical care failures. Multiple reviewers allege medication errors, unconsented medication administration, and at least one report of medication-induced catatonia leading to hospitalization. Wound care neglect, delays in pain medication, and insufficient nursing assessments were also cited. Many reports describe infrequent nurse rounds and long call-light response times (30–40 minutes), with some reviewers claiming call lights were cut off entirely. Several families say they had to provide supplies and perform tracheostomy care themselves because nursing staff did not. Conversely, some reviewers explicitly praise individual nurses and IV/wound care performed by named staff (e.g., “JJ”) as trustworthy and outstanding. This creates a pattern of inconsistent clinical quality that appears to depend heavily on which specific staff are on duty.

Staff behavior and staffing levels: Staffing and staff behavior are recurring, polarizing issues. Numerous reviews accuse CNAs and other staff of neglectful or abusive behavior — leaving residents in soiled conditions, verbal threats, scolding patients for needing assistance, and clocking out while leaving residents unattended. Other reviews counter that many CNAs and nurses are “sweet, loving, and attentive.” Staffing shortages and high turnover are repeatedly mentioned as underlying causes of poor care: families report there are not enough nurses, leading to patients left in bed pans, missed showers, and delayed responses. Several reviewers describe management as unhelpful or abusive, with isolated reports of investigations, staff suspensions, and even claims about a revoked administrator license. Allegations of review manipulation and attempts to bury negative feedback were also raised by multiple posters.

Safety and environment: Safety concerns are substantial in some reviews: reports of multiple falls, bruises, being left on toilets for hours, and general statements that the facility is “not safe” or “not suitable” for loved ones. Environmental hygiene complaints include feces found on the floor near a bed, used gloves left out, roach sightings, and persistent foul smells. Maintenance issues are common: broken showers, stopped-up toilets, broken furniture, and air conditioning problems were cited. While some reviews state the facility is clean and homelike, many others describe a run-down, old building with damaged carpets, trashed rooms, and insufficient common areas.

Rehab, activities, and amenities: Rehab and therapy staff receive frequent praise; several reviewers describe excellent therapy programs, a large therapy room, and effective rehabilitation outcomes. Activities are described inconsistently: multiple positive comments mention Bingo, movies, life-story sessions, and lobby events a few times a week. At the same time, other reviewers say there are few activities and that TVs are loud all day. Dining reports are mixed as well: some families praise nutritious meals and dietary accommodations, while others call the food awful or say the kitchen ignored allergy information. Physical amenities like large patient rooms and proximity to hospitals were noted positively by some reviewers.

Administration, communication, and reputation issues: Communication problems recur: families report difficulty reaching staff by phone or receiving timely updates. Administrative responsiveness is split in the reviews; while some praise particular administrators as supportive and visionary, multiple others describe administration as worthless, abusive, or actively hostile to complaints. Several serious allegations extend to regulatory and legal matters — failed inspections, staff suspensions, revoked licenses, and mentions of class action activity — which have contributed to mistrust among reviewers. There are also accusations that management may have changed the facility name and attempted to manipulate or suppress negative reviews.

Patterns and takeaways: The reviews suggest wide variability in experience that appears to depend strongly on staff on duty, unit, and time. Positive reports consistently highlight compassionate frontline staff (CNAs, specific nurses, and rehab therapists) who go above and beyond, provide excellent rehab outcomes, and create a family-like environment. Negative reports focus on systemic issues: inadequate staffing, poor management, safety risks, neglectful behavior, medication and clinical lapses, and facility maintenance/hygiene problems. Several allegations are severe (hospitalization after medication reaction, feces in rooms, threats of harm), which should be taken seriously by prospective families and by regulators.

Recommendation for prospective families: Given the starkly mixed feedback, anyone considering The Blossoms at Midtown should perform careful, targeted due diligence. Visit in person multiple times (day/evening), ask about staffing ratios and shift coverage, observe call-light response and mealtimes, request recent inspection reports and incident histories, meet specific care staff who would be assigned to the resident, and verify how medication and wound care are managed. Pay special attention to the unit you would be placed in (cleanliness, odors, maintenance) and seek references from recent families on the same unit. The reviews show both exemplary caregivers and serious, systemic problems — the difference appears to be situational and substantial, so an individualized assessment is crucial.

About The Blossoms at Midtown Rehab & Nursing Center

The Blossoms at Midtown Rehab & Nursing Center offers skilled nursing and intermediate care for people who are very frail and need help from nurses every day, and they've got space for up to 154 residents in certified beds but usually see about 86 residents on any given day, and they're part of The Blossoms Nursing And Rehab Center group with a few locations in the area. They often take care of folks who need short-term rehab services, like when someone leaves the hospital but isn't quite ready for home, and their Rehab-to-Home program helps residents get back to daily routines. They try to create a place where people can stay with dignity and respect whether they're staying a short or long time, and you find a wide variety of health care services with a focus on personalized, patient-centered care, paying attention to both physical needs and emotional wellbeing, and there are also recreational activities and amenities meant to help residents enjoy life as much as possible.

The nurse staffing averages about 3.75 hours for each resident each day, which falls below the state average of 4.1 hours, and the nurse turnover rate is pretty high at 66.0%, compared to the state average of 50.9%. The facility's inspection history shows they had 26 documented deficiencies, including ones about infection control-at least one infection-related deficiency was noted in a recent May 8, 2024 inspection, where nine total deficiencies were found-and there are also issues related to food, like not always getting food from approved sources or making sure menus meet all the nutritional needs, and other deficiencies have affected residents' quality of life and care. It's clear that The Blossoms at Midtown focuses on building a personal relationship between caregivers and residents, with a disciplined and compassionate approach, but families should know about the reported staffing and inspection details when considering it.
