Eureka Rehabilitation & Wellness Center sits on 23rd Street in Eureka, right in a two-story building with room for up to 99 people, and they have both short-term and long-term care programs for folks who need extra help, so you'll find services like 24-hour nursing care from skilled staff who can help with everyday things like eating and getting dressed, plus physical therapy, occupational therapy, speech therapy, and even respiratory therapy, which can make a difference for people dealing with breathing troubles. The team there includes kind professionals who work around the clock, always aiming to keep folks safe and well, and there's a real sense of community among residents, which helps make the place feel friendly. They also have special units-like a neonatal, pediatric, medical surgical, burn, and cardiac ICU-along with nursing and rehabilitative care for people with chronic illness or complex needs, even wound care and pain management, so they try to support people the best they can no matter what stage of life they're in. You'll find dietary, religious, and social programs, and there's a strong partnership with a local hospice agency for those needing extra support near the end of life, helping both the patient and family with spiritual, emotional, and medical care. The center's also paying attention to Covid-19 with ongoing health precautions for safety. People can use Medicaid to pay for services, which helps many families. Compassionate staff, systemic staffing failures
I had a mixed experience. The facility is very clean, the patio/flowers and activities are lovely, and many CNAs, nurses and therapists (especially LVN Susan) were compassionate, hardworking and helpful - staff treated my loved one with kindness and we were allowed a goodbye visit. However chronic understaffing, poor responsiveness (phones often unanswered), miscommunication and delays in care led to missed monitoring, bedsores, slow repairs/transport, and even a resident death; safety felt compromised with staff often powerless. I appreciate the caring direct caregivers but cannot fully recommend the facility until systemic staffing and administrative issues are fixed.
